To aid risk assessment, management, and treatment planning it is essential to assess child sexual abusers’ deviant sexual interests (DSI) and preferences (DSP) for sex with children. However, measurement of DSI/DSP is fraught with psychometric problems. In consequence, research interest has shifted to latency-based indirect measures as a measurement approach to complement self-report and physiological assessment. Utilizing the Explicit and Implicit Sexual Interest Profile (EISIP)—a multimethod approach consisting of self-report, viewing time, and Implicit Association Test (IAT) DSI/DSP measures—we replicated phallometric DSI/DSP differences between child sexual abuser subgroups in a sample of intrafamilial, extrafamilial, and child pornography offenders. DSI/DSP was associated with recidivism risk, offense-behavioral measures of pedophilic interest, and sexual fantasizing. It also negatively correlated with antisociality. Distinguishing between child sexual abuser subtypes and being related to recidivism risk, the EISIP is a useful tool for sexual offender assessments.

Toen ik in 1990 ging werken op het enige gespecialiseerde ambulante centrum voor behandeling van seksuele delinquenten in Belgie¨1, golden plegers van zedenfeiten als onbehandelbaar. Er kwamen vragen om behandeling uit het hele land en plegers waren bereid grote afstanden af te leggen, want dichter bij huis was er toch geen alternatief te vinden. Een clie¨nt uit WestVlaanderen of Limburg die wekelijks voor therapie naar Brussel kwam (enkele rit 100 km), was geen uitzondering. Lokale diensten geestelijke gezondheidszorg (vergelijkbaar met een RIAGG in Nederland) hadden geen aanbod voor deze mensen, 'want ze waren niet gemotiveerd'. Deze laatste zin kan op twee manieren gelezen worden: waren de clie¨nten niet gemotiveerd of waren de diensten dat niet? Elders beschreef ik twee behandelconcepten met betrekking tot clie¨ntmotivatie. 2Deze twee concepten vormen een continuu¨m en de therapeut bevindt zich ergens tussen de twee uitersten. Ofwel gaat de therapeut van zijn therapeutisch instrumentarium uit en definieert hij van daaruit een passende clie¨ntmotivatie. Behandelmotivatie is dan een voorwaarde om in aanmerking te komen voor therapie. Ofwel peilt de therapeut de motivatie die bij de clie¨nt in wat voor vorm dan ook aanwezig is en probeert hij erbij aan te sluiten. Behandelmotivatie is dan een thema en werkpunt in de therapie. Forensische clie¨nten die zich omwille van een opgelegde maatregel voor therapie aanmelden, passen niet in het eerste model. Het is aan de dadertherapeut om de clie¨nt af te halen op het punt waar Justitie hem aan de hulpverlening overdraagt. Vanuit deze tweede visie is de vraag legitiem of een therapeut wel voldoende gemotiveerd is om met forensische clie¨nten te werken. In dit artikel zal ik nader ingaan op de achtergronden en werkwijze van deze tweede visie, waarna een en ander wordt geı¨llustreerd aan de hand van drie praktijkvoorbeelden.
consult Joris is veroordeeld tot 5 jaar effectieve gevangenisstraf voor de verkrachting met gebruik van een wapen van een 7-jarig meisje, dochter van goede vrienden. Het was zijn eerste zedenfeit, maar hij heeft een verleden van geweldsdelicten. De psychosociale dienst (PSD) van de strafinrichting adviseert een residentiële psychiatrische behandeling, doch geen enkele instelling blijkt bereid Joris op te nemen. Strafeinde nadert en de PSD stelt een therapievraag bij I.T.E.R. in combinatie met een residentiële opname in een therapeutische gemeenschap. Stijn is bij een Vertrouwenscentrum Kindermishandeling geroepen, omdat zijn dochter op school vertelde dat papa naar vieze kinderplaatjes keek. Stijn vertelt er spontaan dat hij al heel zijn leven geobsedeerd is door kinderporno, eerst op video die hij aankocht in Nederland, dan via internet. Hij heeft zijn kinderen nooit seksueel aangeraakt, bezweert hij, maar masturbeert zich wel, terwijl hij hen begluurt en hun ondergoed besnuffelt. Ooit heeft hij seks gehad met een min-16-jarige, maar dat was wederzijds. Sven meldt zich bij I.T.E.R. aan op aanraden van zijn advocaat. Hij wordt beschuldigd van een veelvoud aan zedenfeiten en zijn proces komt wellicht binnen enkele maanden voor. Hij exhibeert zich voor jongvolwassen vrouwen, houdt ze staande en vraagt of ze hem willen slaan. Hij probeerde dit ook bij de buurvrouw en zo kon zijn profiel gelinkt worden aan een hele reeks onopgehelderde klachten. Uit een nauwkeurig sociaal onderzoek Kris Vanhoeck, Lic.
