ZusammenfassungHintergrund: Die R0-Resektion des Primärtumors mit adäquaten Sicherheitsabstän-den und regionärer Lymphadenektomie steht unverändert im Mittelpunkt der kurativen Therapie kolorektaler Karzinome. Während der Stellenwert adjuvanter Therapien in randomisierten, kontrollierten Multicenterstudien eindeutig belegt wurde, ist der Einfluss der operativen Technik, der behandelnden Klinik und des individuellen Chirurgen auf die Prognose nach R0-Resektion kolorektaler Karzinome umstritten. Material und Methode:Anhand einer Literaturauswahl und den Ergebnissen einer eigenen Untersuchung wurde der Einfluss der Technik, der Klinik und des einzelnen Chirurgen auf die Rezidivrate und das Langzeitüberleben nach Resektionen von Kolon-und Rektumkarzinomen untersucht.Ergebnis: Die Daten bestätigen die Hypothese, dass einfache Beachtung der generellen Prinzipien der onkologischen Chirurgie allein keine flächendeckend identischen Ergebnisse garantiert. Vielmehr haben die Details der chirurgischen Technik verschiedener Kliniken und die technischen Fähigkeiten einzelner Chirurgen bei Kolonkarzinomen und vor allem beim Rektumkarzinom einen erheblichen Einfluss auf die postoperative Prognose der Patienten.Schlussfolgerung: Nur eine strenge Standardisierung der Operationstechnik, die permanente Überprüfung der Operationsergebnisse und die frühzeitige Zwischenanalyse der Langzeitergebnisse anhand sog. Surrogatendpunkte im Sinne der internen Qualitätskontrolle gewährleisten eine gleichbleibend gute Qualität der Patientenversorgung in verschiedenen Kliniken und durch verschiedene Chirurgen innerhalb einer Klinik. Randomized, controlled multicenter trials have yielded exact data concerning adjuvant therapy following R0-resection. However, the prognostic value of the operative technique, the institution and the individual surgeon is under discussion.
During the last decade, perioperative therapy for patients undergoing elective colorectal resection has changed substantially. Most of these changes occurred in the perioperative medical treatment. However, surgical traditions like intraperitoneal drainage are still very frequently utilized.
