Introduction 2018 ESC/ESH guidelines have recommended 24-h ambulatory blood pressure monitoring to assess hypotensive therapy in many circumstances. Recommended target blood pressure in office blood pressure measurements is between 120/70 and 130/80 mmHg. Such targets for 24-h ambulatory blood pressure monitoring lacks. Aim We aimed to define target values of blood pressure in 24-h ambulatory blood pressure monitoring in hypertensive patients. Methods Office blood pressure measurements and 24-h ambulatory blood pressure monitoring data were collected from 1313 hypertensive patients and sorted following increasing systolic (SBP)/diastolic (DBP) blood pressure in office blood pressure measurements. The corresponding 24-h ambulatory blood pressure monitoring to office blood pressure measurements values were calculated. Results Values 130/80 mmHg in office blood pressure measurements correspond in 24-h ambulatory blood pressure monitoring: night-time SBP/DBP mean: 113.74/66.95 mmHg; daytime SBP/DBP mean: 135.02/81.78 mmHg and 24-h SBP/DBP mean: 130.24/78.73 mmHg. Values 120/70 mmHg in office blood pressure measurements correspond in 24-h ambulatory blood pressure monitoring: night-time SBP/DBP mean: 109.50/63.43 mmHg; daytime SBP/DBP mean: 131.01/78.47 mmHg and 24-h SBP/DBP mean: 126.36/75.31 mmHg. Conclusions The proposed blood pressure target values in 24-h ambulatory blood pressure monitoring complement the therapeutic target indicated in the ESC/ESH recommendations and improves 24-h ambulatory blood pressure monitoring usefulness in clinical practice. Objective:In hypertensive population are patients whose office blood pressure measurements differ from those in 24-hour ambulatory blood pressure measurements. They are patients with the white-coat effect (office blood pressure is higher than office measurements) and with the reverse white-coat effect (ambulatory measurements are higher than office blood pressure measurements). The cardiovascular risk and the prevalence of organ damages in both phenomena are unclear. There are studies showing that these phenomena could be clinically significant. The aim of the study was to assess the prevalence of white-coat effect and reverse white-coat effect in treated hypertensive patients, depending on gender and age.Design and method:The office and 24-hour ambulatory blood pressure measurements data were collected from 1465 hypertensive patients (771 men) with mean age 55.9 ± 15.2. The reverse white-coat effect was defined as: office systolic blood pressure – daytime systolic blood pressure mean < -10 mmHg. The white-coat effect was defined as: office systolic blood pressure – daytime systolic blood pressure mean > = 20 mmHg and/or office diastolic blood pressure – daytime diastolic blood pressure mean > = 10 mmHg. Patient data were sorted in accordance with gender and divided into age decades every 10 years. Statistica software (StatSoft) and Microsoft Excel were used to calculate phenomena prevalence.Results:300 (20.5%) patients met the reverse and 286 (19.5%) white-coat effect criteria. The prevalence of both phenomena was similar in the treated hypertensive population. The white-coat effect was more common in older patients, with the highest prevalence in the 65–74 age decade (26.3%). The reverse white-coat effect referred to younger patients - predominating was 25–34 age decade (31.9%). Depending on gender, the white-coat effect was dominant among women aged 75–84 (27.4%) and among men aged 65–74 (27.1%). The reverse white-coat effect was dominant among women and men aged 25–44 (appropriate 43.5% and 26.1%).Conclusions:The reverse white-coat effect occurs the most often in women and men in the 25–34 age decade. The white-coat effect is the most common among women in 75–84 and among men in the 65–74 age decade.

According to World Health Organization, as many as 1.28 billion adults worldwide were suffering from HT in 2011 -a half were unaware of their disease. It is claimed that approximately 10 million of deaths annually are the result of improperly high blood pressure values (BP) [1]. It is estimated that the number of HT patients will exceed 1.5 billion by 2025. Therefore, it classifies HT as a major global healthcare problem [2].According to the WOBASZ II study, more than 40% of the adult Polish population were diagnosed with HT. Cognitive disorders after surgery are a quite common problem, though not fully understood and described in detail. The problem of cognitive disorders concerns not only young people, but also elderly people, which, according to statistical data, constitute an increasingly large population group. The literature on the subject points to the multidimensionality and complexity of the phenomenon of cognitive disorders. Their occurrence may be related to the patient's preparation for surgery, the extent of surgery, post-operative care, the intensity of pain, the way of nourishing the patients before surgery or even leading a hygienic lifestyle. The introduction of psychological tests in the elderly can be helpful in categorizing patients and assessing their need for nursing care. The aim of the study is to present the problem of cognitive disorders in the elderly, to describe them, as well as to identify the most important factors in the care of an elderly patient. Very important in the care of patients with cognitive impairment is good cooperation between doctors, nurses, psychologists, physiotherapists and dietitians.

Wstęp: Idiopatyczne włóknienie płuc (IPF, Idiopathic pulmonary fibrosis) jest przewlekłym, postępującym i destrukcyjnym schorzeniem układu oddechowego, z medianą przeżycia wynoszącą 3–5 lat. Pirfenidon (PIR) i nintedanib (NTB) to jedyne leki, które skutecznie spowalniają progresję choroby. Choć są zarejestrowane w Polsce, nie były w naszym kraju powszechnie dostępne do końca 2016 roku z powodu braku zasad finansowania. Celem badania była ocena powszechnej praktyki klinicznej w Polsce w odniesieniu do rozpoznawania i leczenia IPF w okresie przed wprowadzeniem leków przeciwzwłóknieniowych. Materiał i metody: Przeprowadzono badanie ankietowe wśród lekarzy uczestniczących w dwóch ogólnopolskich kongresach pulmonologicznych w 2016 roku. Wyniki: W badaniu wzięło udział 150 lekarzy. Tylko 55% respondentów deklarowało, że ostateczne rozpoznanie jest stawiane we współpracy z radiologiem. Tylko 40% lekarzy kierujących chorych na chirurgiczną biopsję płuca omawia jej wyniki bezpośrednio z patologiem. Nigdy nie kieruje chorych z podejrzeniem IPF na chirurgiczną biopsję płuca 22%. Wśród respondentów 85% uważa, że płukanie oskrzelowo-pęcherzykowe (BAL, bronchoalveolar lavage) może być przydatne dla rozpoznania. Niestosowanie żadnych leków u chorych na IPF deklarowało 41% respondentów, a 23% lekarzy deklarowało stosowanie kortykosteroidów w większych dawkach w monoterapii lub w skojarzeniu z innymi lekami. Tylko 43% respondentów zastosowałoby leki obniżające kwasowość soku żołądkowego u chorego z objawami refluksu żołądkowo-przełykowego (GERD, gastro-esophageal reflux disease), a tylko 11% niezależnie od rozpoznania GERD. Wnioski: Większość polskich pulmonologów nie jest wspierana przez radiologów w procesie diagnostycznym. Standardy leczenia były niesatysfakcjonujące, głównie z powodu braku regulacji dotyczących finansowania leczenia. Dalsza edukacja jest wskazana w celu poprawy standardów leczenia chorych na IPF w Polsce.
