A relatively simple method for the three-dimensional reconstruction of brain contours from a series of CT scans is presented. A procedure is described for storing the structural information obtained from the CT series and for organizing and displaying the data. Furthermore, it is shown how additional features such as standardized artery information taken from an atlas can be superimposed on the spatially reconstructed brain model. It is proposed that three-dimensional representations in the form of computed stereo pairs are quite suitable for morphological documentation of specific neuropsychological issues, such as the localization of aphasic syndromes.

DefinitionIn der Psychiatrie werden Veränderungen von Antrieb und Affektivität sehr häufig gemeinsam und gleichsinnig beobachtet, z. B. als depressive Hemmung und traurige Verstimmung mit Einschränkung der affektiven Schwingungsfähigkeit oder als maniforme Antriebssteigerung mit gehobener, auch gereizter Stimmung. Wenn solche Veränderungen bei Patienten mit bekannter Hirnkrankheit auftreten, ist es schwierig und oft unmöglich, eine
ZusammenfassungEtwa 95 % der Auffahrunfälle sind als leicht bis mittelschwer einzustufen, über die biomechanischen Abläufe und die möglichen medizinischen Folgen dieser Unfälle werden in der Literatur und in der ärztlichen und gutachterlichen Praxis kontroverse Meinungen vertreten. Diese konnten sich bisher nur auf nicht überprüfte Vermutungen stützen. Inzwischen liegen aus mehreren Instituten unfallmechanische Untersuchungen von naturgetreuen Auffahrunfällen mit Zusammenprall von rückwärts, von vorne und von der Seite vor. Dabei wurden die Bewegungsabläufe mit einer Hochgeschwindigkeitsvideokamera unter gleichzeitiger elektronischer Erfassung anderer, wichtiger Parameter erfasst. Übereinstimmend hat sich gezeigt, dass die gesamte Crashphase etwa 3/10 s dauert und dass ein Schleudermechanismus tatsächlich nicht stattfindet. Das sog. Syndrom der Spätfolgen nach Schleudertrauma ist uncharakteristisch. Eine gleichartige Symptomatik wird bei verschiedenen Befindlichkeitsstörungen beobachtet, deren nosologische Stellung zweifelhaft ist. In vielen Fällen sind die somatischen, emotionalen und kognitiven Beschwerden Ausdruck von Depressivität. Der ¹zervikale Schwindelª ist nach Ansicht von Experten eine zweifelhafte Entität. ¹Zervikaler Nystagmusª ist auch bei Gesunden auslösbar. Viele der von einigen Autoren lebhaft propagierten neurootologischen Untersuchungsmethoden sind in ihrer Ausführung nicht standardisiert. Die Auswertung der Ergebnisse ist nicht statistisch belegt. Neuropsychologische Testuntersuchungen können dazu dienen, erhaltene und beeinträchtigte kognitive Leistungen zu beschreiben, nicht aber, Hirngeschädigte in einer unausgelesenen Population zu identifizieren. Manualmedizinische Befunde sind stark subjektiv gefärbt. Die beschriebenen nuklearmedizinischen Veränderungen werden auch bei Depression ohne vorangegangenes Trauma erhoben. In the past, the biomechanical events and the possible medical sequelae of these accidents were subject to speculation. Recently, investigations in several laboratories with the help of high-speed cameras and of electronic recording devices have demonstrated that the duration of the crash-phase is only about 3/10 s and that the alledged whiplash mechanism does, in fact, not occur. The co-called Late Whipash Syndrome is not characteristic for rear-end or other automobile collisions. The same set of symptoms is observed in various conditions of highly debatable nosological status. The patient's complaints appear to be associated with depression. The problems of cervical vertigo, of neuro-otological studies, of osteopathic findings, the role of SPECT and of MRT of the atlantoaxial joints and alar ligaments in the study of these cases are discussed. There exists a framework of rules on a national as well as international basis for the medico-legal assessment of these patients.
