Optinio neuromielito spektro sutrikimas (angl. Neuromyelitis optica spectrum disorder, NMOSD) yra reta uždegiminė demielinizuojanti centrinės nervų sistemos (CNS) liga, pažeidžianti regos nervus ir nugaros smegenis. Pagrindinis NMOSD patogenetinis mechanizmas yra akvaporino-4 (AQP4), integralaus astrocitų plazminės membranos baltymo, ir antikūnų prieš akvaporiną-4 (AQP4-IgG) sąveika. Sergamumas NMOSD svyruoja nuo 0,039 iki 0,73/100 000 suaugusių gyventojų ir yra didžiausias Afrikoje bei Azijoje. Dažniausiai liga diagnozuojama 32-45 m. amžiuje, moterys serga dažniau. Kliniškai liga gali pasireikšti labai įvairiai. Tai priklauso nuo pažeistų CNS struktūrų. Dažniausiai pažeidžiamos vietos, kuriose AQP4 ekspresija yra gausiausia: nugaros smegenys, regos nervas, dorsalinė pailgųjų smegenų dalis, smegenų kamienas, gumburas ir pogumburis. Liga prasideda optinio neurito arba skersinio mielito simptomais, kiek rečiau – šių ligų deriniu. NMOSD diagnostika remiasi klinikiniais požymiais, magnetinio rezonanso tomografijos radiniais ir AQP4-IgG antikūnų nustatymu kraujo serume. Pradinis ūmaus NMOSD gydymas yra didelės metilprednizolono dozės, t. y. 1 gramas intraveniškai, tęsiamas nuo 3 iki 5 dienų. Recidyvų prevencijai išskiriami trys pirmo pasirinkimo vaistai: azatioprinas, mikofenolato mofetilis ir rituksimabas. Naujausiuose randomizuotuose klinikiniuose tyrimuose įrodytas ir kitų monokloninių antikūnų, tokių kaip inebilizumabas, satralizumabas, tocilizumabas ir ekulizumabas, skyrimo palaikomajam NMOSD gydymui efektyvumas ir saugumas. Veiksniai, lemiantys blogesnę ligos prognozę, yra pirmo priepuolio sunkumas, didesnis recidyvų skaičius per pirmuosius dvejus metus, vyresnis amžius ligos pradžioje, sąsajos su kitais autoimuniniais sutrikimais. Mirštamumas per pirmuosius dešimt metų po ligos diagnozavimo siekia 20-25 %. COVID-19 pandemijos metu iškyla daug iššūkių, susijusių su NMOSD pacientų ligos recidyvais, padidėjusia infekcijos rizika ir gydymo taktikos parinkimu šiuo kritiniu laikotarpiu.
Dermatochalazė yra vokų odos perteklius, susiformuojantis dėl odos išsitampymo, sumažėjus jos elastingumui. Pagrindinis akių vokų odos užkritimą sukeliantis veiksnys yra su amžiumi susijęs odos senėjimas, tačiau mokslinėje literatūroje išskiriami ir kiti rizikos veiksniai, tokie kaip vyriškoji lytis, šviesesnė odos spalva, didelis kūno masės indeksas (KMI) bei rūkymas. Blefaroplastikos metu pašalinamos perteklinės vokų odos raukšlės, riebalai ir, jei reikia, gilesni audiniai. Viršutiniųjų vokų blefaroplastika yra viena iš dažniausių veido plastinių operacijų, kuri gali būti atliekama dėl funkcinių ar estetinių indikacijų. Komplikacijos po šios chirurginės procedūros stebimos retai, daugumos jų išvengti padeda kruopštus operacijos planavimas bei tiksli metodika. Tyrimo tikslas − pristatyti klinikinį atvejį ir, remiantis naujausiais mokslinės literatūros šaltiniais, apžvelgti viršutiniųjų vokų dermatochalazės blefaroplastikos reikšmę objektyviems ir subjektyviems regėjimo funkcijų pokyčiams, gyvenimo kokybei ir bendrą pacientų pasitenkinimą šios operacijos rezultatais. Pristatomas klinikinis atvejis, kuriame 57 metų moteriai buvo atlikta blefaroplastika dėl abiejų viršutiniųjų vokų odos, dengiančios išorinius vokų plyšio kraštus bei krentančios ant blakstienų, pertekliaus. Pacientė skundėsi akipločio susiaurėjimu, viršutiniųjų vokų sunkumo jausmu, odos dirginimu bei pasunkėjusia žiūrėjimo pozicija. Viršutiniųjų vokų plastikos metu pašalinus odos perteklių, pagerėja pacientų regėjimo funkcija dėl viršutinės akipločio dalies defekto sumažėjimo, kontrastinio jautrumo padidėjimo, astigmatizmo ir aukštesnio lygio aberacijų sumažėjimo. Subjektyvūs pacientų išsakomi simptomai, tokie kaip akipločio sumažėjimas, galvos atlošimas siekiant geriau matyti, viršutiniųjų vokų sunkumo jausmas, galvos skausmai, viršutiniųjų vokų odos dirginimas po jų plastikos pasireiškia rečiau, nei prieš operaciją. Po viršutiniųjų vokų plastikos pagerėja pacientų gyvenimo kokybė. Bendrasis pacientų pasitenkinimas operacijos rezultatais yra labai didelis.
