Genital cancers in women play an important role in cancer pathology in the developing countries. Objectives: This study aimed at clarifying the epidemiological and histopathological aspects of woman genital cancers in Côte d'Ivoire. Materials and Methods: This was a retrospective and descriptive study conducted in the anatomical pathology laboratories of the university hospitals in Abidjan. The study was carried out over a period of twenty-four years (1984-2007). The variables studied were: frequency, age, histological type and prognosis. Results: Genital cancers in women accounted for 41.28% (n = 2491) of cancer in women and 21% of cancers. The average age of patients was 46.65 years (range 2-88 years). The cervix (82.85%) was the main location followed by the ovary (8.6%). Histologically, carcinomas (92.93%) were the most common types of sarcomas. Cervical cancers were diagnosed at an average age of 47.36 years (range 2 to 88 years). Carcinomas were the predominant histological type (92.88%) with 57.4% (n = 450) diagnosed at pT2N0M0 stage. The average age of patients bearing ovarian cancer was 39.13 years (range 8 to 82 years). Common malignant epithelial tumours were the most common histological type (57.48%) (n = 123) followed by non-Hodgkin lymphoma (14.95%). Conclusion: Genital cancers in women are common and poorly prognosed in Côte d'Ivoire. The adoption of a policy of routine screening is needed to improve the prognosis of these tumours.
Les cancers digestifs sont fréquents dans le monde. En Afrique, les données sont sporadiques et caractérisées par une fréquence élevée chez les sujets jeunes. But : Décrire les aspects épidémiologiques et histopathologiques de ces cancers en Côte-d'Ivoire. Matériel et méthodes : Il s'agit d'une étude rétrospective et descriptive portant sur les cancers digestifs primitifs histologiquement confirmés et colligés à partir des registres d'anatomie pathologique des CHU d'Abidjan sur une période de 24 ans (janvier 1984 à décembre 2007). Les paramètres étu-diés étaient : la fréquence, l'âge, le sexe, les facteurs prédis-posant et les aspects anatomopathologiques. Résultats : Nous avons recensé sur la période d'étude 11 854 cancers dont 1 620 cancers digestifs, soit une fréquence de 13,67 % des cancers. L'âge moyen était de 48,65 ans, avec des extrêmes de 2 et 107 ans. Le sexe masculin prédominait, avec un sex-ratio de 2,4. Les facteurs de risque étaient observés dans 17 % des cancers gastriques et 9 % des cancers colorectaux. Les localisations les plus fréquentes étaient : gastrique avec 44,57 % ; colorectale avec 24,44 % et hépa-tique avec 14,69 %. Les adénocarcinomes étaient fréquents au niveau de l'estomac, du côlon et du rectum. Les carcinomes épidermoïdes prédominaient au niveau de l'oesophage et du canal anal. Les lymphomes s'observaient au niveau de l'intestin grêle. Au niveau hépatique, on notait une forte prédominance des hépatocarcinomes, et l'association avec les hépatopathies chroniques était importante 81 %.Conclusion : Les cancers digestifs sont fréquents en Côte-d'Ivoire et touchent les sujets adultes jeunes. Le pronostic est sombre malgré les moyens de diagnostic de plus en plus modernes. La mise en place d'une politique sanitaire associant le dépistage des conditions précancéreuses et la prise en charge des facteurs étiopathogéniques reste la condition sine qua non à l'amélioration du pronostic dans notre pays. Notre travail est une étude rétrospective et descriptive réalisée dans les centres hospitaliers universitaires (CHU) de Cocody et de Treichville, deux structures publiques de la Côte-d'Ivoire dotées de laboratoires d'anatomie pathologique sur quatre que compte la Côte-d'Ivoire. La période d'étude était de 24 ans (1 er janvier 1984 au 31 décembre 2007). Il avait pour but de préciser les caractéris-tiques épidémiologiques et histopathologiques des cancers en Côte-d'Ivoire. Les résultats de cette étude sont les suivants : les cancers représentaient 8,5 % des prélèvements sur la période d'étude ; le nombre de cas a connu une alternance progressive au fil des années ; la fréquence annuelle était de 494 cas ; la prédominance féminine était observée avec 51,21 % des effectifs et 48,79 % de sujets de sexe masculin ; l'âge moyen de survenue du cancer tout sexe confondu était de 51,62 ans avec des extrêmes de deux mois et de 107 ans ; chez l'homme, l'âge moyen était de 53,95 ans, alors que chez la femme, il était de 49,23 ans ; chez l'homme, les cancers prépondérants étaient cutané (21,35 %), prostatique (16,01 %), gastrique (8,07 %), bronchopulmonaire (4,43 %) et colorectal (4,24 %) ; chez la femme, les cancers du col utérin (36,61 %), du sein (13,69 %), de la peau (11,56 %), des organes hématopoïé-tique et lymphatique (7,32 %) et de l'estomac (4,61 %) étaient les plus fréquents. À l'histologie, les carcinomes étaient les plus fréquents (65,57 %), suivis des sarcomes (10 %) et des cancers des organes hématopoïétique et lymphatique (8,09 %) ; chez l'enfant, le lymphome de Burkitt était la tumeur maligne la plus fréquente (33,95 %), suivi du rétinoblastome (10,92 %) et du néphroblastome (5,88 %). The number of cases increased over the years; the annual incidence was 494 cases, in which 51.21% were female and 48.79% were male. The average age at the onset of cancer was 51.62 years (range 2 months to 107 years); for men, it was 53.95 years, while for women, it was 49.23 years. The predominant cancers observed in men were skin (21.35%), prostate (16.01%), gastric (8.07%), lungwort (4.43%) and colorectal (4.24%) cancers; in women, cervical(36.61%), breast (13.69%), skin (11.56%), haematopoietic and lymphatic organs (7.32%) and gastric (4.61%) cancers were the most frequent forms.
