Approximately 30% of patients with rheumatoid arthritis receiving biological disease-modifying antirheumatic drugs (bDMARDs) take them as monotherapy. Although etanercept (ETN) monotherapy has been evaluated in clinical trials, data in the real-world setting are sparse. We compared the efficacy and safety of ETN, given alone or in combination with methotrexate (MTX), in routine clinical practice. This was a subanalysis of patients who received either ETN alone or ETN + MTX during a 52-week prospective, observational study conducted at 329 German centers. The primary endpoint was "Funktionsfragebogen Hannover" (Hannover Functional Ability Questionnaire [FFbH]; low FFbH = worse function) functional remission at week 26 and week 52. Secondary endpoints included the 28-joint count Disease Activity Score (DAS28), DAS28 remission (DAS28 < 2.6), and adverse events (AEs). Participating centers applied ETN monotherapy in 43.1% of patients and ETN + MTX in 56.9%. A smaller proportion of patients achieved FFbH functional remission with ETN vs ETN + MTX (31.9%, 95% confidence interval [CI] 29.1-34.9% vs 39.8%, 37.2-42.5%, respectively; p < 0.001) at 26 weeks and at 52 weeks (38.4%, 35.1-41.7% vs 44.3%, 41.5-47.2%, respectively; p = 0.007). After 52 weeks, the mean DAS28 (±SD) decreased from 5.5 ± 1.3 to 3.4 ± 1.4 (ETN) vs 5.3 ± 1.3 to 3.2 ± 1.3 (ETN + MTX) and DAS28 remission was achieved by 32.5% (95% CI 29.0-36.1%) of patients with ETN vs 38.8% (35.8-41.9%; p = 0.007) with ETN + MTX. Overall, 20.6 (ETN) and 19.7% (ETN + MTX) of patients reported treatment-related AEs. Patients received ETN monotherapy almost as often as ETN + MTX. ETN + MTX appeared marginally more effective than ETN monotherapy in some, but not all, outcomes measured.
Using "paper patients" we compared the therapeutic approaches of general practitioners and rheumatologists to rheumatoid arthritis and to osteoarthritis of the knee. The mailed survey contained a mild, a moderate and a severe case of rheumatoid arthritis (RA) and a case of osteoarthritis (OA) of the knee. 111 out of 252 general practitioners and 78 out of 132 rheumatologists selected at random participated in the study. We found that rheumatologists would choose more non-steroidal anti-inflammatory drugs (NSAID), disease modifying antirheumatic drugs (DMARD), steroids and physical therapy. In case 4 (OA of the knee) rheumatologists would more frequently recommend analgesics, local steroids, occupational therapy, surgery and ortheses. Primary care physicians on the other hand prescribed more chondroprotective agents. Patients with RA and OA of the knee would be treated differently by primary care physicians and rheumatologists. In order to develop a more uniform therapeutical concept and therefore to reach a better management of the rheumatic patient, a close co-operation of the two physician groups is needed.
