A 30-year-old gravida 2 para 1 was admitted to hospital 2 years after cesarean section at 20 weeks’ gestation with acute onset of abdominal pain and hypovolaemic shock. Emergency laparotomy revealed a uterine rupture located in the anterior uterine wall caused by a placenta percreta and supracervical hysterectomy was performed. This site of invasion and finally rupture was in projection of the previous lower-segment cesarean section. This report illustrates the dramatic consequences of abnormal placentation after prior uterine surgery, which can already occur early during pregnancy and prior to the onset of labour.

Seit den 60er Jahren werden Therapien zur Ovulationsinduktion bei der Frau zunehmend angewandt. Eine dramatische Ausweitung an Behandlungszyklen und Indikationen hat die Sterilitätstherapie seit der Geburt des ersten Kindes mithilfe der In-vitro-Fertilisation erfahren. Ein immenser technischer Fortschritt ermöglichte die Entwicklung zusätzlicher Techniken wie die intrazytoplasmatische Spermieninjektion (ICSI) oder die Kryokonservierung von Pronukleusstadien. Gleichzeitig wurden die eingesetzten Medikamente und Behandlungsprotokolle ergänzt und modifiziert. Maûgeblich sind dabei die Entwicklung von GnRH-Analoga, zuletzt von GnRH-Antagonisten, sowie die Herstellung rekombinanter Gonadotropine. Damit wurde nicht nur die Effizienz der Therapie erhöht, sondern auch Risiken und Nebenwirkungen der Ovulationsinduktion vermindert. Die medikamentösen Verfahren der Ovulationsinduktion, sowohl als reine Therapie als auch zur assistierten Reproduktion, werden ständig optimiert. Zusätzlich finden auch neuartige medikamentöse Behandlungsansätze wie Insulinsensitizer oder Aromataseinhibitoren Eingang in bestehende Therapiekonzepte. Therapieansätze bei endokrin-ovarieller Dysfunktion Eine der häufigsten Sterilitätsursachen ist die endokrin-ovarielle Dysfunktion der Frau. Nach initialer Basisdiagnostik, die bei der Primärkonsultation neben der Hormonanalyse und dem Zyklusmonitoring auch die Initiierung der andrologischen Diagnostik umfassen sollte, steht diese bei ca. 24% der Paare im Vordergrund. In speziellen Kinderwunschzentren sinkt der Anteil auf ca. 18%, da Störungen der Ovulation meist gut therapierbar sind [1, 2]. Neben der zentralen Regulation über die Hypothalamus-/Hypophysenachse hängt die Ovarialfunktion auch von der Funktion anderer peripher-endokriner Organe ab. Insbesondere die Schilddrüsen-und Nebennierenrindenfunktion sollte bei der Basisdiagnostik erfasst werden, um vor Beginn einer Ovulationsinduktion endogene Störfaktoren zu erkennen und ggf. korrigieren zu können. Dazu zählen die Hyper-/Hypothyreose (letztere häufig vergesellschaftet mit einer Hyperprolaktinämie), die Hyperprolaktinämie sowie die adrenale/ovarielle Hyperandrogenämie, die häufig mit einer peripheren Insulinresistenz vergesellschaftet ist. Demzufolge haben sich begleitend oder vor Beginn einer Ovulationsinduktion bestimmte therapeutischen Maûnahmen bewährt. Bei Euthyreose sollte besonders in endemischen Jodmangelgebieten bei Frauen mit Kinderwunsch eine groûzügige Jodidsubstitution nach Ausschluss einer GebFra-Refresher An ungewollter Kinderlosigkeit leiden in Deutschland ca. 20 % der Paare.
