Background: Nosocomial salmonellosis is often assumed to occur because infection control and surveillance practices are inadequate, but published evidence is lacking to support the related contention that rigorous application of these practices can impact the severity of outbreaks. Objective: Describe active surveillance, early recognition, and intensive mitigation efforts used in an effort to control an outbreak of nosocomial Salmonella enterica serotype Newport infections without hospital closure. Animals: Large animals hospitalized at a referral hospital. Methods: This prospective outbreak investigation was initiated when Salmonella Newport infections were detected among hospitalized animals by active surveillance. Data were analyzed to identify temporal and spatial patterns for epidemic spread of Salmonella in the hospital. Mitigation efforts were aggressively adjusted in response to surveillance data. Genetic relatedness of isolates was investigated by pulsed‐field gel electrophoresis. Results: Of 145 large animals sampled, 8 (5.6%) were infected with the Salmonella strain associated with this outbreak, and all but 1 shed Salmonella in the absence of or before the onset of disease. This strain was recovered from 14.2% (42/295) of environmental samples (ENV samples), indicating that widespread environmental contamination had occurred. Isolates of Salmonella Newport obtained from infected animals and the environment were genetically indistinguishable, confirming clonal dissemination. Conclusions and Clinical Importance: Active surveillance allowed early detection of nosocomial Salmonella transmission and hospital contamination. Use of aggressive interventions was followed by cessation of transmission. Active surveillance can allow earlier recognition and mitigation compared with programs by only sampling of clinically affected animals.

Animal shelter workers are a vulnerable population whose exposure to zoonotic disease may be greater compared with the general population. The aim of this project was to identify baseline zoonotic disease knowledge of animal shelter workers and to develop and evaluate zoonotic disease awareness training. Ten animal shelters in six western states were randomly selected. One hundred and eleven trainees were evaluated by identical pre- and post-training tests. Training topics included identification of clinical signs, susceptible species, and transmission of disease to animals and to humans. Zoonotic diseases included rabies, plague, leptospirosis, internal parasites, methicillin-resistant Staphylococcus aureus (MRSA) and salmonella. A statistically significant difference in overall total scores between pre-test (58.5%) and post-test (69.5%) was observed (P = 0.0001). No association was observed between test scores and length of time working in animal shelters, or with the participants' role at the animal shelter. Overall test scores were raised by 11%. The lowest baseline levels of knowledge were found with leptospirosis, MRSA, plague and rabies, emerging diseases with increasing prevalence and high consequence. Zoonotic disease awareness training is a valuable service to animal shelters. In the current study, training was modestly successful in transferring short-term knowledge to animal shelter workers. To understand and evaluate the effectiveness of training completely, observable or measureable behaviours should be compared before and after training. Long-term assessment with measureable outcomes is needed.

BackgroundIn Nepal, knowledge of proper handling, management and causes of cattle diseases is still limited. The main objective of this study was to explore the impact of deworming on milk production and its effect on milk qualities.MethodsA total of 200 faecal samples (100 buffaloes and 100 cows) were collected and analysed for parasitic burden. Half of the infected cattle (buffaloes, Bos bubalis; cow native, B indicus; European, B taurus) were then dewormed with Levamisole Hydrochloride-Oxyclozanide bolus, and the remaining 50 per cent were left untreated. The milk yield from both infected and dewormed cattle was recorded for 30 days and the qualities of milk were analysed.ResultsThe prevalence of parasitic infection was found to be 22.0 per cent. Fasciola hepatica was the predominant parasite (81.8 per cent), followed by Toxocara vitulorum (34.1 per cent), Strongyloidespapillosus (6.8 per cent) and Bunostomum phlebotomum (4.5 per cent). The average milk yield (litre/day/cow) significantly increased, which was 1.22 litres per day for treated cows and 1.06 litres for treated buffaloes. The intervention effect of deworming among cows was 0.79 (14.06 per cent increment) and for buffaloes was 0.42 (8.32 per cent increment). After deworming the infected cattle, the protein percentage was significantly improved in cows (P=0.035), whereas the lactose percentage and solid percentage had increased significantly in buffaloes (P=0.002 and P=0.028).ConclusionAntiparasitic treatment in cattle had positive effects on milk qualities such as solid non-fat, lactose, solid percentage and total protein percentage.

Salmonella enterica is an important zoonotic agent and nosocomial infections and epidemics have occurred in animal facilities. The objective of this study was to estimate the prevalence of Salmonella in the environment in animal shelters. From 12 to 25 samples were taken from each shelter to represent environmental contamination. Samples were collected from surfaces in areas used by animals and in public access areas including animal receiving rooms, kennels, paediatric wards, treatment, visitation rooms, isolation, euthanasia, outdoor runs and play areas, reception rooms, animal transport vehicles, offices, break rooms and restrooms. Samples were tested for Salmonella and compared within shelters to identify high- and low-risk areas; and between shelters to identify differences in environmental contamination by geographical location, infection control policies, and shelter size characteristics. Twenty-eight per cent of sampled Colorado shelters had environmental Salmonella contamination. Two regions in the eastern 1/3 of the state had prevalences of 30% and 100%. Within-shelter sample prevalence ranged from 0 to 100%. Results of this study indicate that animal shelters can be frequently contaminated with Salmonella spp., a variety of Salmonella species may be present, contamination can be widespread within a facility and recovered isolates may harbour antibiotic resistance The findings from this study may influence and help focus educational policy on issues of infection control and zoonotic disease awareness in animal shelters.
