Introducción: Fibroma Osificante Periférico, una lesión pseudotumoral, considerada dentro de las hiperplasias reactivas simples, las cuales en general tienen una causa definida y son, con frecuencia reversibles. Objetivo: Presentar un caso clínico de una lesión en un paciente, que se manifestó como lesión radiolúcida y asintomática. Su diagnóstico diferencial, el cual está basado en las manifestaciones clínicas y en el estudio radiográfico convencional, es controvertido. Descripción del caso: Se presentó el caso clínico en un paciente de género masculino con 51 años de edad, con una lesión localizada en la zona incisivo-canino del maxilar, cuyos aspectos eran sugestivos de granuloma telangiectásico. Previa valoración clínica y radiográfica, se sometió al paciente a resección quirúrgica de la lesión y fue enviada para un examen histopatológico, evidenciando material osteoide compacto, revelando así el diagnóstico final, fibroma osificante periférico. Conclusión: El profesional odontólogo, debe poseer conocimiento y la capacidad de manejo clínico de patologías bucales, para emitir un correcto diagnóstico y tratamiento, para ello es imprescindible la realización de una biopsia, para evitar posibles controversias entre el diagnóstico clínico e histológico.
