SummaryWe assessed the therapeutic significance of systematic aortic and pelvic lymphadenectomy followed by adjuvant therapy in nodepositive endometrial carcinoma. Among 173 stage lll patients, 30 (17%) had positive nodes: ten in the pelvic region alone (group P) and 20 in the aortic region alone or in both regions (group A). The adjuvant therapy was administered as follows: subjects in group P received 50 Gy pelvic radiation, including three post-surgical T3 (pT3) patients who received either one or three cycles of cisplatin-based chemotherapy before radiation. Subjects in group A were given three cycles of chemotherapy followed by 50 Gy pelvic and 50 Gy extended field periaortic radiation using a four-field or conformational technique. Five-year survival was 95% for 143 patients with negative nodes and 84% for 30 patients with positive nodes (100% for group P and 75% for group A). In group A, 5-year survival was 38% for eight patients with both pT3 and histology other than endometrioid type Gl, and 91% for the remaining 12 patients. Either way, both group P and group A patients had a better prognosis than previously reported. In summary, aortic and pelvic lymphadenectomy and subsequent chemotherapy and radiation therapy based on node status seem to improve the survival of endometrial cancer patients with positive nodes.

The apparent molal volumes (φV), apparent molal adiabatic compressibilities (φKS), and relative viscosities (nr) at 25 °C have been obtained for the following amino acids in aqueous alkali-chloride solutions from measurements of the density, the ultrasonic velocity, and the flow time: glycine, l-alanine, β-alanine, α-aminoisobutyric acid, l-serine, and l-threonine. Viscosity measurements were also carried out in water for reference. The alkali chlorides used were lithium, sodium, and potassium chlorides. The limiting values φVo, φKSo and the extended Jones-Dole viscosity coefficients, B and D, were calculated by a linear extrapolation using the least-squares method. All the φVo, φKSo, and B values obtained in the mixed solvents were larger than the corresponding ones in water, except in a few cases of the B coefficient. The results are discussed in terms of the dehydration effect of the electrolytes upon the amino acids.

The efficacy and adverse events of neoadjuvant chemotherapy with irinotecan hydrochloride and nedaplatin were evaluated in patients with bulky stage Ib2 to IIb cervical squamous cell carcinoma. Eligibility included patients who received irinotecan (60 mg/m2) on days 1 and 8 and nedaplatin (80 mg/m2) on day 1 of a 21-day cycle. After 1-3 courses of chemotherapy, radical hysterectomy was performed. Sixty-eight patients were enrolled. Sixty-six were included in the full analysis set. Their median age was 47 years (range 22-71), the FIGO stage was Ib2 in 18 patients, IIa in 10, and IIb in 38. Radical hysterectomy was performed after NAC in 63 patients (95.5%). The number of administered courses of NAC was 1 in 13 patients, 2 in 43, and 3 in 10. The response rate, the primary endpoint of this study, was 75.8% (CR in 2 patients, PR in 48, SD in 12, PD in 0, and NE in 4). The mean number of treatment courses required for a response was 1.42 (1 course in 30 patients, 2 courses in 19, and 3 courses in 1). The incidences of grade 3 or 4 hematological toxicities were: neutropenia 72.2%, leukopenia 16.7%, anemia 13.6%, thrombocytopenia 7.6%, febrile neutropenia 1.5%, and elevations of alanine aminotransferase and aspartate aminotransferase 1.5%. Grade 3 or 4 non-hematologic toxicities were as follows: diarrhea 6.1%, nausea 3%, anorexia 1.5%, vomiting 1.5%, fever 1.5%, allergic reactions 1.5%, ileus 1.5% and vesicovaginal fistula 1.5%. Neoadjuvant chemotherapy with irinotecan and nedaplatin was an effective and well-tolerated treatment for patients with bulky stage Ib2 to IIb squamous cell carcinoma of the uterine cervix.

The limiting values φVo and φKSo were calculated by a linear extrapolation using the least-squares method. These data were used to derive the volumes of transfer and the adiabatic compressibilities of transfer of the amino acids from water to aqueous urea solutions. Both of these transfer functions were positive. The results were interpreted in terms of structure-breaking interaction of urea upon water.
