Syndrome of inappropriate antidiuretic hormone
secretion (SIADH) is known as a rare adverse
event with chemotherapy. We report the case of a SIADH
occurring after vinorelbine treatment. Case Report: In a
79-year-old woman breast cancer was first diagnosed in
2000. Three years after the first diagnosis the patient developed
bone and liver metastases. Seven days after receiving
the 1st course of palliative chemotherapy with vinorelbine
the patient suffered from decreased mental
awareness, fatigue, and physical weakness. After the diagnosis
of SIADH based on laboratory findings in combination
with clinical symptoms, we started therapy with
balanced fluid intake and intravenous infusion of normotonic
saline. Conclusion: The development of SIADH as a
rare adverse event with vinorelbine treatment has to be
taken into consideration.
Einführung: Eine Adipositas hat während der Schwangerschaft, unter der Geburt sowie im Wochenbett eine erhebliche klinische Bedeutung. Allgemein ist von einer Risikoerhöhung für die Mutter, den Feten und das Neugeborene auszugehen. Material und Methoden: In einer matched-pairs-Studie wurden je 299 adipöse (BMI > 30 kg/m 2 ) und normalgewichtige Schwangere (BMI 20 -25 kg/m 2 ) verglichen. Dabei waren die Patienten nach Alter und Parität gematched. Zur Verifizierung der Ergebnisse verglichen wir die Daten mit der Sächsischen Perinatal-und Neonatalerhebung (SPN) 2000. Ergebnisse: Bei den adipösen Schwangeren fanden wir signifikant häufiger (p < 0,05) Komplikationen wie schwangerschaftsinduzierte Hypertonie (SIH), Gestationsdiabetes, Frühgeburtlichkeit, Oligo-/ Polyhydramnion und eine pathologische Gewichtsentwicklung. Die Adipösen wurden signifikant häufiger per Sectio caesarea und nach Geburtseinleitung entbunden, wobei die Sectiorate mit steigendem BMI zunahm. Vergleicht man unser Kollektiv der Adipösen mit der SPN 2000, so kann festgestellt werden, dass in Schwangerschaften mit einem BMI > 30 kg/m 2 deutlich mehr Erkrankungen (SIH, Gestationsdiabetes) und Komplikationen (Frühgeburten, Sectiones) auftraten. Wird jedoch eine intensive Betreuung der adipösen Schwangeren bezüglich Diagnostik und Therapie umgesetzt, so ist das Fetal Outcome nicht schlechter als bei normalgewichtigen Schwangeren. Schlussfolgerungen: Das Augenmerk sollte vor allem auf einer Gewichtsreduzierung vor einer geplanten Schwangerschaft liegen, denn die Erhöhung der Komplikationen korreliert eher mit dem Gewicht vor der Schwangerschaft als mit einer übermäßigen Gewichtszunahme während der Schwangerschaft.
