IOP measurements with the ORA are significantly higher than DCT or GAT with no apparent linear correction factor. To date, we cannot estimate the impact of differences in CH in relation to IOP and its measurement.
Surgically induced changes of the cornea may not only be characterized by anatomic parameters such as corneal thickness but also by biomechanical parameters such as corneal hysteresis.
Fragestellung: Durch den Clear Cornea Zugang bei der Kataraktoperation werden viskoelastische Eigenschaften der Hornhaut verändert. Diese Veränderungen sollen durch die Messung der Kornealen Hysterese sowie des Augeninnendruckes durch Non-Contact-Tonometrie (NCT-IOP) und der zentralen Hornhautdicke (CCT) untersucht werden. Methodik: Mit dem Ocular Response Analyzer (ORA) wird präoperativ und am ersten postoperativen Tag die Korneale Hysterese (CH), der Augeninnendruck (NCT-IOP) sowie auch die CCT (US-Pachymetrie) bei 76 Augen gemessen. Ergebnisse: Die durchschnittliche Korneale Hysterese betrug präoperativ 10,3 ± 2,3 mmHg und postoperativ 9,31 ± 2,7 mmHg (p < 0,01). Der NCT-IOP betrug präoperativ 17,4 ± 3,4 mmHg und postoperativ 20,3 ± 6,7 mmHg (p < 0,01), die CCT stieg von 555,0 ± 31,7 µm präoperativ auf postoperativ 578,8 ± 41,7 µm (p < 0,01). Schlussfolgerung: Am ersten postoperativen Tag nach Clear Cornea Kataraktoperation ist bei signifikant dickerer Hornhaut die Korneale Hysterese reduziert. Dies deutet auf eine geminderte Dämpfungskapazität der Hornhaut direkt postoperativ hin.
