Gemessen an ihrem häufigen Vorkommen werden Divertikel selten symptomatisch. Die Mehrheit der Patienten (75 bis 80 %) bleibt asymptomatisch und wir sprechen von einer Divertikulose. Führen Kolondivertikel zu Beschwerden, liegt eine Divertikelkrankheit vor. Diese hat im Wesentlichen zwei Komplikationen, nämlich die Divertikelblutung (25 % der Fälle) und die Divertikulitis (75 % der Fälle). Etwa jeder vierte Patient mit einer Divertikulitis hat einen komplizierten Verlauf. Die häufigsten Komplikationen sind der Abszess, die Fistelbildung, die Stenosierung oder sogar die freie Perforation des Divertikels in die Bauchhöhle. Die chronische Divertikelkrankheit zeichnet sich durch eine Lumeneinengung aus. Diese ist Folge der Hypertrophie der Muscularis propria einerseits und narbiger Veränderungen nach durchgemachten, oft subklinisch verlaufenden wiederholten Entzündungsschüben andererseits. Diese Stenosierung trägt wiederum zur Auslösung neuer Divertikulitisschübe bei (• " Abb. 1). Einleitung Erstmals wurde die akute Divertikulitis durch Virchow 1853 als Perisigmoiditis beschrieben. Eine vollständige Beschreibung erfolgte 1917 durch Telling. Seit Anfang der 90er-Jahre des letzten Jahrhunderts wurde die Sonografie zunehmend zur Methode der Wahl zum Nachweis der akuten Divertikulitis. Für das geübte Auge ist auch eine Differenzierung von einfachen und komplizierten Verlaufsformen mög-lich. EpidemiologieIn industrialisierten Ländern werden Divertikel in weniger als 10% bei unter 40-Jährigen, in 30% bei über 50-Jährigen, in 50% bei über 70-Jährigen und zu 66 % bei über 85-Jährigen gefunden, geschlechtsspezifische Unterschiede bestehen nicht. In ländlichen Gebieten Afrikas und Asiens sind die Kolondivertikel auch heute noch selten. Die Divertikulitis war bis 1920 ein seltenes Krankheitsbild und erfährt in den industrialisierten Ländern eine starke Zunahme seit ihrer ersten Beschreibung [10]. AnatomieDivertikel sind Ausstülpungen der Darmwand, die entweder angeboren sind und sämtliche Wandschichten enthalten ("echte Divertikel") oder erworben werden, indem sich eine Mukosahernie ausbildet ("falsche Divertikel") (• " Abb. 2). Im Dünndarm finden sich vor allem echte Divertikel (Duodenum 20 %, Jejunum 0,5 bis 1 % der Bevölkerung). Im Dickdarm entwickeln sich mit zunehmendem Lebensalter vor allem im Sigma und im Colon descendens falsche Divertikel. In Europa und den USA ist das Sigma zu 95 % der Fälle beteiligt und in 50 % der Fälle der allein betroffene Darmabschnitt. In nur 15 % der Fälle findet sich ein Befall des rechtsseitigen Kolons.
