Corticotomy facilitated orthodontics has been practising for longer periods, but a novel approach of a periapical incision to make horizontal cuts 5 mm above the apex of the maxillary anteriors along with vertical cuts requires a detailed investigation. The purpose of this study was to assess the outcome of skeletal cases with spacing in the anteriors using a novel method of modified surgical approach utilizing the corticotomy facilitated orthodontics. Patients for this study were randomly selected from a very intensive screening in the Department of Orthodontics, Saveetha Dental College and Hospital, Chennai, out of which seven patients were selected for this procedure. The patients were subjected for corticotomy procedure under local anesthesia after the leveling and aligning stage (0.019" × 0.025" stainless steel wire), and the retraction was continued still the space was closed. AutoCAD and palatal rugae assessment programs were used to assess the orthodontic tooth movement. The results of the study demonstrated that when compared to conventional orthodontics alone, the corticotomy facilitated approach produced faster tooth movements in all the seven patients reducing the overall treatment time by 60%. The use of this modified technique of labial, palatal and apical cuts could be beneficial for adult patients with difficult surgical access from the lingual side.
Aim:The aim of this study is to evaluate the efficiency of canine distraction and also to evaluate the position of canine in the arch after distraction.
Materials and methods:The study sample consisted of five patients, (4 males and 1 female), in the age group of 14 to 25 years who needed canine retraction (ten maxillary canines including both left and right) and first premolar extraction as their treatment protocol were included in the study. This study used a in-house manufactured periodontal ligament distractor.
Results:The maxillary canines were distracted distally by 6.42 mm in three and half weeks with a distal tipping of around 15.1° and the maxillary first molars tipped mesially by an average of 6.1°. The maxillary canines rotated mesiobucally by an average of 7.8° bilaterally.
Conclusion:Canines can be distracted rapidly with minimal effects on the vitality and the periodontal condition and almost all of extraction space can be used for anterior dental alignment or retraction. Maxillary canines can be rapidly retracted into the first premolar extraction space at the rate of about 2.43 mm per week.Clinical significance: The overall treatment time is reduced as the time taken for retraction of canine is decreased considerably from 6 and 8 months to 3 weeks. Also there is no significant anchorage loss during this period and is well suitable for cases with group a anchorage where the whole extraction space is needed for retraction of anterior teeth.
Background: Oral submucous fibrosis (OSMF) is graded according to various histological factors which include the epithelial changes and the connective tissue changes. These features could be identified in routine Hematoxylin and Eosin (H and E) staining in shades of pink. However, the use of special stains provides a contrast to various connective tissue components thereby aiding in better visualization of these connective tissue changes in advanced OSMF cases. Objective: To compare and evaluate muscle involvement and degeneration in advanced oral submucous fibrosis using three different stains namely, H&E, Van Gieson, and Masson’s Trichrome. Material and Methods: 10 Formalin-fixed paraffin-embedded tissue sections of advanced OSMFwere stained using 3 different stains namely Hematoxylin and Eosin (H&E), Van-Gieson, and Masson trichrome. The results obtained were tabulated and statistically analyzed using Kruskal Wallis ANOVA. Results: The hyalinization and fibrosis involving the skeletal muscle were better visualized in Masson’s Trichrome but were not statistically significant. The muscle degeneration in deeper areas was better visualized in Masson’s trichrome when compared to the H&E and Van Gieson. Conclusion: Hematoxylin and eosin stains all the connective tissue components in various shades of pink, use of special stains bestows contrast between different components of connective tissue, thus improvising grading of OSMF. Masson’s trichrome stain can be used as a single adjunct to hematoxylin and eosin stain as changes in the superficial and deeper connective tissue could be ascertained.
