WstępWe współczesnej kardiochirurgii stosuje się coraz więcej technik małoinwazyjnych. Do jednej z nich należy endoskopowe pobranie żyły odpiszczelowej przy operacjach wieńcowych. Technika ta daje możliwość pobrania naczynia, znacznie ograniczając wielkość urazu w miejscu interwencji przy zachowaniu odpowiedniej jakości pomostu.CelCelem pracy było porównanie endoskopowego oraz klasycznego pobrania żyły odpiszczelowej wśród chorych, u których występuje zwiększone ryzyko chirurgiczne. Ocenie został poddany wpływ tych metod na jakość pobranego naczynia, przebieg leczenia i gojenie się rany u operowanych pacjentów.Materiał i metodyW lipcu 2016 r. w Śląskim Centrum Chorób Serca zabiegowi kardiochirurgicznemu poddano 136 chorych. U 66 chorych został wykonany zabieg wieńcowy z wykorzystaniem żyły odpiszczelowej. Chorych, będących w grupach średniego lub wysokiego ryzyka infekcji pooperacyjnej według skali BHIS, podzielono na dwie grupy liczące po 10 chorych. W jednej grupie żyła odpiszczelowa została pobrana metodą endoskopową, w drugiej metodą klasyczną. Śródoperacyjnie oceniano jakość naczynia oraz przepływ w pomostach. Ocenie został poddany także miejscowy obszar gojenia się rany kończyny dolnej, który następnie porównano między grupami.WynikiW czasie zabiegów wykorzystano wszystkie pobrane naczynia do pomostowania aortalno-wieńcowego. W ocenie śródoperacyjnej nie wykryto mikroskopowych uszkodzeń naczynia. W obserwacji pooperacyjnej chorych, u których naczynie zostało pobrane techniką endoskopową, nie stwierdzono powikłań związanych z gojeniem się rany.WnioskiPobranie żyły odpiszczelowej metodą endoskopową jest tak samo bezpieczne jak metodą klasyczną, zapewnia jednak lepsze gojenie się rany, znacząco poprawia efekt kosmetyczny operowanej kończyny oraz zmniejsza ryzyko infekcji rany.

The shortages of medical staff, especially medical ones, are increasingly contributing to the failure of Polish health care. The available data and forecasts are pessimistic, therefore it seems appropriate to create a new medical profession that will be able to fill the gap that has arisen among medical personnel (especially in terms of surgical specialties and closely related anasthesiology and intensive therapy). The Physican Assistant is a common and respected profession in the health care system of many developed and developing countries. Currently, 18 countries around the world have a fully developed PA training system in which, after training, Physician Assistants fully perform their specific duties. In many countries, the PA program is a pilot program. These countries include, among others, Poland, where the duties of PA have so far been performed in the pilot program by paramedics after undergraduate studies. The PA pilot program was very positively assessed by the entire hospital community, both medical staff and management.
W ostatnich latach myszy stały się najczęściej wykorzystywanymi zwierzętami laboratoryjnymi w badaniach przedklinicznych. Wraz z rosnącym ich wykorzystaniem także w badaniach układu krążenia (zarówno w terapiach komórkowych regenerujących mięsień sercowy po zawale, leczeniu niewydolności serca, jak i w ocenie kardiotoksyczności leków) konieczne stało się opracowanie dokładnej metody oceny funkcji serca, która rzetelnie mogłaby określić skuteczność i bezpieczeństwo badanych terapii. Przeniesienie techniki śledzenia markerów akustycznych (speckle tracking echocardiography-STE) do warunków echokardiografii doświadczalnej pozwala na bardziej rzetelną i wiarygodną ocenę ich skuteczności i bezpieczeństwa. Pomiary regionalnej deformacji serca uzyskiwane tą metodą są dużo bardziej czułe i specyficzne niż parametry klasycznej echokardiografii. Technika STE wydaje się bardzo atrakcyjną metodą, nie jest jednak pozbawiona wad i ograniczeń. Analiza obrazów 2D techniką STE jest bardzo wrażliwa na artefakty, które zacierając granice wsierdzia, uniemożliwiają prawidłowe śledzenie markerów akustycznych. Uzyskanie powtarzalnych obrazów o dobrej jakości jest wciąż dużym wyzwaniem, szczególnie w warunkach echokardiografii doświadczalnej. W dostępnym piśmiennictwie techniczne i praktyczne aspekty tego badania są często pomijane lub opisane bardzo enigmatycznie, a nierzadko to właśnie one są podstawą do
