The COVID-19 pandemic represents a worldwide threat to mental health. To optimize the allocation of health care resources, research on specific vulnerability factors, such as health anxiety, intolerance of uncertainty, and distress (in)tolerance, and particularly their effect on the time course of SARS-CoV-2 related anxiety appears crucial for supporting high risk groups suffering from elevated mental distress during the pandemic. N = 887 participants (78.4% female; Mage = 38.15, SD = 17.04) completed an online survey in Germany (April to mid-May 2020), comprising measures of SARS-CoV-2 related anxiety, health anxiety, safety and preventive behavior, intolerance of uncertainty, and distress intolerance. Higher levels of health anxiety pre and during COVID-19 were associated with an initially intensified increase (b = 1.10, p < 0.001), but later on a more rapid dampening (b = −0.18, p < 0.001) of SARS-CoV-2 related anxiety. SARS-CoV-2 related preventive behavior was intensified by both pre (b = 0.06, p = 0.01) and during (b = 0.15, p < 0.001) COVID-19 health anxiety, while reassurance behavior only was associated with health anxiety during COVID-19 (b = 0.14, p < 0.001). Distress intolerance and intolerance of uncertainty did not moderate the relationship between health anxiety and SARS-CoV-2 related anxiety and behavior. The results suggest detrimental effects of health anxiety on the emotional and behavioral response to virus outbreaks.
The Inhibitory Learning Theory (ILT; Craske et al., 2008) changed the focus of exposure-based treatment from erasing excitatory associations and fear reduction (habituation (HA)) to reinforcing inhibitory associations and fear toleration (inhibitory learning (IL)). Studies which directly compare both approaches, IL versus HA, are scarce. The present study aimed at implementing and comparing an IL-based ( n = 26; Age: M = 23.59, SD = 4.38) with a HA-based ( n = 28; Age: M = 25.46, SD = 6.22) experimental exposure approach (including in vivo, interoceptive, and in sensu exposure) in a sample of people with heightened health anxiety. A significant pre- to post-intervention reduction of state health anxiety ( p < .001), which was especially associated with an increase of distress tolerance (DT) pre- to post-exposure ( F (1, 50) = 12.2, p < .001, ηp2 = .20), was observed. A superiority of the IL-based over the HA-based exposure intervention was not detected in relation to major outcomes (e.g., state health anxiety), as well as in relation to a change of DT. The present study underlines the importance of strengthening DT (for heightened health anxiety) during an exposure-based intervention.
Zusammenfassung. Theoretischer Hintergrund: Aktuelle Modelle der Wahrnehmung körperlicher Beschwerden gehen davon aus, dass negative emotionale Zustände (z. B. Angst und Traurigkeit) eine zentrale Rolle bei der Symptomwahrnehmung spielen. Eine kausale Wirkung von Ärger auf den körperlichen Symptombericht wurde hingegen noch nicht ausreichend nachgewiesen. Fragestellung: Führt eine experimentelle Induktion von Ärger und Traurigkeit zu einem verstärkten Bericht körperlicher Beschwerden? Methode: 90 Studienteilnehmende wurden zufällig drei experimentellen Bedingungen zugewiesen. Mit einer autobiografischen Methodik wurden Ärger (n = 30) und Traurigkeit (n = 30) induziert. Eine dritte Gruppe (n = 30) diente als neutrale Kontrollbedingung. Bei allen Teilnehmenden wurden vor und nach der Induktion die Emotionswahrnehmung sowie der körperliche Symptombericht erhoben. Ergebnisse: Nach der Induktion von Ärger und Traurigkeit war ein Anstieg der intendierten Emotionen Ärger in der Gruppe „Ärger“, t (29) = -5.43, p < .001, und Traurigkeit in der Gruppe „Traurigkeit“ festzustellen, t (29) = -4.81, p < .001. Lediglich nach der Induktion von Traurigkeit, t (29) = -4.04, p < .001, nicht jedoch nach der Induktion von Ärger, t (29) = -1.52, p = .14, war eine Verstärkung des körperlichen Symptomberichts zu beobachten. Schlussfolgerungen: Die Ergebnisse belegen unterschiedliche Effekte negativer Emotionen (am Beispiel Ärger und Traurigkeit) auf den körperlichen Symptombericht. Implikationen der Befunde für zentrale psychopathologische Phänomene werden diskutiert.

In den letzten Jahren gab es Fortschritte bei der Erforschung und Entwicklung effektiver psychotherapeutischer Behandlungen. Kognitiv-verhaltenstherapeutische Therapiekonzepte repräsentieren derzeit die Methode der Wahl zur Behandlung der Hypochondrischen Störung.
