Objective

BackgroundAcupuncture therapy for preventive and treatment of postoperative nausea and vomiting(PONV), a condition which commonly present after anaesthesia and surgery is a subject of growing interest. ObjectiveThis paper included a systematic review and meta-analysis on the effect of different type of acupuncture and acupoint selection in PONV prevention and treatment. MethodsRandomised controlled trials(RCTs) comparing acupuncture with non-acupuncture treatment were identified from databases PubMed, Cochrane, EBSCO, Ovid, CNKI and Wanfangdata. Meta-analysis on eligible studies was performed using fixed-effects model with RevMan 5.2. Results were expressed as RR for dichotomous data, with 95%CI. ResultsThirty RCTs, 1276 patients (intervention) and 1258 patients (control) were identified. Meta-analysis showed that PC6 acupuncture significantly reduced the number of cases of early vomiting (postoperative 0-6h) (RR=0.36, 95%CI 0.19,0.71; P=0.003) and nausea (postoperative 0-24h) (RR=0.25, 95%CI 0.10,0.61; P=0.002), but not early nausea (postoperative 0-6h) (RR=0.64, 95%CI 0.34,1.19; P=0.150) and vomiting (postoperative 0-24h) (RR=0.82, 95%CI 0.48,1.38; P=0.450). PC6 acupressure significantly reduced the number of cases of nausea (RR=0.71, 95%CI 0.57,0.87; P=0.001) and vomiting (RR=0.62, 95%CI 0.49,0.80; P=0.000) at postoperative 0-24h. PC6 electro-acupoint stimulation significantly reduced the number of cases of nausea (RR=0.49, 95%CI 0.38,0.63; P<0.000) and vomiting (RR=0.50, 95%CI 0.36,0.70; P<0.000) at postoperative 0-24h. Stimulation of PC6 with other acupoint(s) significantly reduced the number of cases of nausea and vomiting (RR=0.29, 95%CI 0.17,0.49; P<0.000) at postoperative 0-24h. Stimulation of other acupoint(s)(non PC6) also significantly reduced the number of cases of nausea and vomiting (RR=0.63, 95%CI 0.49,0.81; P=0.000) at postoperative 0-24h. However, the quality of study was generally low in studies of PC6 combined with other acupoint(s) and other acupoint(s). Details of blinding were not reported in most reports. ConclusionsBesides PC6, PC6 combined with other acupoint(s) and other alternative acupoint(s) might be beneficial in prevention and treatment of PONV, the evidence justifies future high-quality studies.

