I read with great interest the fascinating case report by Gerstein et al, 1 which potentially could be used worldwide in treating refractory ventricular fibrillation. It is worth adding to one's arsenal of treatments should one encounter resistant ventricular fibrillation. As a result, I wonder if the authors would comment on the optimum time to resort to this treatment modality, any special precautions in its use, and whether they foresee any role for its use in the management of arrhythmias.
