Workers and volunteers in occupations least likely to have had prior disaster training or experience were at greatest risk of PTSD. Disaster preparedness training and shift rotations to enable shorter duration of service at the site may reduce PTSD among workers and volunteers in future disasters.

Coastal storms can take a devastating toll on the public's health. Urban areas like New York City (NYC) may be particularly at risk, given their dense population, reliance on transportation, energy infrastructure that is vulnerable to flood damage, and high-rise residential housing, which may be hard-hit by power and utility outages. Climate change will exacerbate these risks in the coming decades. Sea levels are rising due to global warming, which will intensify storm surge. These projections make preparing for the health impacts of storms even more important. We conducted a broad review of the health impacts of US coastal storms to inform climate adaptation planning efforts, with a focus on outcomes relevant to NYC and urban coastal areas, and incorporated some lessons learned from recent experience with Superstorm Sandy. Based on the literature, indicators of health vulnerability were selected and mapped within NYC neighborhoods. Preparing for the broad range of anticipated effects of coastal storms and floods may help reduce the public health burden from these events.

BackgroundStudies have consistently documented declines in respiratory health after 11 September 2001 (9/11) among surviving first responders and other World Trade Center (WTC) rescue, recovery, and clean-up workers.ObjectivesThe goal of this study was to describe the risk of newly diagnosed asthma among WTC site workers and volunteers and to characterize its association with WTC site exposures.MethodsWe analyzed 2003–2004 interview data from the World Trade Center Health Registry for workers who did not have asthma before 9/11 (n = 25,748), estimating the risk of newly diagnosed asthma and its associations with WTC work history, including mask or respirator use.ResultsNewly diagnosed asthma was reported by 926 workers (3.6%). Earlier arrival and longer duration of work were significant risk factors, with independent dose responses (p < 0.001), as were exposure to the dust cloud and pile work. Among workers who arrived on 11 September, longer delays in the initial use of masks or respirators were associated with increased risk of asthma; adjusted odds ratios ranged from 1.63 [95% confidence interval (CI), 1.03–2.56) for 1 day of delay to 3.44 (95% CI, 1.43–8.25) for 16–40 weeks delay.ConclusionsThe rate of self-reported newly diagnosed asthma was high in the study population and significantly associated with increased exposure to the WTC disaster site. Although we could not distinguish appropriate respiratory protection from inappropriate, we observed a moderate protective effect of mask or respirator use. The findings underscore the need for adequate and timely distribution of appropriate protective equipment and the enforcement of its use when other methods of controlling respiratory exposures are not feasible.

Heat waves can be lethal and routinely prompt public warnings about the dangers of heat. With climate change, extreme heat events will become more frequent and intense. However, little is known about public awareness of heat warnings or behaviors during hot weather. Awareness of heat warnings, prevention behaviors, and air conditioning (AC) prevalence and use in New York City were assessed using quantitative and qualitative methods. A random sample telephone survey was conducted in September 2011 among 719 adults and follow-up focus groups were held in winter 2012 among seniors and potential senior caregivers. During summer 2011, 79 % of adults heard or saw a heat warning. Of the 24 % who were seniors or in fair or poor health, 34 % did not own AC or never/rarely used it on hot days. Of this subgroup, 30 % were unaware of warnings, and 49 % stay home during hot weather. Reasons for not using AC during hot weather include disliking AC (29 %), not feeling hot (19 %), and a preference for fans (18 %). Seniors in the focus groups did not perceive themselves to be at risk, and often did not identify AC as an important health protection strategy. While heat warnings are received by most New Yorkers, AC cost, risk perception problems, and a preference for staying home leave many at risk during heat waves. Improving AC access and risk communications will help better protect the most vulnerable during heat waves.

Air quality health impact assessment (HIA) synthesizes information about air pollution exposures, health effects, and population vulnerability for regulatory decision-making and public engagement. HIAs often use annual average county or regional data to estimate health outcome incidence rates that vary substantially by season and at the subcounty level. Using New York City as an example, we assessed the sensitivity of estimated citywide morbidity and mortality attributable to ambient fine particulate matter (PM2.5) and ozone to the geographic (county vs. neighborhood) and temporal (seasonal vs. annual average) resolution of health incidence data. We also used the neighborhood-level analysis to assess variation in estimated air pollution impacts by neighborhood poverty concentration. Estimated citywide health impacts attributable to PM2.5 and ozone were relatively insensitive to the geographic resolution of health incidence data. However, the neighborhood-level analysis demonstrated increasing impacts with greater neighborhood poverty levels, particularly for PM2.5-attributable asthma emergency department visits, which were 4.5 times greater in high compared to low-poverty neighborhoods. PM2.5-attributable health impacts were similar using seasonal and annual average incidence rates. Citywide ozone-attributable asthma morbidity was estimated to be 15 % lower when calculated from seasonal, compared to annual average incidence rates, as asthma morbidity rates are lower during the summer ozone season than the annual average rate. Within the ozone season, 57 % of estimated ozone-attributable emergency department for asthma in children occurred in the April–June period when average baseline incidence rates are higher than in the July–September period when ozone concentrations are higher. These analyses underscore the importance of utilizing spatially and temporally resolved data in local air quality impact assessments to characterize the overall city burden and identify areas of high vulnerability.
