Background: Peritoneal dialysis is a clinical procedure performed to assist functions of the failing kidney. Its application in modern clinical practice is gradually curtailed by hemodialysis, a system with a robust technology. Case Report: This case describes an infant with primary urolithiasis whose parents refused for any medical evaluation until the infant came to medical attention again at seven months of age in a tertiary medical center in northern Malaysia. At that point, his left kidney was already non-functional. He presented with severe acute-on-chronic renal failure with profound electrolyte imbalances resulting in status epilepticus and cardiac dysrhythmia. He was contiguously planned for further diagnostic and therapeutic surgical intervention but due to his unstable condition, he was not able to be transferred to a referral center for the definitive care. Peritoneal dialysis was subsequently done in our center and delivered an excellent outcome. The patient was later safely transferred for definitive care. Conclusion: Peritoneal dialysis proves to be a cheap, safe and cost-effective method in the management of severe acute kidney injury in an infant with a single functioning kidney. Despite being a rudimentary technique, peritoneal dialysis proves beneficial in situation where pediatric surgical expertise is lacking.

ObjectiveTo estimate stature from the sternal length in American white population using linear regression equation.RationaleThere is an extensive study to correlate the stature from long bones and sternum in the literature in the Asian continent population. However these studies are sporadic for white and Hispanic American population. In this study we used regression analysis to address the reliability of sternal length for estimation of stature in White American population.Material and MethodsThis preliminary research project was conducted at Touro College of Osteopathic Medicine during the academic year 2018–2019 and 21 female and 14 male cadavers from the Department of Anatomy were used for the study. The research project was approved by the Institutional research committee. Cadavers with any noticeable physical anomaly affecting the stature were excluded from the study. The age and ethnicity of the cadavers were obtained by the departmental records. Stature was measured using a steel tape from Heel to Vertex. After the reflexion of the skin, the length of the sternum was measured from incisura angularis to mesoxiphoid point using a digital Vernier Caliper. Regression equations were derived using SPSS statistical software.ResultsRegression equation for female: Y= 135.039 + (0.146) X where Y= stature and X= length of the sternum. Regression equation for male: Y=141.603 + (0.17) X where Y= stature and X= length of the sternum. The SD was 13.88, 16.21 and P value was 0.18, 0.12 respectively for females and males.ConclusionBased on the calculations and the P value, we concluded that the length of the sternum is not a reliable factor for stature estimation for American white population. Background: Benign prostatic hyperplasia (BPH) is a non malignant enlargement of the prostate. This study was conducted to determine the pattern of presentation of BPH with and without acute urinary retention (AUR) and to determine the risk factors for acute urinary retention, in addition, to assess the results of doxazosin in the treatment of BPH with and without AUR. Patients and methods: Ninety two patients were enrolled in this study. All patients presented to the emergency department and the referred clinic of the Gezira Hospital for Renal Disease and Surgery (GHRDS) with lower urinary tract symptoms with and without acute urinary retention suggestive of BPH were evaluated according to the European guidelines. All of them underwent trans-rectal ultra sound and measurement for the prostatic volume. Serum PSA level was estimated. Doxazosin was given to all patients and they were followed for twelve weeks. A trial without a catheter at the end of the first week was attempted for patients with acute urinary retention. Results: The patients' mean age ± SD at baseline was 67.8 ± 7.7 and 69.4 ± 9.9 for Non-AUR/BPH and AUR/BPH respectively. Comparing baseline clinical parameters of patients with and without acute urinary retention revealed that IPSS, prostate volume and PSA level are significantly different between the two groups with P< 0.01, 0.01 and 0.00 respectively. In the TWOC, 65% of patients passed urine spontaneously. By the end of the study, the overall response of patients showed 78.9% successful rate (n=73) and 21.1% failure rate (n=19). Prostatic volume and PSA level were found to affect doxazosin treatment significantly. With cutoff values of 41cc and (3.45) ng/ml, prostatic volume and PSA level influenced doxazosin treatment failure. Conclusion: Serum PSA and prostate volume are powerful predictors of the risk of AUR. Alpha blockers are efficacious in treating retaining and non-retaining BPH patients. Knowledge of baseline serum PSA and/or prostate volume are useful tools to aid physicians and decision makers in predicting the risk of BPH-related outcomes and choosing dexazosin as therapy for BPH.
