Introdução: Embora o conhecimento acerca da Síndrome da Sela Vazia seja bem vasto e documentado na literatura, existem poucos trabalhos esclarecendo os procedimentos aplicados em casos relatados, as dificuldades diagnósticas e de técnicas da intervenção, além dos resultados obtidos. Objetivo: Relatar o caso de um paciente jovem portador da Síndrome da Sela Vazia com herniação de giros retos, através da revisão retrospectiva do prontuário físico, abordando aspectos clínicos e cirúrgicos. Método: Foi realizado uma revisão retrospectiva do prontuário físico do paciente. Relato de caso: Paciente, 17 anos, apresentou-se inicialmente com sintomas de origem endócrina característicos de hipocortisolismo e hipogonadismo. Através do exame de imagem foi notada a presença de sela túrcica sem o habitual conteúdo hipofisário, mas com a presença de herniação de giros retos. Foi indicado tratamento cirúrgico e, com isso, houve melhora significativa no quadro de hipotensão, sendo necessário associar ao tratamento terapia de reposição hormonal. Discussão: A Síndrome da Sela Vazia (SSV) é caracterizada pela herniação da membrana aracnóide para dentro da sela túrcica, que pode atuar com efeito de massa, levando a falência da função pituitária O quadro clínico dependerá do fator etiológico, sendo necessário geralmente reposição hormonal para esses pacientes e em alguns casos a intervenção cirúrgica. Conclusão: A SSV é uma patologia importante que deve estar sempre entre os diagnósticos diferenciais em pacientes com clínica de hipogonadismo.
Introdução: a comunicação humana pode ser verbal, quando expressa por escrita e fala, ou não verbal, quando envolve o comportamento não expresso por palavras, como gestos, silêncio, expressões faciais e postura corporal. Estudos demonstram que 55% dos sentimentos são expressos através da comunicação não verbal, 38% pela voz e apenas 7% por palavras. a partir desse dado, pode-se inferir que a comunicação não verbal é de grande relevância para a relação médico-paciente, podendo fortalecer ou enfraquecer o vínculo de confiança entre eles e influenciar o sucesso do plano terapêutico e a satisfação do paciente quanto ao atendimento. a coerência e a complementariedade da comunicação verbal e não verbal também merecem destaque importante nessa relação, pois o conflito entre as duas mensagens pode dificultar a comunicação. o estudo da comunicação não verbal e a conscientização e o estímulo à reflexão de estudantes de medicina para a importância do tema na relação médico-paciente são necessários para o desenvolvimento de um atendimento mais humanizado e de melhor qualidade. Objetivos: Identificar que elementos da comunicação não verbal são mais relevantes para o paciente. Detectar quais gestos específicos que os médicos fazem que provocam um aumento da eficácia da comunicação médico-paciente e uma melhor qualidade do atendimento. Conscientizar os estudantes de medicina para a importância da comunicação não verbal na relação médico-paciente. Métodos: a pesquisa é do tipo exploratória-qualitativa e está sendo realizada em um complexo hospitalar universitário, contemplando pacientes que estão internados em enfermarias, recebendo atendimento ambulatorial e mães internadas em puerpério imediato. Os dados estão sendo coletados por intermédio de entrevista estruturada, após o consentimento do paciente. Resultados: Foram destacados, até o presente momento, alguns elementos da comunicação não verbal que melhoram a qualidade da consulta, como: dar atenção ao paciente e aos familiares, fazer o exame físico demonstrando que se importa com o paciente, saber interagir com o paciente e com os familiares, demonstrar apoio ao paciente diante de suas angústias e utilizar um tom de voz acolhedor. Atitudes como olhar, conversar e ser educado também foram salientados e associados a um comportamento que fez diferença para o atendimento. Conclusões: a comunicação não verbal é um elemento da comunicação que muitas vezes não recebe a devida atenção e importância pelos médicos e por outros profissionais de saúde. a percepção do paciente sobre o comportamento e as atitudes do médico é apurada e relevante para a consolidação da confiança no médico e para o estabelecimento de uma boa relação médico-paciente. Ter consciência desse complexo processo que é a comunicação humana durante a formação acadêmica auxilia compreender com maior clareza o ser humano e permite oferecer um atendimento de melhor qualidade.
RESUMOO schwannoma é uma neoplasia de células de Schwann a qual pode ocorrer em diversas partes do sistema nervoso, mas corresponde a uma pequena parcela dos tumores de cauda equina, sendo incomum o acometimento ósseo. Relatamos o caso de um paciente masculino, 49 anos, o qual apresentou quadro álgico com redução de força. Exames de imagem revelaram tumor na região da cauda equina causando compressão nervosa e erosão do corpo vertebral de L3. Os achados histopatológicos corroboram diagnóstico de schwannoma e o tratamento cirurgico acarreta resolução do quadro. Palavras-chave:Coluna vertebral. Neoplasias. Neurocirurgia. ABSTRACTThe schwannoma is a Schwann cell neoplasm that can occur in many regions of the nervous system, although it accounts for a small portion of the cauda equina tumors being uncommon the bone involvement. We report the case of a male patient, 49 years old, presenting algic condition with strength reduction. Imaging revealed a cauda equina tumor causing nerve compression and L3 of the condition.
