Maßgebende Parameter für die Herzfunktion wie Herzfrequenz (HF), Schlagvolumen und Herzzeitvolumen (SV und HZV) sowie die Anspannungszeit (PER) und die Austreibungszeit (LVET) können durch die Aufnahme und die Auswertung des Impedanzkardiogramms (IKG) zusammen mit dem Elektrokardiogramm (EKG) und dem Phonokardiogramm (PKG) nicht invasiv bestimmt werden /1,2/. Zur Erfassung dieser Parameter wurde das mikroprozessorgestützte System -IMPEDANZ ANALYZER -eingesetzt. Das SV wird nach einer von Kubicek vorgeschlagener Formel ermittelt /3/. Stromeinkoppelung und Signalableitung erfolgen mit je zwei handelsüblichen EKG-Klebepunktelektroden /4/. In einer Untersuchung an einem Patientenkolektiv unter klinischen Bedingungen wurden die Herzzeitvolumina mit dem IMPEDANZ ANALYZER bestimmt und mit den durch invasive Methoden gewonnenen Volumina verglichen. Bei den invasiven Methoden handelt es sich um folgenden zwei Gruppen: -angiographische Methode: Ventrikulographische Bestimmung der enddiastolischen und -systolischen Volumina und -HZV Bestimmung nach Fickschem Prinzip aus der arteriovenösen Sauerstoffdifferenz oder durch Thermodilution. Bei linearer Korrelation ergeben sich Regressionskoeffizienten von R1 = 0.60 und R2 = 0.92 sowie ein Korrelationskoeffizient von K = 0.74,. Die untersuchten kardiologischen Patienten litten über-wiegend an einer Erkrankung der Herzkranzgefäße, einige hatten Kardiomyopathien, bei einer kleinen Patientengruppe erfolgte die Untersuchung zum Ausschluß von Herzerkrankungen. Patienten mit Klappenfehlfunktionen und Links-Schenkelblock wurden von der Auswerung ausgeschlossen, da aufgrund der pathologischen Strömmungsverhältnisse ein falsches SV aus dem IKG bestimmt wird. In einer Patientengruppe wurde das HZV mit dem IMPEDANZ ANALYZER für jeweils 15 Minuten während körperlicher Ruhe gemessen. Daraus wurde der Mittelwert des HZV über diesem Zeitraum bestimmt. Die Messungen erfolgten zwei bis fünf Stunden nach einer angiographischen Untersuchung. In Abb. 1 sind auf der Ordinate die ventrikulogrnphisch und auf der Abszisse die impedanzkardiographisch ermittelten 'HZV von zwanzig Patienten aufgetragen. Der Korrelationskoeffizient, bei linearer Korrelation, betrögt k=0.74. In einer zweiten Gruppe wurde während der Katheterisicrunq das IKG aufgenommen und ausgewertet. Die Untersuchung erfolgte in Ruhe und unter ergometrischcr Belastung.
