BackgroundBreast cancer is the most common cancer in women worldwide. In the Moroccan context, the role of well-known reproductive factors in breast cancer remains poorly documented. The aim of this study was to explore the relationship between menstrual and reproductive factors and breast cancer risk in Moroccan women in the Fez region.MethodsA case–control study was conducted at the Hassan II University Hospital of Fez between January 2014 and April 2015. A total of 237 cases of breast cancer and 237 age-matched controls were included. Information on sociodemographic characteristics, menstrual and reproductive history, family history of breast cancer, and lifestyle factors was obtained through a structured questionnaire. Conditional logistic regression models were used to estimate odds ratios and 95% confidence intervals for breast cancer by menstrual and reproductive factors adjusted for potential confounders.ResultsEarly menarche (OR = 1.60, 95% CI: 1.08–2.38) and nulliparity (OR = 3.77, 95% CI: 1.98–7.30) were significantly related to an increased risk of breast cancer, whereas an early age at first full-term pregnancy was associated with a decreased risk of breast cancer (OR = 0.41, 95% CI: 0.25–0.65).ConclusionThe results of this study confirm the role of established reproductive factors for breast cancer in Moroccan women. It identified some susceptible groups at high risk of breast cancer. Preventive interventions and screening should focus on these groups as a priority. These results should be confirmed in a larger, multicenter study.
Rationale: The Global Burden of Disease program identified smoking and ambient and household air pollution as the main drivers of death and disability from chronic obstructive pulmonary disease (COPD).Objectives: To estimate the attributable risk of chronic airflow obstruction (CAO), a quantifiable characteristic of COPD, due to several risk factors.Methods: The Burden of Obstructive Lung Disease study is a crosssectional study of adults, aged $40, in a globally distributed sample of 41 urban and rural sites. Based on data from 28,459 participants, we estimated the prevalence of CAO, defined as a postbronchodilator FEV 1 -to-FVC ratio less than the lower limit of normal, and the relative risks associated with different risk factors. Local relative risks were estimated using a Bayesian hierarchical model borrowing information from across sites.From these relative risks and the prevalence of risk factors, we estimated local population attributable risks. Measurements and Main Results:The mean prevalence of CAO was 11.2% in men and 8.6% in women. The mean population attributable risk for smoking was 5.1% in men and 2.2% in women. The next most influential risk factors were poor education levels, working in a dusty job for $10 years, low body mass index, and a history of tuberculosis. The risk of CAO attributable to the different risk factors varied across sites.Conclusions: Although smoking remains the most important risk factor for CAO, in some areas, poor education, low body mass index, and passive smoking are of greater importance. Dusty occupations and tuberculosis are important risk factors at some sites.
The health survey, the 12-Item Short-Form (SF-12) survey instrument, was developed as a shorter alternative to the SF-36 for use in scale studies. The aim of our study was to adapt this instrument into Moroccan Arabic and to examine its psychometric properties. The SF-12 was translated from English to dialectical Moroccan Arabic following the International Quality of Life Assessment translation procedure. The psychometric properties were tested in September 2007. Testing involved a sample of families and friends of patients (≥ 16 years old) in the diagnostic centre of the Fez University hospital, Morocco. The SF-12 was assessed by examining item-level characteristics, estimates of scale reliability (internal consistency) and construct validity. The study was conducted on 141 subjects. The physical component summary (PCS-12) and the mental health component summary (MCS-12) of the SF-12 demonstrated good internal consistency reliability, with alpha coefficients of 0.80 and 0.79, respectively. Multitrait analysis showed that the subscales of the SF-12 had good convergent and discriminant validity. Construct validity, assessed by the method of extreme groups, determined that the SF-12 summary scores varied for individuals who differed according to age and medical con¬ditions. The Moroccan Arabic version of the SF-12 appears to be a valid tool with which to for assess the health status of the general Moroccan population. On the other hand, issues such as test-retest reliability, longitudinal construct validity and responsiveness were not addressed in this study and should be considered in future ones.Peer reviewed.
Introduction: Epidemiological studies have shown the association between risk of developing cervical cancer and the persistence of high-risk papillomavirus types in addition to some co-factors. However, little is known about co-factors associated with human papillomavirus (HPV) infection, especially in developing countries. This study aims to determine HPV status and associated risk factors in women with normal cytology living in the north-central area of Morocco. Methodology: From February 2007 to December 2008, a total of 925 women consulting in the gynaecological department of Fez University Hospital were asked about sociodemographic characteristics and reproductive and sexual health. Cervical samples were collected for cytological examination and HPV DNA detection. Data collected from 751 women with normal cytology were used in this study to assess the correlation between HPV infection and potential risk factors. Results: High prevalence of HPV infection was detected (42.5%). The highest infection rate was observed in women aged >45 years and in those with history of abortion (OR:3.76; 95%CI[1.77-7.98]) fibroma, polyp or cysts (OR:1.68; 95%CI[1.07-2.65]). No significant association was detected with other reproductive health and risk factors including oral contraception. Conclusion: In spite of the insignificant association of HPV infection with age, health authorities should seriously consider and implement strategies to increase and maintain a cervical cancer screening programme in women aged 45 and above. More attention must be given to women with gynaecological history (abortion, fibroma, polyp or cysts) since these events may be predictors of HPV infection. Investigations on partner sexual behaviour and some specific hygienic habits, especially public Turkish bath use, are needed to clarify the HPV incidence in this region.
