451Настоящий обзор посвящен теме впервые возникшей фибрилляции предсердий (ФП) в условиях острого коронарного синдрома (ОКС). Использованы источники литературы баз PubMed, Scopus. ФП часто встречается в общей популяции и в популяции ОКС, при этом выделяют впервые возникшую ФП, и ФП, бывшую на момент развития ОКС. У пожилых пациентов с хронической сердечной недостаточностью, явле-ниями гемодинамической нестабильности и дилатацией левого предсердия возникновение ее более вероятно. Хорошо известно о предик-торах неблагоприятного исхода ОКС. Согласно некоторым данным, впервые возникшая ФП приводит к ухудшению прогноза, по другим данным причина ее сочетания с высоким уровнем летальности обусловлена тяжестью протекания ОКС, и возникновение ее не является не-зависимым предиктором смерти. Отсутствие субъективных симптомов во время пароксизма ФП не позволяет оценить его продолжительность, и тем самым дифференцировать впервые возникший пароксизм и постоянную или персистирующую форму ФП. Удается проследить связь между ишемией миокарда и возникновением ФП. Неэффективная реперфузия во время проведения чрескожного коронарного вмеша-тельства или тромболитической терапии сопровождается возникновением фибрилляции предсердий, напротив, при достижении эффек-тивной проходимости сосуда она возникает реже. Впервые возникшая при ОКС, и, в частности, сопровождающаяся кардиогенным шоком, ФП требует экстренной кардиоверсии: медикаментозной или электроимпульсной. В ряде случаев проведение активной кардиоверсии не требуется, поскольку восстановление синусового ритма происходит спонтанно. Неоднозначность и многофакторность данной проблемы указывает на необходимость более детального понимания механизмов аритмогенеза и разработки инструментов для стратификации рисков возникновения ФП при ОКС.Ключевые слова: фибрилляция предсердий, острый коронарный синдром, левое предсердие, асимптомная фибрилляция предсердий, чрескожное коронарное вмешательство, кардиоэмболический инсульт. The present review is focuses on new onset atrial fibrillation (AF) in conditions of acute coronary syndrome (ACS). Literature sources from PubMed and Scopus was used. AF is common in the general population and in the ACS population, at that new onset AF, and AF, which was by the time of ACS development are determined. Appearance of AF is more likely amongst the elderly patients with congestive heart failure, with signs of the hemodynamic instability and with the left atrium dilatation. It is well-known about the predictors of adverse outcome in ACS. According to some literature data new onset AF leads to worsening of prognosis, and in line with other sources a reason of its association with high level of mortality is due to the severity of ACS and appearance of AF is not independent predictor of death. The absence of subjective symptoms at the time of paroxysm of new onset AF does not allow estimating its duration and distinguishing between the new onset AF, persistent and constant AF. Установлено, что дети значительно легче переносят COVID-19. Исследователи отмечают преобладание у детей бессимптомных и легких форм заболевания. По данным лабораторных исследований, у детей крайне редко имела место выраженная лимфопения. Показано, что COVID-19 у детей имеет и существенно более благоприятный исход. Отмеченные особенности клинико-лабораторного течения COVID-19 у педиатрических пациентов, вероятно, связаны с целым рядом факторов, среди которых возрастные особенности иммунного ответа (в первую очередь — врожденного иммунитета), более здоровые дыхательные пути (не подвергавшиеся воздействию табачного дыма, частых респираторных инфекций), а также возрастные особенности функционирования ангиотензинпревращающего фермента-2, используемого коронавирусами в качестве клеточного рецептора. Особое внимание авторы обращают на то, что именно дети как категория пациентов с наиболее высоким уровнем бессимптомного и легкого течения заболевания составляют основной трансмиссивный потенциал для продолжения пандемии.
Failure to conceive and preterm birth are high risk factors for perinatal pathology. In the search for a means to reduce the rate of non-pregnancy, we observed in 30 pregnant women the tricolic effect of the combined use of ethanol and Alupent. Twenty women were treated after 30 weeks' gestation in an obstetric hospital, and 10 from 18 to 27 weeks' gynaecological hospital. No cause of termination of pregnancy was identified.
In everyday practice, the doctor rarely encounters only one specific disease, more often a patient with comorbid pathology comes to him. Such a group of concomitant diseases are mental disorders. Their prevalence in cardiology practice reaches 80%. Mood affective, anxiety and somatization disorders, as well as cognitive impairment are observed most often. The review looked at mental disorders that occur in cardiac diseases with the highest number of deaths, such as coronary heart disease, including myocardial infarction and cardiac arrhythmias, arterial hypertension and cerebrovascular diseases. Including attention is paid to the senile asthenia syndrome, which is accompanied by cognitive impairment, loss of previous vital interests and depression. The review highlights the questions of regular and adequate psychopharmacotherapy of cardiovascular diseases, which leads to a statistically significant decrease in the frequency of their exacerbations, which reduces the number of doctors who come to see for somatogenic symptoms, and also allows to improve the prognosis of the underlying disease and significantly reduce mortality. It was observed that the doctor should take into account the fact that modern cardiological preparations have effects that can cause side effects in the form of mental disorders when choosing a therapy. Understanding the processes of formation and occurrence of mental diseases in a patient with cardiovascular pathology, as well as methods for their correction, can increase the effectiveness of the therapy and improve the prognosis of the underlying disease.