Results: Both groups were similar in gestational age (31.0 ± 5.9 for the control group Vs 31.1 ± 5.12 for women with pregestational diabetes). Both, VI and VFI were significantly lower in the pregestational diabetes group (2.24 ± 1.44 and 0.85 ± 0.64 for control group Vs 1.51 ± 0.75 and 0.55 ± 0.32) for pregestational diabetes group respectively (p = 0.02 and p = 0.04). FI did not differ significantly between the two studied groups. Conclusions: Fetuses of pregestational diabetic pregnancies showed lower values of some of the 3-D angio Doppler indexes. This finding could suggest a deficient fetal vascularitation and/or cerebral angiogenesis. Further studies are needed to evaluate the relationships between these indexes and both the grade of metabolic diabetic control and perinatal outcome. Objectives: To compare the predictive values of three ultrasonographic parameters: placental thickness (PT), fetal cardiothoracic ratio (CTR), and middle cerebral artery peak systolic velocity (MCAPSV), alone or in combination, in pregnancies affected by homozygous α 0-thalassaemia. Methods: At-risk women were studied from 1995 to 2006 using serial ultrasonography at 12 to 15 weeks, 16 to 20 weeks, and 30 weeks' gestation. We measured CTR and PT after 1995, and MCAPSV as well after 1997. An invasive prenatal test would be performed if cardiomegaly with or without placentomegaly were detected. The results of MCAPSV were retrospectively used for analysis. Results: Of a total of 711 at-risk fetuses studied, 152 (21.4%) were affected. At 12 to 15 weeks' gestation, 598 ultrasound examinations were performed. The highest sensitivity (98.3%) was achieved by the combination (CTR or MCAPSV) at a false positive rate (FPR) of 15.1%. At 16 to 20 weeks' gestation, 113 and 297 ultrasound examinations were performed for the first time and follow up respectively. Both CTR and MCAPSV predicted the affected pregnancies equally well as shown by the area under ROC. The sensitivity of CTR was 100.0%, but the FPR was 38.5%. On the other hand, the FPR of MCAPSV alone and the combination (CTR and MCAPSV) was 10% and 0% respectively, although their sensitivities were smaller than 65%. Conclusions: The data suggest that adding MCAPSV to CTR can increase the sensitivity at 12-15 weeks and decrease the FPR at 16-20 weeks' gestation. Objective: Assess the accuracy of MCA-PSV in normal and complicated pregnancies, evaluate the impact of gate placement and angle correction, establish reference ranges and assess their applicability in diagnosing fetal anemia. Methods: In 166 low-risk fetuses MCA-PSV was measured at 2 locations with and without angle correction. Reference ranges were established with linear regression. We compared them with existing ranges. In 34 fetuses at risk for anemia MCA-PSV was measured and values plotted on our curves. If values suggested anemia, FBS/IUT or laborinduction was performed. Robinson model underestimate dating by 7 days at CRL of 2 mm, by about 1.5 days at 10-60 mm and overestimate by 1.5 days at 80 mm. Wisser formula underestimate by 2 days at 2 mm, marginally overestimate at 10-60 mm and underestimate by 1 day at 80 mm. Conclusions: We establish a new dating formula relevant for a wider range of CRL (2-80 mm) and with markedly lower 95% CI and 95% PI compared to conventionally used models.

Results: Mean gestational age at birth was 33.0 ± 1.4 weeks (range 30.3-35) and mean birth weight was 2003 ± 335 gm (range 1262-2880). Fourteen of them were given dexamethasone. The mean velocity of pulmonary artery was 67 cm/sec. The mean acceleration/ejection time ratio of right and left pulmonary arteries were 0.16 ± 0.05 and 0.15 ± 0.05 respectively. After birth, 7 of them needed mechanical ventilation, and 3 needed surfactants. However, we found no significant correlation between prenatal pulmonary blood flow findings and postnatal pulmonary functions. Conclusion: Doppler blood flow of the pulmonary artery in preterm births has no relationship with the postnatal pulmonary functions. HongDepartment of Obstetrics and Gynecology, Seoul National University Bundang Hospital, Seongnam, South Korea Aims: To determine whether decreased cervical length, intraamniotic infection at presentation and prolonged latency are associated with an increased risk of the development of histologic chorioamnionitis after preterm premature rupture of membranes (PPROM). Methods: Transvaginal ultrasonographic assessment of cervical length was performed and antibiotic therapy was initiated immediately after amniocentesis in 81 consecutive women with PPROM at 23.4 to 35.0 weeks of gestation. Amniotic fluid obtained by amniocentesis was cultured for aerobic and anaerobic bacteria and for Mycoplasmas, and WBC count was determined. PPROM was defined as gross rupture and histologic chorioamnionitis was diagnosed in the presence of neutrophil infiltration into any tissues samples (amnion, chorion-decidua, umbilical cord, or chorionic plate). Results: The prevalence of acute histologic chorioamnionitis in patients after PPROM was 41% (33/81) and that of positive amniotic fluid cultures was 22% (18/81). Logistic regression indicated that decreased cervical length, a positive amniotic fluid culture, and gestational age at amniocentesis, but not prolonged latency, bear a significant relationship with the development of histologic chorioamnionitis after PPROM. Similarly, in terms of the development of the funisitis as the outcome variable, logistic regression demonstrated that only gestational age at amniocentesis was significantly and independently associated with an increased risk of development of funisitis after PPROM. Conclusions: We demonstrated the shorter the cervix in patients with PPROM, the greater the risk of ascending transcervical infection after PPROM documented by the increasing odds ratios of placental inflammation.
