Bogota is one of the most polluted cities in Latin America, increasing the prevalence of respiratory symptoms in children under five. As the most unequal city in the region, there is a direct gradient between socioeconomic and environmental inequalities. The aim of the study was to know the impact of indoor and outdoor pollution of respiratory disease in children under five in Bogota. A cross sectional study was developed in kindergartens of 15 localities in Bogota, divided into two groups according to exposure level to air pollution. 5407 children under five were selected by two-stage random sampling. A survey instrument based on an ISAAC questionnaire was applied to identify associated factors to respiratory outcomes. Statistical analysis included descriptive x 2 test and logistic regression by PSPP. Deprived zones of Bogotá were the most affected areas. Wheezing during exercise was associated with outdoor pollution plus unpaved roads (1.68, 1.18-2.38); last year wheezing to indoor factors as second hand smoke (1.82, 1.32-1.79), dampness (1.32, 1.01-1.74) and in-home factory (1.41, 1.02-1.96); dry cough at night to second hand smoke (1.49, 1.12-1.98) and proximity of chimneys (1.43, 1.11-1.86). Main predictors for respiratory disease were related to indoor air pollution and social vulnerability, although outdoor pollution was relevant to wheezing during exercise. Second hand smoke is a significant factor in the burden of disease in children under five. Public health policies in Bogotá should address not only outdoor pollution, but also housing inequalities.

El 60 % de los residentes informaron mala calidad del sueño según el índice de calidad del sueño de Pittsburgh (ICSP>5), con una media de 7,19 (desviación estándar, DE=3,931), y se encontraron puntuaciones patológicas así: calidad subjetiva del sueño, 27 %; latencia del sueño, 39 %; duración del sueño, 33 %; eficiencia habitual del sueño, 37 %; alteraciones del sueño, 30 %; disfunción diurna, 40 %, y uso de hipnóticos, 5 %. Según la escala de somnolencia de Epworth (ESE>10), se reportó 28 % de somnolencia diurna. Con relación a la prevalencia de la mala calidad del sueño según el índice, 17 % de quienes reportaron no dormir debido al ruido lo asoció al tráfico aéreo. Se encontró correlación entre el índice y la escala (ρ=0,329, IC 95% 0,20-0,44). Conclusiones. Los habitantes de la localidad presentaron mala calidad del sueño por exposición al ruido, siendo la operación aeroportuaria una de las principales fuentes generadoras. Es necesario revisar las estrategias de mitigación de ruido en la localidad y considerar las implicaciones de la ampliación del Aeropuerto El Dorado para la salud pública. La contaminación del aire es uno de los principales problemas de Salud Ambiental en el país y en el Distrito Capital. El promedio anual de la concentración de material particulado inferior a 2.5 micrómetros (PM2.5) para la ciudad, ha venido disminuyendo a través de los años a causa de diferentes acciones encaminadas a reducir fuentes de emisión pasando de 24 ug/ m3 en el 2014 a 18 ug/m3 en el 2019. Este contaminante tiene una relación estrecha con los efectos sobre la salud, sobre todo en el Sistema Cardiovascular; es así que el riesgo atribuido a la contaminación del aire en la ciudad varia de la siguiente manera conforme a los diferentes eventos en salud: Accidente Cerebrovascular 25 %, Enfermedad isquémica del corazón 24 %, Cáncer de Pulmón 14 %, Enfermedad Pulmonar Obstructiva Crónica (EPOC) e Infección Respiratoria Aguda (IRA) 8 %. A pesar de los esfuerzos realizados para la disminución de la contaminación del aire, anualmente en el primer trimestre del año, se reciben en la ciudad aportes regionales de contaminantes debido a patrones regionales de vientos; sumado a esto, por fenómenos meteorológicos como la inversión térmica no permite su dispersión en la atmósfera bogotana comparado con otros meses, ocasionando un aumento inusitado de contaminación del aire principalmente por material particulado inferior a 2.5 micrómetros (PM2.5), ocasionando que se decreten alertas ambientales y se despliegue el accionar de la Administración Distrital para preservar la salud de la población. Ante este panorama, la Secretaría Distrital de Salud decidió formular el Plan de respuesta sectorial de gestión integral de riesgos en salud por calidad del aire de Bogotá, como medida que permita abordar de manera organizada las acciones del sector salud frente a situaciones rutinarias y de emergencia ante la contaminación del aire en la ciudad.
