Мета дослідження -оптимізація самостійної роботи студентів при вивченні курсу "Медична хімія". Матеріали та методи дослідження. Розробка нових методичних матеріалів відбувається на основі ознайомлення з сучасною вітчизняною та закордонною періодичною літературою, підручниками, електронними ресурсами. На основі аналізу одержаної інформації здійснюється вибіркове впровадження новітніх методів викладання.Результати й обговорення. Навчальний процес з курсу "Медична хімія" формується на основі типової навчальної програми і включає лекції, практичні і лабораторні заняття, семінари.Відповідно до вимог кредитно-трансферної системи значна частина навчального навантаження для студентів покладена на самостійну роботу. Самостійна робота студентів формує у майбутніх лікарів звичку до самоосвіти як форму вдосконалення знань, життєву необхідність для якісної професійної діяльності. Основною умовою успішного процесу самоосвіти є достатній рівень початкових знань.З метою оптимізації самостійної роботи студентів на кафедрі розроблена методична література -навчально-методичні по-сібники для студентів і викладачів, розробки для позааудиторної самостійної роботи студентів, збірники тестів, практичних завдань із лабораторним практикумом, збірники завдань і вправ, збірники лекцій, аудіо-, відео-та мультимедійні матеріали. Всі зазначені види розробок пропонуються студентам трьома мовами: українською для вітчизняних студентів, російською та англійською для іноземних студентів. Електронні варіанти методичних розробок представлено на сайті кафедри.На кафедрі використовують кілька форм опитування і контролю самостійної роботи студентів: -усне обговорення теоретичного матеріалу на практичних заняттях; -контроль практичних навичок при виконанні лабораторних робіт; -поточне тестове опитування з метою контролю засвоєння базового матеріалу; -обговорення рефератів, підготовлених студентами за темами, запропонованими для самостійного освоєння; -семінарські заняття. Найвищий рівень самостійності в освоєнні і, що найголовніше, творчому використанні теоретичного матеріалу студент може проявити при підготовці рефератів. Кращі реферати виставляються на сайті кафедри та беруть участь у кафедральному конкурсі.Висновок. Представлена система мотивації студентів до виконання самостійної роботи при вивченні курсу "Медична хімія" є ефективною та активно використовується співробітниками кафедри.Ключові слова: самостійна робота; мотивація; оцінювання; методичні розробки.The aim of the study -to optimize unsupervised work of students when they learn the "Medical Chemistry" course. Daytime sleepiness is one of the clinically significant non-motor manifestations of Parkinson's disease (PD). One of its insufficiently studied aspects is a relationship between daytime sleepiness and nighttime sleep disorders.Objective:to clarify the clinical characteristics of PD in patients with different types of daytime sleepiness and to estimate of the ratio of daytime sleepiness to clinical and polysomnographic characteristics of nighttime sleep in patients with advanced stages of PD.Patients and methods. The investigation included 110 patients (56 men and 54 women) (mean age, 63.78+0.6 years) with PD (Hoehn and Yahr stage 2.6+0.2; disease duration, 6.3+3.2 years) without dementia. All the patients received therapy with levodopa at a mean daily dose of 667.8 mg; 98 of them had the drug in combination with dopamine receptor agonists at a stable dose. The unified PD rating scale, the PD sleep scale (PDSS), and the Epworth sleepiness scale (ESS) were applied. Nocturnal polysomnography (PSG) and the multiple sleep latency test (MSLT) were performed.Results and discussion. There was daytime sleepiness in 44% of the patients: permanent sleepiness in 15%, sudden daytime sleep attacks (along with low daytime sleepiness (ESS) in 14%, and permanent drowsiness concurrent with sleep attacks in 15%. The PSG findings showed a decrease in sleep efficiency, an increase in the duration of the first stage of sleep, a reduction in the duration of the second and third sleep stages, an extension of rapid eye movement (REM) sleep latency, and frequent awakenings (sleep fragmentation). PSG also demonstrated REM sleep behavior disorders (RBD) in half of the examinees.Patients with sleep attacks differed from those with permanent drowsiness without sleep attacks with more severe sleep disorders (PDSS) and shorter sleep latency (MSLT). Patients with the RBD phenomenon had shorter sleep latency (MTLS) than those without this parasomnia. Patients with moderate or severe sleepiness (ESS scores of >10) differed from those with milder drowsiness (ESS scores of 410) and a lower representation of the third sleep stage.Conclusion.There is evidence for the association of daytime sleepiness in PD with reduced efficiency, changes in the nighttime sleep pattern, and RBD.
