Samenvatting In dit artikel wordt achtereenvolgens ingegaan op de biologische en psychologische verklaringsmodellen voor het ontstaan van obesitas bij kinderen. Vervolgens worden de huidige inzichten omtrent de behandeling beschreven. Sinds enkele jaren is er een nieuwe tendens merkbaar in de therapieliteratuur: niet gewichtsverlies maar gewichtscontrole en het opbouwen van een gezonde levensstijl worden belangrijke behandeldoelen. Een combinatie van drie behandelingredie¨nten is daarbij noodzakelijk: voedingsrichtlijnen, een bewegingsprogramma en gedragstherapie. Deze vernieuwende aanpak wordt geı¨llustreerd aan de hand van een casus. Uit de beschrijving van onze ervaringen met Sanne, een negenjarig meisje met overgewicht, blijkt dat ouderbegeleiding vaak een noodzakelijk onderdeel van de behandeling is. InleidingOnlangs wees de Wereldgezondheidsorganisatie (WHO, 1998 ) er op dat obesitas een ernstige bedreiging vormt voor de gezondheid. Gezien de omvang ervan spreekt men momenteel van een 'obesitas-epidemie'. Men schat dat alleen al in de Verenigde Staten zo'n 15 miljoen mannen en 18 miljoen vrouwen er last van hebben (Williamson, 1995 ). Het is ruimschoots aangetoond dat dikke mensen hoge kosten betekenen voor de maatschappij, vooral door de verhoogde kansen op fracturen, ontwikkeling van diabetes type II, hypertensie, respiratoire en cardio-vasculaire aandoeningen (Pi-Sunyer, 1993 ).Reeds tientallen jaren beschouwt men 'op dieet gaan' als de ultieme oplossing voor het verliezen van overtollige kilo's. Helaas, steeds opnieuw blijkt dat men na stopzetting van het dieet weer in gewicht toeneemt en dat men na afloop soms meer weegt dan voordien (Wilson, 1994a ). Bovendien zou op dieet gaan ook nog schadelijke neveneffecten hebben zoals bijvoorbeeld verhoogde kans tot het ontwikkelen van eetbuien en stoornissen in de regulatie van het basaal metabolisme (Garner & Wooley, 1991 ). Wetenschappelijk onderzoek is tot de conclusie gekomen dat er voor volwassen personen met obesitas momenteel geen geschikte behandelingsmethode bestaat en dat er maar enkele 'hoopgevende' behandeldoelen zijn (Wilson, 1994a ):1. streven naar gewichtscontrole in plaats van gewichtsverlies, waarbij maximaal 10% gewichtsverlies is toegelaten; 2. opbouwen van een gezonde levensstijl, wat een meer gunstige prognose op het voorkomen van ziektes inhoudt; 3. voorko´men van overgewicht, onder andere via vroegtijdige interventie bij dikke kinderen.In dit artikel zullen achtereenvolgens de biologische en psychologische verklaringsmodellen aan bod komen van het ontstaan van obesitas bij kinderen, indicaties voor behandeling en de huidige behandeling.
