It is unclear whether endovascular therapy (ET) can improve the prognosis of posterior circulation progressive ischemic stroke (PIS) due to large vessel stenosis/occlusion. The purpose of this study was to evaluate the efficacy and safety of ET for patients with PIS in posterior circulation who were selected based on ischemic penumbra detected on brain imaging. According to inclusion and exclusion criteria, patients with posterior circulation PIS hospitalized in the Second Affiliated Hospital of Nanchang University from May 2020 to December 2021 were retrospectively included and divided into ET + standard medical therapy (SMT) and SMT according to actual treatment conditions. Our research shows that ET is more effective than conservative therapy of Progressive Ischemic Stroke in Posterior Circulation with Large Artery Stenosis/Occlusion. In those patients with high NIHSS scores, ET had better neurological function recovery than conservative treatment. Delayed recanalization still has better therapeutic effect.
