The prevalence of benzodiazepine therapy for older people in Ontario has steadily declined between 1993 and 1998. There is a trend of dispensing relatively more short-acting than long-acting benzodiazepines and of replacing benzodiazepines with antidepressants in older people without a remarkable increase in barbiturate consumption. These findings suggest that, without undue regulation, physicians are making progress in the prescribing of benzodiazepine therapy on the basis of current knowledge available.
SUMMARY
The age and frequency distributions of whole‐blood 5‐hydroxyindole (5‐hi) levels in normal subjects, and in patients with Down's syndrome, phenylketonuria and severe mental retardation, are presented. The average blood 5‐hi level was depressed in phenylketonuria and Down's syndrome and elevated in the severely retarded. The csf 5‐hi levels were within or above normal limits in Down's syndrome and the severely retarded. By contrast, in phenylketonuria, the csf HI levels were markedly depressed. This new finding once again raises the possibility that serotonin deficiency is important in the pathogenesis of mental retardation in phenylketonuria.

Taux de 5–hydroxyindole dans le sang et le LCR dans le mongolisme, la phénylcétonurie et l'arriération mentale
L'âge et les frékquences de distribution dans le sang total des taux de 5–hydroxyindole (5hi) sont indiquks: (a) chez 138 sujets normaux; (b) chez 68 mongoliens; (c) chez 72 malades avec phénylcétonurie non traitée et (d) chez 103 malades avec retard mental grave. Il a été observé que les tam sanguins de 5hi diminuent avec 1'âge durant les trois premières décades et que les fréquences de distribution sont nettement modifiées. Le taux sanguin moyen de 5hi est diminué dans la phénylcétonurie e tle mongolisme, élevé dans l'arriération mentale. La fréquence de distribution des taux sanguins de 5hi chez les arrétrés présentait une importante dispersion et le taux moyen n'était pas liéà l'âge. Dans le mongolisme on observait une chute de taux sanguin des 5hi avec 1'âge.
Les taux dans le lcr de 5hi, dans le mongolisme et l'arriération mentale étaient au‐dessous ou dans les limites normales, sans relation avec les taux sanguins. En contraste, les taux sanguins et dans le lcr des 5hiétaient diminués dans la phénylcétonurie.
Il est suggéré que le taux sanguin des 5hi n'est pas une indication nette de métabolisme cérébral de la séotonine dans le mongolisme ou I'arriération mentale mais que dans la phénylcétonurie, les taux abaissés de 5hi dans le sang et le lcr indiquaient une diminution générale de la formation de sérotonine. éeci pose de nouveau la question de savoir si la déficience en sérotonine est importante dans la pathoéhie du retard mental au cours de la phénylcétonurie.
ZUSAMMENFASSUNG
5‐Hydroxyindolspiegel im Blut und im Liquor bei Down Syndrom, Phenylketonurie und schwerer geistiger Retardierung
Die Blutspiegel von 5‐Hydroxyindol (5‐hi) wurden auf Alters‐ und Frequenzverteilungen bei folgenden Gruppen untersucht: (a) 138 Normalpersonen; (b) 68 Patienten mit Down Syndrom; (c) 72 Patienten mit unbehandelter Phenylketonurie; und (d) 103 Patienten mit schwerer geistiger Retardierung. Es fand sich, dalß bei Normalpersonen der 5hi‐Spiegel im Blut mit zunehmendem Alter während der ersten drei Dekaden abnimmt und daß die Frequenzverteilungskurve eine positive Abweichung zeigt.
