Zusammenfassung Hintergrund: Die Symptomatik der Schizophrenie ist vielgestaltig und kann sowohl interindividuell als auch intraindividuell stark variieren. Eine dimensionale Betrachtung der Symptomgruppen hat sich insgesamt als erfolgreich durchgesetzt, mit einem Vorteil für komplexere Modelle. Allerdings lässt sich trotz der differenzierten Erhebung selten ein direktes neuronales Korrelat mit Methoden der modernen Hirnforschung erstellen. Methode: Neben der Diskussion dimensionaler Modelle stellen wir hier eine neuartige, neurobiologisch informierte Psychopathologie vor. Abgeleitet aus den Kerneinschränkungen in der Kommunikation und den zugrundeliegenden Hirnsystemen haben wir ein dreidimensionales Modell psychotischer Erkrankungen erstellt. Hierbei werden Sprache, Affekte und Motorik abgebildet. Ergebnis: Untersuchungen an verschiedenen Stichproben etablierten die Dimensionsstruktur und zeigten deutliche Zusammenhänge zwischen veränderter Neuroanatomie und Symptomausprägung. Ziel wäre es, mit dem Wissen über die Pathomechanismen präzise Interventionen für die geeigneten Patienten zu entwickeln.
