Objetivo: analisar a taxa de incidência da tuberculose, proporção de doentes e de casos curados nos municípios prioritários do estado do Paraná. Método: estudo ecológico baseado em dados secundários do período de 2001-2012, dos municípios prioritários para controle da tuberculose no estado do Paraná. Resultados: verificou-se um total de 15.053 casos novos de tuberculose no período analisado, 11.538 foram curados, sendo que quatro cidades não conseguiram atingir a meta de cura de 85% dos casos e 3.239 casos realizaram o Tratamento Diretamente Observado. Dois municípios apresentaram uma correlação forte (Curitiba r=0,7 e São José dos Pinhais com r=0,8). Conclusão: o estudo reforça a importância da atenção básica no controle da tuberculose, com a finalidade de contribuir para a diminuição da transmissão da doença, a partir da melhoria da adesão ao tratamento, reduzindo assim as taxas de abandono e o surgimento de cepas resistentes.
Objective: analyzing the aspects associated with case outcomes and the international border situation when it comes to patients with tuberculosis (TB) associated with comorbidities. Method: exploratory descriptive epidemiological study, analyzing all new cases of TB with comorbidities reported in 2013 on SISNAN in the cities at the border of the state of Paraná and Paraguay. The variables used were: sociodemographic, clinical and operational. To analyze the data, descriptive analysis and the chi-square test were applied. Results: 227 cases of tuberculosis associated with comorbidities were reported and 70.9% were from cities bordering Paraguay. Alcoholism (22.0%) and AIDS (13.7%) were the most common comorbidities. The chi-square test presented a statistical association of border cities with negative outcomes and not performing directly observed treatments. Conclusion: Through the study, it was possible to find that the location of border cities can be determinant for a negative outcome.
Objetivo: caracterizar o perfil clínico epidemiológico e a distribuição espacial da incidência dahanseníase em territórios fronteiriços da América do Sul.Método: trata-se de um estudo ecológico. O estudo compreendeu a Província de Misiones naArgentina e a Região Sul do Brasil. A população foi composta por 10.319 casos novos de hanseníase,diagnosticados entre 2010 e 2016.Resultados: o estado do Paraná foi o mais endêmico, representando 70,2% (n=7,247) doscasos. Houve predomínio da classificação operacional multibacilar (79,8%, n=8.233) e Grau 0de incapacidade física (50,6%, n=5.223). Em Misiones 18,9% utilizaram esquema de tratamentosubstitutivo. No período, notou-se uma situação hiperendêmica em 780 (65,5%) dos municípios/departamentos estudados.Conclusão: o estudo mostrou que as regiões estudadas apresentam alta endemicidade, transmissãoativa e diagnóstico tardio da hanseníase. Essas tendências entrelaçadas à força de morbidade e detransmissão recente e persistente da doença, ampliam a relevância da hanseníase como problemade saúde pública na região.
Objective The aim was to analyze the spatial association and relative risk (RR) of leprosy cases diagnosed in southern Brazil and in the Argentinean province of Misiones during 2010 to 2016. Methods This ecological-type epidemiological study analyzed data from the Health Ministries of both countries. The analysis included frequency measures, spatial autocorrelation, RR cluster analysis and map construction. Results A hyperendemic occurrence was identified in all study regions, in the state of Paraná 71.2% of the municipalities were hyperendemic and in Misiones, Argentina 41.2%. The GI* statistical analysis showed clusters of high incidence rates in the state of Paraná and low-risk clusters in much of the state of Rio Grande do Sul, both in Brazil. The analysis indicated an area with RR equal to 3.87 - (p < .0001) when considering the entire territory and an RR of 2.80 - (p < .0001) excluding the state of Paraná, with the number of departments of Misiones, Argentina included in the risk clusters increasing significantly. Conclusions The findings indicate a high probability of similar illness in adjacent areas, according to their relative position in space, as the occurrence of the disease is influenced by neighboring clusters.
A hanseníase é uma doença infectocontagiosa de evolução lenta, causada pelo Mycobacterium leprae. Analisar as características sociodemográficas e clínicas de pacientes com hanseníase com intolerância ao tratamento padrão na cidade de Foz do Iguaçu-Paraná. Estudo epidemiológico, descritivo, transversal, do tipo levantamento. De 2012 a 2014, foram tratados e finalizados 146 casos, dos quais 48 receberam troca de tratamento, o medicamento com maior intolerância foi dapsona (91,5%), seguido de dapsona + clofazimina (6,4%) e ofloxacina (2,1%). Dimórfico e Virchowiano foi a forma clínica mais frequente, a anemia hemolítica ocorreu em 72,1% dos pacientes com intolerância ao dapsona. Concluiu-se que houve mais relatos da forma clínica dimórfica entre os pacientes intolerantes a dapsona e dapsona + clofazimina e o paciente intolerante a oflaxacina foi relatado como Virchowiano. É necessário intensificar e fortalecer a vigilância em hanseníase, no desempenho do diagnóstico e tratamento, visando eliminar fontes de infecção e evitar sequelas. To analyze the sociodemographic and clinical characteristics of leprosy patients intolerant to standard treatment in the city of Foz do Iguaçu-Paraná. Epidemiological, descriptive, cross-sectional study, type survey. From 2012 to 2014,146 cases were treated and finalized, of which 48 received treatment changes, the most intolerant drug was dapsone (91.5%), followed by dapsone + clofazimine (6.4%) and ofloxacin (2.1%). Dimorphic and Virchowian was the most frequent clinical form, hemolytic anemia occurred in 72.1% of the patients with dapsone intolerance. It was concluded that there were more reports of the dimorphic clinical form among the patients intolerant to dapsone and dapsone + clofazimine and the patient intolerant to oflaxacin was reported as Virchowiano. It is necessary to intensify and strengthen surveillance in leprosy, in the performance of diagnosis and treatment, aiming to eliminate sources of infection and avoid sequelae.
