Xeroderma pigmentosum (XP) is a rare autosomal recessive disorder. Considering that XP patients have a defect of the nucleotide excision repair (NER) pathway which enables them to repair DNA damage caused by UV light, they have an increased risk of developing skin and eyes cancers. In the present study, we investigated the involvement of the prevalent XPA and XPC genes mutations—nonsense mutation (c.682C>T, p.Arg228X) and a two-base-pair (2 bp) deletion (c.1643_1644delTG or p.Val548Ala fsX25), respectively—in 19 index cases from 19 unrelated families in the West of Algeria. For the genetic diagnosis of XPA gene, we proceeded to PCR-RFLP. For the XPC gene, we validated a routine analysis which includes a specific amplification of a short region surrounding the 2 bp deletion using a fluorescent primer and fragment sizing (GeneScan size) on a sequencing gel. Among the 19 index cases, there were 17 homozygous patients for the 2 bp deletion in the XPC gene and 2 homozygous patients carrying the nonsense XPA mutation. Finally, XPC appears to be the major disease-causing gene concerning xeroderma pigmentosum in North Africa. The use of fragment sizing is the simplest method to analyze this 2 bp deletion for the DNA samples coming from countries where the mutation c.1643_1644delTG of XPC gene is prevalent.
Une mutation des deux allèles du gène Rb durant le développement rétinien norma est responsable de l’apparition du rétinoblastome. La recherche de mutations au niveau du gène Rb reste difficile, car la majorité des altérations sont uniques et dispersées tout au long du gène. Nous rapportons dans notre étude le résultat de l’analyse du gène Rb au niveau constitutionnel et tumoral dans la population algérienne.Notre étude a porté sur un échantillon de 21 patients atteints de rétinoblastome. Le promoteur et les 27 exons avec leurs séquences introniques flanquantes composant le gène Rb ont été amplifiés et analysés par HPLC (chromatographie liquide à haute performance) suivie d’un séquençage. Les variations de bases identifiées au niveau constitutionnel et tumoral sont au nombre de 7 dont 3 mutations non sens, une mutation affectant le site d’épissage, une délétion et 2 polymorphismes. L’analyse du gène Rb a été réalisée chez des enfants atteints de rétinoblastome ne présentant aucun antécédent familial de la maladie. Par cette étude, nous avons pu identifier deux cas sur les 21 étudiés ayant un rétinoblastome pouvant être transmissible.Deux mutations rapportées n’ont jamais été décrites dans la littérature.
Le rétinoblastome est la tumeur maligne intraoculaire la plus fréquente chez l’enfant, avec une incidence de 1 sur 15000 à 20000 naissances dans le monde. L’incidence exacte du rétinoblastome en Algérie n’est pas connue. Il est cité parmiles cancers de l’enfant et occupe de ce fait la 3ème place avec 6,3% après les leucémies et les lymphomes. Son diagnostic est souvent tardif. Des décès liés à la maladie sont encore enregistrés. L’objectif de cette étude est d’étudier le profilépidémiologique et clinique du rétinoblastome dans l’Ouest Algérien.Matériels et méthodes Nous avons effectué une étudedescriptive, rétrospective sur une période de 14 ans, du 1er mars 2002 au 31 décembre 2015. Elle regroupe tous les cas de rétinoblastome originaires dela région Ouest Algérien, recrutés au service d’ophtalmologiepédiatrique de l’EHS Canastel à Oran. L’analyse statistique a été réalisée en utilisant le test Chi2 pour la comparaison des pourcentages et un test « t » de student pour la comparaison des moyennes. Les courbes de survie ont été réaliséesselon la méthode Kaplan-Meier et comparées par un test de log-Rank.Résultats Au total, 140 cas de rétinoblastome ont été diagnostiqués, 91 cas unilatéraux (65%), 48 cas bilatéraux (34,3%) et un cas trilatéral (0,7%). La moyenne enregistréeétait de10 cas/an. Le sexe ratio M/F=1,25. L’âge moyen au diagnostic était de 26,56 mois.74% de ces malades vivaient en milieu rural. Les cas familiaux représentaient 8,57 %. Le premier signe d’appel était la leucocorie dans 58%, la buphtalmie 15%, le strabisme vient en 3ème position avec 13,5% suivi de la celluliteorbitaire avec 10%. Le retard de consultation était en moyenne de 4,1mois. 97% des rétinoblastomes unilatérauxont été diagnostiqués au stade IV ou Vet 20% des yeux des bilatéraux aux stades IV ou V. Le strabisme et la leucocorie sont associés à un meilleur taux de survie par rapport à la cellulite orbitaire et la buphtalmie (p=0,018).ConclusionLe profil clinique des rétinoblastomes dans l’Ouest Algérien ressemble à celui des pays en développement notamment maghrébins. Le diagnostic est souvent tardif par manque de dépistage et méconnaissance des signes révélateurs précoces de la maladie. Les efforts doivent être concentrés sur la vulgarisation des signes d’appels les plus précoces de la maladie au sein de la population et des médecins de santé primaire.
Retinoblastoma (RB) is an intraocular malignant tumour which occurs in children during the first months of life, with an incidence of 1 in 15,000 live births. Antioncogene RB1 located on chromosome 13q14.2, is at the origin of this pathology. For retinoblastoma development, two allele mutations of this gene are required. The aim of this study firstly, was to identify mutations that affect the RB1 gene in constitutional level, to detect early subject at risk or asymptomatic carriers and secondly to contribute on the understanding of the molecular pathogenesis. The study concerned 61 patients with retinoblastoma in Western Algeria. DNA from blood was used for amplification and gene sequencing. The results were completed by in-silico analysis using bio-informatic methods to know the mutation impact on the pRB1 protein function. Amplification and sequencing results gave nineteen different variations bases, including eight exonic changes: three missense mutations and five nonsense mutations located in exons 1,7,8,12,18,19,20 and 23. There are an important number of mutations located in twelve RB1 gene introns. These mutations were identified in germinal level for children with no family history of the disease. In conclusion, this study reported two new RB1 mutations in exons 1 and 7 among the eight identified. The mutations described in sporadic forms of retinoblastoma are transmissible forms in 13.11% of cases in our studied population. This study would improve role of genetic testing for management and family screening.
