La prévalence de la maladie d’Alzheimer et des syndromes apparentés allant croissant au sein d’une population vieillissante, nombre de recherches scientifiques trouvent place dans la littérature, témoignant d’un véritable enjeu sociétal. Mais qu’en est-il du sujet dont la psyché s’égare dans le désordre démentiel ? À partir de la rencontre et d’extraits d’entretiens cliniques avec une patiente atteinte d’un syndrome démentiel à un stade très avancé et vivant dans un lieu d’hébergement pour personnes âgées dépendantes, les auteurs interrogent la question de la temporalité, dans son intime nouage avec l’identité et la subjectivité. Qu’en est-il du vécu du temps présent lorsque la mémoire du passé s’efface et lorsque la capacité à anticiper le futur s’amenuise ? Lorsque le présent ne se soutient plus des temps qui le bornent et lui donnent corps, le vécu du présent semble envahi par l’angoisse et le sentiment de vide. Cet article propose d’en penser quelques aspects.
La rencontre clinique des personnes âgées s’inscrit dans une démarche de soin éthique. Rester ouvert auprès de la personne dite démente offre la possibilité de la rencontrer dans sa singularité, depuis son monde et sa temporalité. Une telle quête métapsychologique laisse sourdre une résistance à l’effacement insoupçonnée, par un désir d’ipséité – compris comme le fondement de l’identité. Auprès de trois patients âgés nous éprouvons le rôle du silence, de l’écoute et de la parole, pour prendre le temps de rendre possible un entre , où une rencontre advient au possible. À partir d’un éclairage phénoménologique et psychanalytique, notre réflexion interroge la manière dont l’intime peut être déployé sans trop le trahir, mutatis mutandis comment la mise à l’ombre sociétale des personnes âgées sous le dictat de la honte, de la peur et du dégoût, au sein de l’espace intimiste de l’ ehpad , peut se transformer.
La rencontre avec les patients souffrant d’anorexie génère un écho singulier. Face à des sujets venant incarner le mort-vivant en donnant à voir un corps rachitique, les enjeux de vie et de mort sont d’emblée présents, non seulement sur la scène transférentielle mais également dans le réel de la rencontre. A travers le cas clinique d’une jeune femme anorexique, déployé à partir d’une séance de psychothérapie, les auteurs montrent comment, depuis l’entrelacs des mouvements transférentiels et contre-transférentiels, va s’interroger ce qui se joue et se rejoue, dans l’entrecroisement de l’Autre et de soi, afin de proposer une lecture psychopathologique à ce qui peut apparaître comme un « paradigme anorexique ».
Apparaissant à la fois comme une résistante au consumérisme ambiant et comme la figure prototypique du capitalisme actuel, l’anorexique incarne un mode aporétique de l’être. De plus, se caractérisant à partir de l’effet qu’elle suscite chez l’Autre, elle est une pathologie de l’intersubjectivité. Prenant acte de ce double trait, nous nous proposons d’en tirer les conséquences pour une psychopathologie de l’anorexie, puis d’esquisser à partir de son acte une théorie de l’action politique.
