Key words:prosthetic graft, vascular access, chronic renal failure, hemodialysis, transposed brachiobasilic AVF 〈Abstract〉 Purpose:A transposed brachiobasilic AVF(TBBAVF)provides vascular access in the absence of adequate superficial veins in the forearm;however, this procedure is not commonly utilized in Japan. This study assessed the feasibility of this procedure. Methods:TBBAVF was created in 43 patients(25 females and 18 males) between January 2003 and May 2008. There were three patients excluded from this study because two patients were lost to follow up and one patient did not require hemodialysis. A retrospective review of case notes was performed. Patency rates were calculated using Kaplan-Meier life-table analysis. The mean follow-up period after surgery was 2 years. Results:The primary patency rates at one and two years was 70% and 56%, respectively. The secondary patency rates at one and two year was 94% and 83% respectively. Major complications were steal syndrome in 4 cases(10%)and venous hypertension in one case(2.5%). Local infection was not seen in this study. Conclusion:The TBBAVF offers excellent patency rates with acceptable major complication rates. We conclude that TBBAVF should be considered before resorting to a prosthetic graft vascular access;however, these preliminary results require further follow-up and a larger cohort of patients for confirmation.