Objective: non-interventional study ELBRUS (Etoricoxib in the Treatment of Back Pain) was conducted to investigate the efficacy and safety of daily administration of Rixia® (Etoricoxib) 60 mg per day in patients with chronic non-specific low back pain (CNSLBP).Patients and methods. The study included 50 patients (31 women and 19 men, mean age 54.3±16.8 years) with CNSLBP. Educational conversation, cognitive therapy, regular therapeutic exercises, identification and treatment of comorbidities were conducted. Patients received etoricoxib 60 mg once daily. Pain intensity was assessed on a 10-point numerical rating scale (NRS), pain-related disability was assessed on the Oswestry Scale (ODS), and emotional state was assessed on the Hospital Anxiety and Depression Scale (HADS).Results and discussion. The causes of CNSLBP were: in 5 (10%) patients – sacroiliac joint involvement, in 14 (28%) – lower lumbar facet joints involvement, in 3 (6%) – myofascial pain, the remaining 28 (56%) patients had a combination of several reasons. As a result of complex treatment, the intensity of pain at rest decreased on average from 4.0±2.5 to 1.4±1.3 points, while moving – from 6.6±1.9 to 2.8±1.8 points, at night – from 2.7±2 to 0.7±0.9 points according to the NRS, disability – from 39±18.9 to 19.9±14.6% according to the ODS, the severity of anxiety – from 6.5±3.9 to 3.3±2.4 points and depression from 5.0±3.7 to 3.1±2.9 points according to HADS (p<0.001). The duration of treatment was 14.14±3.6 days on average. No adverse events were observed during treatment with etoricoxib. Conclusion. The efficacy and safety of etoricoxib in the complex therapy of patients with CNSLBP was noted. Sudden-onset sleep (SOS) is one of the manifestations of excessive daytime sleepiness (ЕDS) in Parkinson’s disease (PD).Aim: сlarify the clinical and polysomnographic characteristics of patients with SOS at the expanded (2 and 3 stages according to Hoehn–Yahr) stages of PD.Material and methods. 42 patients (18 men and 24 women, average age 66.5 ± 7.9 years, duration of PD 8.9 ± 4.6 years, Hoehn–Yahr stage of PD 2.6 ± 0.5) who received combined dopaminergic therapy with levodopa and dopamine agonist were examined. 32 patients had ЕDS, 10 patients did not have it. The Unified Parkinson’s disease rating scale (UPDRS parts II, III in “on” state of dopaminergic medication), the diary of the assessment of the waking period, the Epworth Sleepiness Scale, the Parkinson Disease Sleep Scale I (PDSS I), the Sсales for Outcomes of Parkinsons disease-Cognition (SCOPA-Cog), the Beck Depression Inventory, the State-Trait Anxiety Inventory, the Apathy Scale, the Stroop test. Objective sleep-wake assessment: standard nocturnal video-polysomnography (PSG), multiple sleep latency test (MSLT).Results. SOS was detected in 69% of patients with complaints of ЕDS. Patients with SOS had a shorter latency of falling asleep according to MSLT data (8.1 ± 5.9 min versus 14.2 ± 3.9 min in patients without of ЕDS, p < 0.05).Patients with SOS had a high severity of motor function disorders, apathy and depression (p < 0.05). Рatients with SOS had no significant differences in the main PSG characteristics of night sleep.Conclusion. SOS can be considered as one of the manifestations of a more severe course of PD. A decrease in the effectiveness of night sleep and a change in its structure are not a determining factor in the development of SOS.
