A one-day point prevalence study to investigate the patterns of antibiotic use was undertaken in 43 latin American (LA) intensive care units. Of 510 patients admitted, 231 received antibiotic treatment on the day of the study (45%); in 125 cases (54%) due to nosocomial-acquired infections. The most frequent infection reported was nosocomial pneumonia (43%). Only in 122 patients (53%) were cultures performed before starting antibiotic treatment. 33% of the isolated microorganisms were enterobacteriaceae (40% extended-spectrum beta-lactamase-producing), 23% methicillin-resistant Staphylococcus aureus and 17% carbapenems-resistant non-fermentative Gram-negatives. The antibiotics most frequently prescribed were carbapenems (99/231, 43%); alone (60/99, 60%) or in combination with vancomycin (39/99, 40%). "Restricted" antibiotics (carbapenems, vancomycin, piperacillin-tazobactam, broad-spectrum cephalosporins, tigecycline, polymixins and linezolid) were most frequently indicated in severely ill patients (APACHE II score at admission >15, p=0.0007 and, SOFA score at the beginning of the antibiotic treatment >3, p=0.0000). Only 36% of antibiotic treatments were cultured-directed.Our findings help explain the high rates of multidrug-resistant pathogens in LA settings (i.e. ESBL-producing Gram-negatives) and the severity of the registered patients illnesses.
Introducción: La mortalidad relacionada a la neutropenia febril puede ser muy alta en ciertas poblaciones de alto riesgo. El objetivo del presente estudio fue identificar factores predictores tempranos de mortalidad en una cohorte de pacientes oncológicos con neutropenia febril admitidosen una unidad de cuidados intensivos (UCI). Métodos: Estudio observacional retrospectivo en pacientes con neutropenia febril mayores de 18 años ingresados en la UCI del Hospital Oncológico Solón Espinosa Ayala de Quito. Se recogieron variables sociodemográficas, clínicas, de laboratorio y microbiológicas de los registros clínicos al ingreso. Con dichas variables se construyeron modelos predictivos de mortalidad mediante análisis de regresión logística para identificar predictores de muerte. Resultados: Se registraron 107 pacientes con episodios de neutropenia febril, el 53.3 % de los casos presentaron neutropenia severa y en un 29.9 % de los casos la neutropenia duró más de 10 días. La prevalencia de bacteriemia fue del 34.6 %. El 34.6 % de los pacientes murieron (n=37), de estos el 22.4 % falleció en la UCI. Edad superior de 40 años, valores de procalcitonina superiores a 4 ng/ml, puntuaciones altas en APACHE II y la necesidad de apoyo ventilatorio se asociaron con mayor riesgo de muerte en el modelo multivariado; el valor predictivo en la validación interna tuvo una precisión de 81.3 %; sensibilidad de 63.6 %; especificidad de 90.5 %; valor predictivo positivo de 77.8 %; valor predictivo negativo de 82.6 %; área bajo la curva de 0.87. Conclusión: Factores como la edad mayor de 40 años, procalcitonina > 4 ug/ml al ingreso, valores de APACHE II y necesidad de ventilación mecánica están asociados con mayor riesgo de muerte.
Background: Procalcitonin (PCT) could predict the presence of bacteremia in critically ill patients with febrile neutropenia. The objective was to determine the predictive value of PCT in the diagnosis of bacteremia and its ability to differentiate between gram-negative and gram-positive organisms. Methods: Retrospective cohort study conducted at the Oncology Hospital "Solón Espinosa Ayala, " Quito-Ecuador, which included patients who were admitted with febrile neutropenia to intensive therapy. PCT values were evaluated at admission together with blood culture samples, from 2010 to 2016. Results: The association between PCT levels and the diagnosis of bacteremia was investigated in 117 patients. PCT demonstrated bacteremia discrimination with a point of cut of 15.5 ng/ml and an area under the curve (AUC) of 0.76. The concentration of PCT in patients with bacteremia by gram-negative germs was higher compared with patients with bacteremias by gram-positive germs, 26 vs. 7.1 ng/ml (p = 0.001). Values > 26.8 ng/ml predict bacteremia by Gram-negative bacilli (AUC: 0.81). Conclusions: A cutoff value of PCT > 15.5 ng/ml is a predictor of bacteremia (AUC: 0.76) and values > 26.8 ng/ml predict bacteremia by gram-negative bacilli (AUC: 0.81).
Las infecciones siguen siendo una importante causa de morbimortalidad en los pacientes oncológicos, ocasionando retraso en la continuidad del tratamiento y aumentando los costos en salud. Pueden ser producidas por gérmenes que ganan acceso a sitios estériles a través de las barreras epiteliales alteradas por la quimioterapia y la inmunosupresión; con una mortalidad que varía entre el 14 y 42% según diferentes estudios. Algunos microorganismos, no patógenos en otras condiciones, se convierten en patógenos, desencadenando bacteriemias y disfunción multiorgánica severa, como es el caso que presentamos a continuación.
