The implantation of a SG may be an alternative to open surgery in selected groups of patients with primary IAA. Aneurysms of the central part of the thoracic or abdominal aorta have a more favorable prognosis with endovascular treatment.
Úvod: Klasická cévní chirurgie a endovaskulární intervence se soustavně rozvíjejí a jejich využití se liší podle různých oblastí cévního systému. Chceme poukázat na to, že současné endovaskulární výkony neznamenají omezení, ale naopak zvýšení počtu pacientů, kteří mohou být úspěšně léčeni. Metody: Retrospektivně jsme vyhledali všechny otevřené operace a endovaskulární intervence provedené pro stenózu bifurkace a. carotis interna a pro aneurysma abdominální aorty (AAA) v letech 1990/1993 až 2014 na Oddělení cévní chirurgie Nemocnice Na Homolce. Výsledky: V letech 1990-2014 bylo na našem oddělení provedeno 1 659 otevřených operací AAA. Od roku 1996 bylo provedeno 1 023 endovaskulárních implantací stentgraftu pro výduť (EVAR) a od zavedení robotické chirurgie bylo provedeno 64 náhrad AAA s robotickou asistencí. Mortalita ve skupinách otevřených operací, EVAR a robotiky byla 1,7 %, 1,5 % a 0,4 %. Podíl EVAR se v průběhu posledních pěti let stabilizoval na 32 % celkového počtu léčených osob. V letech 1993-2014 bylo na našem oddělení provedeno 5 363 otevřených karotických endarterektomií, z toho 2 856 pro symptomatickou a 2 507 pro asymptomatickou stenózu. Kombinovaný podíl perioperační mozkové příhody nebo úmrtí v celém souboru byl 1,6 %. Symptomatičtí, asymptomatičtí a akutně operovaní pacienti měli kombinovaný podíl třicetidenní perioperační mozkové příhody nebo úmrtí 1,0 %, 1,7 % a 4,4 %. Ve stejném období bylo provedeno 274 perkutánních transluminálních angioplastik (PTA) karotické bifurkace a 55 PTA a. carotis communis. Technický úspěch endovaskulárních intervencí byl vyšší než 95 %. Závěr: Ve skupině AAA se podíl řešení pomocí EVAR stabilizoval v průběhu posledních pěti let na 32 % z celkového počtu léčených pacientů. Vzhledem k výborným výsledkům otevřených karotických operací a nepřesvědčivým výsledkům studií zabývajících se stentováním karotických stenóz považujeme klasické operace pro stenózu karotidy za lepší než primární stenting.
Objective: Heparin is used worldwide by vascular surgeons as prophylaxis for arterial thrombo-embolic complications during open and endovascular arterial surgery. Possible harmful effect of heparin use is more perioperative blood loss, resulting in a higher morbidity and mortality. To evaluate the evidence for the use of heparin during aorto-iliac arterial surgery a review was performed.Methods: A systematic review was performed of literature from MED-LINE, EMBASE and Cochrane databases, last search performed on March 8, 2012.Results: For open surgery for abdominal aortic aneurysm (AAA), only 5 studies were eligible for review and for endovascular aneurysm repair (EVAR) only 1 study. Overall methodological quality of the included studies was poor. One randomised trial could be retrieved. Possible harmful effects of heparin were found of increasing operation time, more blood loss and more transfusion requirements when heparin was used for open AAA surgery in one study. No data were found comparing heparin to no intervention for EVAR. One study compared heparin to a direct thrombin antagonist during EVAR, showing no differences in clinical outcomes.Conclusion: Despite limitations this review showed no compelling evidence on the beneficiary effect of the prophylactic perioperative use of heparin during open surgery for (r)AAA. Authors will promote a randomised controlled multi-center trial on this topic for elective open surgical repair of AAA.
