Low-and middle-income countries NDT Neurodevelopmental therapy OCEBM Oxford Centre for Evidence-Based Medicine AIM To identify and assess published studies concerning physical therapy in Brazilian children and adolescents with cerebral palsy (CP) using the International Classification of Functioning, Disability and Health (ICF) framework. METHOD Articles in English and Portuguese published until October 2020, with no date restrictions, were searched in several different databases. Study characteristics, journal metrics, sample characteristics, and ICF domains explored intervention components and outcomes were extracted. Studies were classified according to the Oxford Centre for Evidence-Based Medicine hierarchy levels to characterize the evidence.RESULTS Ninety-four studies were included. Spastic CP with fewer limitations in gross motor abilities was the most reported; 67% of the studies had low levels of evidence and were published in journals without an impact factor. The three most frequent interventions were neurodevelopmental treatment, suit therapy, and transcranial direct current stimulation. Intervention components explored body functions and structures (73.4%), activity (59.6%), environment (2.1%). They did not explore participation (0%). The outcomes investigated addressed activity (79.8%), body functions and structures (67.0%), and participation (1.1%), but not environment (0%).

Background: Gross motor development curves for children with Cerebral Palsy (CP), grouped by Gross Motor Function Classification System (GMFCS) levels, help health care professionals and parents to understand children's motor function prognosis. Although these curves are widely used in Brazil to guide clinical decision-making, they were developed with Canadian children with CP. Little is known about how these patterns evolve in children and adolescents with CP in low-income countries like Brazil. The PARTICIPA BRAZIL aims to: (i) to identify and draw a profile of functioning and disability of Brazilian children and adolescents with CP by classifying them, for descriptive purposes, with all five valid and reliable functional classifications systems (gross motor function, manual ability, communication function, visual and eating and drinking abilities); (ii) to create longitudinal trajectories capturing the mobility capacity of Brazilian children and adolescents with CP for each level of the GMFCS; (iii) to document longitudinal trajectories in the performance of activities and participation of Brazilian children and adolescents with CP across two functional classification systems: GMFCS and MACS (Manual Abilities Classification System); (iv) to document longitudinal trajectories of neuromusculoskeletal and movement-related functions and exercise tolerance functions of Brazilian children and adolescents with CP for each level of the GMFCS; and (v) to explore interrelationships among all ICF framework components and the five functional classification systems in Brazilian children and adolescents with CP. Methods: We propose a multi-center, longitudinal, prospective cohort study with 750 Brazilian children and adolescents with CP from across the country. Participants will be classified according to five functional classification systems. Contextual factors, activity and participation, and body functions will be evaluated longitudinally and prospectively for four years. Nonlinear mixed-effects models for each of the five GMFCS and MACS levels will be created using test scores over time to create prognosis curves. To explore the interrelationships among ICF components, a multiple linear regression will be performed.

Improvement in rider mobility represents an important functional gain for people with disabilities undergoing hippotherapy. However, there is no validated measuring instrument to track and document the rider's progress in riding activities. In this study, we aimed to develop and establish validity evidence for an instrument to assess hippotherapy participants’ mobility on horseback. We report on this development through the stages of: (a) content validation, (b) construct validation, (c) inter- and intra-rater reliability and (d) internal consistency analysis. We evaluated its factor structure with exploratory factor analyses, calculated values for inter- and intra-rater reliability using the intra-class correlation coefficient, and calculated its internal consistency using Cronbach's alpha. We followed recommendations by the Guidelines for Reporting Reliability and Agreement Studies. We found good inter-rater reliability (intra-class correlation coefficient – ICC = 0.991–0.999) and good intra-rater reliability (ICC = 0.997–1.0), and there was excellent internal consistency (Cronbach's α = 0.937–0.999). The instrument’s factor structure grouped its three domains into one factor. As this instrument is theoretically consistent and has been found to be appropriate and reliable for its intended use, it is now available for the measurement of horseback mobility among hippotherapy riders.

Resumo O Denver II tem como objetivo avaliar o desenvolvimento de crianças entre 0 e 6 anos de idade. O objetivo deste estudo foi verificar a confiabilidade intra e interexaminadores, validade concorrente, sensibilidade e especificidade da versão brasileira do Denver II. Estudo metodológico transversal. Participaram 254 crianças, entre 0 e 72 meses, com risco para atraso no desenvolvimento. Dois examinadores verificaram a confiabilidade intra e interexaminadores do Denver II. Validade concorrente, sensibilidade e especificidade foram verificadas com relação à versão brasileira do Ages & Stages Questionnaires como teste de critério. Análise estatística utilizou o Intraclass Correlation Coeficiente, Teste de Correlação de Spearman e Tabela de Contingência, nível de significância α=0.05. Os resultados identificaram que a confiabilidade intra e interexaminadores foi excelente em toda a amostra. A validade concorrente apresentou índices moderados a muito fortes entre 13 e 60 meses. Índices de sensibilidade e especificidade variaram de 73-99% e 58-92%, respectivamente. A versão brasileira do Denver II apresenta bons índices de propriedades psicométricas sendo um instrumento confiável e válido para ser aplicado em crianças brasileiras sob risco de atraso no desenvolvimento.

A arquitetura e a organização dos edifícios escolares podem ser facilitadores ou barreiras na concretização do processo de inclusão escolar, sendo que a avaliação para adequação delas compõe parte importante do papel da equipe de reabilitação nesse processo. O objetivo do estudo foi verificar a acessibilidade física de escolas públicas de uma cidade de Minas Gerais. Foi realizado um estudo descritivo, transversal, com avaliação dos espaços físicos de 14 escolas, especialmente áreas de acesso, circulação e mobiliário, sanitários e estacionamentos, de acordo com os critérios descritos na Norma Brasileira 9050/2004. Os resultados foram apresentados em forma de estatística descritiva. No total foram avaliados 493 itens, desses, 85,6% encontravam-se inadequados. No setor "Acessos, circulação e mobiliário", 81,4% dos itens estavam inadequados. No setor "Sanitários", 94,6% dos itens estavam inadequados, sendo que não foi encontrado nenhum banheiro que contemplasse todas as normas de acessibilidade. No setor "Estacionamentos", 83,4% dos itens estavam inadequados. Nenhuma escola estadual da cidade estudada encontrava-se preparada para realizar a inclusão escolar de crianças com necessidades especiais no que diz respeito à acessibilidade.

Purpose: To estimate test-retest reliability of the two versions of the PEDI-CAT administered via telehealth to caregivers of Brazilian young people with DS, to compare scores on the two versions, and to determine caregiver acceptance of telehealth administration of the assessment. Method: A methodological study approved by the research ethics committee. Data collection was performed online, with a mean duration of 45.0 minutes for the content-balanced version of the PEDI-CAT and 17.5 minutes for the speedy version. Results: In total, 28 caregivers of individuals with DS up to age 21 years participated (mean = 5.9 years; SD = 4.9 years). Intra-class correlation coefficients for the four domains of the PEDI-CAT content-balanced version and four domains of the PEDI-CAT speedy version ranged from 0.77 to 0.97. There was a statistical difference between the versions in the scores of the social-cognitive domain ( p < 0.05). A mean of 105 items (SD = 21) was administered in the content-balanced version and a mean of 51 items (SD = 8) in the speedy version. All the caregivers found the method of administration of the PEDI-CAT acceptable. Conclusions: This study demonstrated that either version of the Brazilian version of the PEDI-CAT can be used by telehealth in clinical practice to assess children, adolescents, and young adults with DS.
