Neue molekularbiologische und genetische Erkenntnisse ermöglichen eine genauere Klassifikation zystischer Nierenerkrankungen. ᭤Vererbte zystische Nierenerkrankungen ᭤Zystenniere Typ I nach Potter ᭤Inzidenz Der autosomal-rezessiven polyzystischen Nierenerkrankung liegt eine Mutation im Chromosom 6 (6p21-cen) zugrunde. Sie entspricht der Zystenniere Typ I nach Potter. Zystische Nierenerkrankungen stellen eine heterogene Gruppe mit verschiedenen Ursachen und unterschiedlicher klinischer und funktioneller Bedeutung dar. 10% aller Patienten mit dialysepflichtiger terminaler Niereninsuffizienz leiden an zystischen Nierenerkrankungen. Molekularbiologische Techniken haben in den letzten Jahren zur Aufklärung der Pathogenese zahlreicher genetisch bedingter Nierenerkrankungen geführt [7]. Eine hilfreiche klinische ᭤ Klassifikation der zystischen Nierenerkrankungen sollte genetische und morphologische Befunde einbeziehen. Mikrodissektionsstudien haben wertvolle Informationen über mögliche Entstehungsmechanismen der Zystenbildung erbracht und stellen die Basis der Potter-Klassifikation dar [17]. Jedoch entspricht diese aufgrund der rein morphologischen Befunde nicht allen klinischen und genetischen Formen. Hieraus resultieren teilweise widersprüchliche und verwirrende Nomenklaturen. Ziel dieses Kapitels ist die Vorstellung einer klinisch praktikablen Einteilung zystischer Nierenerkrankungen in drei ätiologisch unterschiedliche Gruppen (Tab.1). Vererbte zystische Nierenerkrankungen Autosomal-rezessiv erbliche polyzystische Nierenerkrankung Die synonym als ᭤ Zystenniere Typ I nach Potter bezeichnete Erkrankung ist charakterisiert durch eine zystische Dilatation der Sammelrohre beider Nieren, dysgenetische Gallengangsveränderungen und portale Fibrose [23]. Veraltete Begriffe wie "infantile polyzystische Nierenerkrankung" sollten nicht mehr verwendet werden, da unterschiedliche klinische Verlaufsformen mit Manifestationen auch im späten Kindesalter auftreten können [23, 26]. Die ᭤ Inzidenz liegt zwischen 1:10.000 und 1:40.000 Neugeborenen [3]. Die typischen Merkmale dieser autosomal-rezessiven Erkrankung sind die fehlende Symptomatik Heterozygoter, eine fehlende Geschlechtsbevorzugung und ein Wiederholungsrisiko für weitere Geschwister in 25%. Zerres u. Mitarb. gelang es durch Koppelungsanalyse, einen Genort für diese Erkrankung auf dem Chromosom 6 in der Region 6p 21-cen zu kartieren [27], ohne daß ein Hinweis auf Genlokusheterogenität vorliegt. Dies erlaubt eine genauere pränatale Diagnostik. Voraussetzung hierfür ist jedoch die zweifelsfreie klinische Diagnosestellung, die in jedem Fall einen normalen elterlichen Ultraschallbefund der Nieren voraussetzt.
