Objetivo: determinar el riesgo de desarrollar diabetes mellitus tipo 2 y prediabetes por medio del cuestionario FINDRISK en pacientes entre 35 y 75 años que asisten a consulta externa. Métodos: estudio descriptivo transversal, aplicando el cuestionario FINDRISK validado para Colombia. A aquellos con puntajes igual o mayor a 12 se les investigaron marcadores glicémicos y perfil lipídico. Con los datos obtenidos se realizó análisis uni y bivariado, además de correspondencias múltiples. Resultados: de 796 participantes 11% (n= 88) presentó prediabetes y 0,7% (n=6) diabetes, 36,8% (n=293) tuvo un FINDRISK igual o mayor de 12, de estos a 30% se le diagnosticó prediabetes y 2% diabetes; el 54,9% cursó con hipertensión arterial, 50% con historia familiar de diabetes, 43% antecedentes de dislipidemia, 86% perímetro abdominal mayor de los rangos establecidos y dislipidemia en más de 50%. Conclusión: el cuestionario FINDRISK es una herramienta útil en la detección de estados prediabéticos, y diabetes, favoreciendo el manejo temprano y la prevención de complicaciones. Además, aquellos con trastornos glicémicos y puntaje FINDRISK mayor o igual a12 muestran tendencia acursar con dislipidemia, lo que indica la necesidad del estudio metabólico completo.
Introducción: el consumo de metformina se asocia con déficit de vitamina B12. Objetivo: identificar las características clínicas predictoras del déficit en mayores de 18 años con diabetes mellitus tipo 2 (DM2) tratados con metformina. Materiales y métodos: estudio de corte transversal analítico en 100 pacientes entre 50 y 85 años con DM2 tratados con metformina por más de 3 meses, con registro de niveles de vitamina B12 en la historia clínica, atendidos en un programa de diabetes de medicina familiar en Bogotá DC, Colombia. Resultados: la media de duración de la enfermedad fue 9.6 años, el uso de metformina varió entre 1 y 5 años (32%), la dosis más utilizada estuvo entre 1001 y 2000 mg (65%), polifarmacia en 45% y la prevalencia del déficit en 27%. En el modelo de regresión logística se encontró que el tiempo de uso se comporta como factor predictor de déficit de vitamina B12 (OR=0,01 IC95% 0,01-0,03) (p<0,05), la polifarmacia (OR=1.21 IC95% -0,06-2,5) y la duración de la diabetes (OR=1.14 IC95% 0,99-1,32) emergen como factores predictores, pero sin diferencia estadísticamente significativa. Conclusión: el tiempo de uso de metformina es una característica clínica que puede ser predictora del déficit de vitamina B12, la prevalencia del déficit en nuestro estudio fue alta, consideramos recomendable realizar una búsqueda activa en la práctica clínica.
Introducción: la herramienta FINDRISC permite calcular el riesgo de desarrollar diabetes con punto de corte para Colombia de 12. Existe evidencia de que el riesgo cardiovascular se incrementa a medida que lo hace el puntaje, pero en Colombia no existe información cuando es ≥ 12. Objetivo: establecer el riesgo cardiovascular (RCV) en pacientes con FINDRISK-C ≥ 12 mediante score Framingham ajustado para Colombia. Materiales y métodos: subanálisis transversal retrospectivo en 796 pacientes a quienes se les aplicó el cuestionario FINDRISC-C, de ellos 293 con puntaje ≥ 12 y 262 cumplieron los criterios de elegibilidad. Antes se les calculó el RCV mediante análisis uni y multivariado, significancias estadísticas y análisis de correspondencias múltiple. Resultados: 262 pacientes, 63% mujeres, 87% tuvieron sobrepeso y obesidad, promedio de perímetro abdominal 97 cm, 10% eran fumadores y 48% tenían antecedente familiar de diabetes mellitus tipo 2. Se encontró una media de RCV de 8,10 (IC 7,29-8,91), al estratificar por FINDRISC-C la media para cada una de las categorías fue: FINDRISC-C moderado 7,83; FINDRISC-C alto 7,87, FINDRISC-C muy alto 12,61. La prevalencia de dislipidemia fue de 46,2 % (IC 95%: 40-50) siendo mayor en hombres (53,6%). Conclusión: los pacientes con FINDRISC-C ≥ 12 tienen un RCV entre moderado y alto, existiendo tendencia al incremento del porcentaje de riesgo calculado según score Framingham ajustado para Colombia, a medida que aumenta el puntaje FINDRISC-C. La prevalencia de dislipidemia en pacientes con FINDRISC-C ≥ 12 fue elevada.
