POAF was diagnosed in 21% of post-CABG patients, and the major predictors were: age ≥ 70 years, preoperative stable angina, as well as low cardiac output syndrome following CABG.
Rewaskularyzacja chirurgiczna stanowi uznaną metodę leczenia choroby niedokrwiennej serca. Stale zwiększa się liczba osób poddawanych pomostowaniu aortalno-wieńcowemu, zarówno w populacji młodych pacjentów z chorobą wieńcową, jak i chorych w podeszłym wieku. Szacuje się, że nawet co trzeci chory poddawany pomostowaniu aortalno-wieńcowemu w okresie pooperacyjnym może doświadczyć objawów depresyjnych. Liczne czynniki osobnicze i czynniki związane z zabiegiem operacyjnym wpływają na wystąpienie depresji. Do najczęściej wymienianych należą: wiek, płeć, sytuacja socjoekonomiczna, schorzenia współistniejące oraz występowanie depresji przed operacją. Obecnie poszukuje się markerów biochemicznych, których stężenie przed zabiegiem byłoby predyktorem wystąpienia depresji po pomostowaniu aortalno-wieńcowym. Sugeruje się związek odpowiedzi zapalnej, szczególnie nasilonej w okresie okołooperacyjnym, z występowaniem depresji po rewaskularyzacji kardiochirugicznej. Identyfikacja tych czynników jest niezwykle istotna, gdyż umożliwi opracowanie stratyfikacji pozwalającej na wytypowanie chorych szczególnie zagrożonych wystąpieniem depresji po operacji. Działania mające na celu identyfikację zagrożonych chorych są niezwykle istotne, ponieważ depresja nie tylko pogarsza jakość życia, ale także wpływa na rokowanie krótko- i długoterminowe.
The most common concomitant conditions in the elderly patients undergoing cardiac surgery are hypertension and impaired renal function. The majority of operated patients already suffered a MI. AF and low cardiac output syndrome are the most common postoperative complications. Among patients above 80 years of age, operative mortality risk predicted using the EuroSCORE I and EuroSCORE II may be overestimated. Patient selection for cardiac surgery must be based on individual factors, taking into account the feasibility of postoperative rehabilitation and the potential for improved survival and quality of life.
Doktor nauk medycznych Kamil Michta jest absolwentem I Wydziału Lekarskiego Warszawskiego Uniwersytetu Medycznego (2009 r.). Jego rozprawa doktorska (promotor: prof. dr hab. n. med. Beata Wożakowska-Kapłon, promotor pomocniczy: dr n. med. Edward Pietrzyk) dotyczyła pooperacyjnego migotania przedsionków u chorych po izolowanym pomostowaniu aortalno-wieńcowym. Doktor Michta, obecnie w trakcie specjalizacji z kardiochirurgii, jest autorem lub współautorem 14 artykułów (5 oryginalnych, 3 poglądowych, 6 kazuistycznych), z łącznym dorobkiem naukowym IF 4,365 punktu, MNiSW 128 punktów. W kręgu jego zainteresowań medycznych pozostają metody rewaskularyzacji wieńcowej, operacji wad zastawkowych oraz tętniaków aorty wstępującej i łuku aorty. Pracuje w Klinice Kardiochirurgii Świętokrzyskiego Centrum Kardiologii w Kielcach (Kierownik: dr n. med. Edward Pietrzyk). Jest to referencyjny 15-łóżkowy ośrodek wykonujący operacje kardiochirurgiczne w zakresie pomostowania aortalno-wieńcowego, leczenia wad zastawkowych, tętniaków aorty wstępującej i łuku aorty, wad wrodzonych i nabytych serca, ubytków w przegrodach międzykomorowych i międzyprzedsionkowych, urazów oraz guzów serca. Wolny czas dr Michta poświęca realizowaniu pasji motoryzacyjnej, w szczególności sportom motocyklowym, a także kolarstwu górskiemu i szosowemu oraz jeździectwu.
A 62-year-old woman was admitted to our hospital because of increasing dyspnoea, limited exercise tolerance and a dry hacking cough. She denied upper respiratory tract infection. The patient had been regularly treated for severe hypertension for the previous 20 years. The patient was admitted in an unnstable condition. Her physical examination revealed aortic murmur and tachycardia, and blood pressure was increased. Transthoracic echocardiography revealed communication between ruptured aneurysm of the noncoronary sinus of Valsalva and the right atrium. The diagnosis was confirmed in transoesophageal echocardiography (Fig. 1). The patient was operated upon with cardiopulmonary bypass, hypothermia (30°C) and cardioplegic arrest. Intraoperatively, we found ruptured aneurysm of the noncoronary sinus of Valsalva with fistulous tract between the noncoronary sinus and the right atrium (Fig. 2). Surgical findings also included atrial septal defect type secundum. The fistula was closed with plasty of the sinus of Valsalva (Dacron 3 × 2 cm) and the atrial septal defect was repaired (Fig. 3). The early postoperative period was uneventful. The patient was discharged from the hospital in good condition on the ninth day of treatment. Aneurysm of the sinus of Valsalva is a rare cardiac anomaly. It is usually an asymptomatic disease, but when it ruptures, symptoms appear and the patient's condition deteriorates rapidly. This aneurysm ruptured in the right atrium, causing haemodynamic complications. The diagnosis was obtained by transthoracic and transoesophageal echocardiography. The prognosis of ruptured aneurysm of sinus of Valsalva is always serious and surgery is the treatment of choice. A left-to-right shunt to the right atrium may cause an increase of haemodynamic loading of the heart, and the patient will require urgent surgery. This was indeed the treatment used for our patient.