Einleitung und Fragestellung: Seit den 50er Jahren werden Veränderungen der Haut und des Subkutangewebes in Zusammenhang mit inneren Organerkrankungen gebracht (sogenannte bindegewebige Zonen). Sie sind Grundlage verschiedener physikalischer Behandlungen und weiterer, reflexiv orientierter Therapie. Die bisherigen Beobachtungen sind empirischer Natur, die Lokalisation und die Qualität solcher «Zonen» sollte an einem gut standardisierten Krankheitsbild überprüft werden. Als nosologische Entität wurde die sogenannte Pillenhepatopathie ausgewählt (mögliche Veränderungen im Leber-Galle-System bei antiovulatorischer Medikation). Methoden: In einer untersucherblinden Querschnittsstudie werden 100 junge Frauen auf die Existenz und auf den eventuellen Ausprägungsgrad bindegewebiger’ Veränderungen untersucht. An empirisch festgelegten Regionen des Thorax wird untersucht, wie gut eine Hautfalte (Cutis und Subcutis) von der Körperfaszie abge-hoben werden kann (sogenannte Verhaftung), es wird die Konsistenz der jeweils ab-gehobenen Gewebe geschätzt (sogenannte Induration), und es wird die Schmerzhaftigkeit bei mechanischer Manipulation (Kneifschmerz) beurteilt. Für jede dieser 3 Qualitäten werden Scores (1-6 Punkte) geschätzt. In der späteren Auswertung werden für jeden Patienten die individuellen Differenzen zwischen den Befunden an der rechten und an der linken Körperhälfte gebildet. Erst nach einem verbindlichen Protokoll der erhobenen Gewebebefunde werden die Probandinnen nach ihrer für das Leber-Galle-System relevanten Anamnese und Beschwerden, nach ihrem Medi-kamenten- und Alkoholkonsum und nach der Einnahme oraler, hormoneller Ovulationshemmer gefragt. Ergebnisse: Bei 44 Probandinnen mit antiovulatorischer Medikation seit mindestens einem Monat (Gruppe I; Median = 21, Minimum = 1, Maximum = 75) sind solche bindegewebigen Qualitäten signifikant starker ausgeprägt als bei 33 Probandinnen (Gruppe III) mit bislang nicht stattgehabter hormoneller Antikonzeption. 23 Probandinnen mit seit mindestens einem Monat abgesetzter Antikonzeption (Gruppe II; 18, 3, 84 Monate) liegen im Ausprägungsgrad ‘bindegewebiger’ Veränderungen überwiegend zwischen den bisher genannten Gruppen. In bezug auf die medizinische Vorgeschichte, aktuelle, eventuell auf die Leber zu beziehende Beschwerden, Medikamenteneinnahme und Alkoholkonsum unterscheiden sich die 3 Gruppen nicht. Als die zuverlässigsten Körperregionen mit der grössten Trennschärfe (Gruppen I und III) erweisen sich der Bereich des Brustkorbrandes in Höhe von Th 8-9 paravertebral, der Bereich des Angulus inferior scapulae in Höhe von Th 6 und der Bereich zwischen dem inneren oberen Schulterblattwinkel und der Wirbelsäule in Höhe des Uberganges C 4/Th 2. Von den untersuchten Gewebequalitäten (s. oben) trennt am besten das Phänomen «Schmerzhaftigkeit». Weitere Dosis-Wirkungs-Beziehungen bestehen in der Gruppe I für die Dauer der Hormoneinnahme; für die Dosis gestagener und östrogener Äquivalenzen pro Monatszyklus können entsprechende Beziehungen nicht hergestellt werden. Our objective was to compare the therapeutic approaches of German and Turkish physicians to rheumatoid arthritis (RA) and to osteoarthritis (OA) of the knee, by means of a mailed survey. The survey contained four case histories representing a mild, a moderate and a severe case of RA and a case of OA of the knee. One hundred and thirty-two physicians from Germany (internal medicine based (IR) and orthopaedics based (OR) rheumatologists) and thirty-three from Turkey (rheumatologists and physical medicine and rehabilitation specialists (PT)) participated in the study. German respondents would give more disease-modifying drugs (DMARD) in early RA (48.7% vs 18.2%, p < 0.05), whereas their Turkish colleagues would prescribe more analgesics, ultrasound and kryotherapy in OA of the knee (63.6% vs 22.1%, 30.3% vs 6.5% and 24.2% vs 0.0% respectively p < 0.05). German physicians chose more exercise, physical and occupational therapy, radiation synovectomy and surgery in all cases. In OA of the knee German OR's would recommend less analgesics, but more local steroids, chondroprotective agents and surgery than the other groups. We may conclude that clinical practice of RA and OA of the knee differs considerably in Germany and Turkey. Cultural, social, educational and economic factors could influence the decisions of the physicians.
