Introducción. Dentro de las infecciones bacterianas que afectan al ser humano, la infección de las vías urinarias es una de las más frecuentes, afectando hasta 150 millones de personas en el mundo cada año. El uso indiscriminado de antibióticos y su manejo inadecuado han generado un aumento en la resistencia antimicrobiana. El objetivo del presente estudio es describir patrones fenotípicos de resistencia antimicrobiana de los microorganismos más frecuentes en pacientes con diagnóstico de infección de vías urinarias en centro de referencia de Bucaramanga. Metodología. Estudio de corte transversal, muestreo no probabilístico en pacientes con sospecha o diagnóstico de infecciones de vías urinarias que requirieron manejo intrahospitalario en una institución de salud de alta complejidad durante julio del 2017 y abril del 2018, de quienes se obtuvo urocultivo y antibiograma. Resultados. De 120 pacientes incluidos, se obtuvo un reporte de 116 urocultivos. El microorganismo más frecuente fue Escherichia coli, seguido de Klebsiella pneumoniae. El primero presentó alta sensibilidad a carbapenémicos y aminoglucósidos, baja sensibilidad a la Ceftriaxona y a la Ampicilina/Sulbactam. Por otro lado, el microorganismo Klebsiella pneumoniae presentó alta sensibilidad a carbapenémicos, pero resistencia elevada a la Ampicilina/Sulbactam y a la Ceftriaxona. El antibiótico empírico más utilizado fue la Ceftriaxona. Discusión. La distribución de microorganismos aislados es comparable con la ya descrita a nivel nacional e internacional. Los perfiles de resistencia antimicrobiana tienen puntos comunes como la resistencia a los betalactámicos, pero difieren en algunos aspectos, como la resistencia a cefalosporinas y quinolonas. Conclusiones. Los microorganismos aislados presentan altas tasas de resistencia a los betalactámicos y a las cefalosporinas, las cuales son ampliamente utilizadas. Esta información debe orientar el desarrollo de protocolos que permitan optimizar la toma de decisiones clínicas con respecto a la terapia antibiótica.
Introducción. La sepsis se define como una disfunción multiorgánica secundaria a un proceso infeccioso que puede progresar a choque séptico con aumento en el riesgo de mortalidad. Recientes actualizaciones de Sepsis nos permiten realizar un diagnóstico precoz y abordaje terapéutico oportuno con disminución de la morbimortalidad a corto y mediano plazo. División de los temas tratados. Se realizó una revisión bibliográfica no sistemática en bases de datos y bibliotecas electrónicas (PubMed, Cochrane, Lilacs) que incluyó artículos en inglés y español desde 2005 hasta 2018 que contuvieran los tópicos de interés. Se realizó una descripción detallada de la definición de sepsis y choque séptico, clasificación, fisiopatología, diagnóstico, monitorización hemodinámica y tratamiento. Conclusiones. La sepsis es una urgencia médica con una alta tasa de mortalidad que requiere de un diagnóstico oportuno, el cual se puede realizar por medio del puntaje del SOFA (Sepsis Related Organ Failure Assessment), y el inicio rápido de tratamiento antibiótico empírico y reanimación guiada por metas, ya que son medidas que han demostrado disminuir los desenlaces fatales. La fibrilación auricular es la arritmia más frecuente en la prácticaclínica, cuya incidencia viene en ascenso alcanzando el 8 % en edades que superanlos 80 años, con un impacto importante en la morbimortalidad relacionado conlas complicaciones cerebrovasculares. El objetivo es identificar los aspectosfarmacológicos y de manejo práctico de los nuevos anticoagulantes orales enpaciente con fibrilación auricular no valvular. División de los temas tratados. Se realizó una revisión bibliográfica no sistemáticaen bases de datos y bibliotecas electrónicas (PubMed, Cochrane, Lilacs) incluyendoartículos desde 2008 hasta 2019, en idioma inglés y español que contuvieran lostópicos de interés. Se realizó una descripción detallada de las generalidades de losanticoagulantes orales en fibrilación auricular, incluyendo dabigatrán, rivaroxabán,apixabán y edoxabán; monitorización de la actividad anticoagulante; descripcióndel manejo perioperatorio de los nuevos anticoagulantes orales y reversión de laanticoagulación para los nuevos anticoagulantes orales.Conclusiones. El uso de nuevos anticoagulantes orales en paciente con fibrilaciónauricular no valvular es una buena alternativa, con facilidad de administración oral,sin aumento significativo del riesgo de sangrado, comparado con warfarina, con laventaja de no requerir monitoría continua con paraclínicos.
Introducción. El dolor es el principal síntoma que se presenta en el 40% de los pacientes oncológicos en tratamiento y en entre el 80% y el 85% de pacientes con la enfermedad avanzada. Dentro de las herramientas farmacológicas, los opioides son una opción con los consecuentes efectos secundarios, momento en el cual los procedimientos intervencionistas adquieren su importancia. El objetivo del artículo es mostrar el impacto sobre el control del dolor y la calidad de vida en paciente con dolor oncológico abdominal sometido a bloqueo celíaco o hipogástrica en un periodo de 3 meses, con el fin de generar conocimiento del tema en el área de la salud. Metodología. Se realizó una serie descriptiva de casos en un período de 3 meses. Se incluyeron 34 pacientes adultos con dolor abdominal de origen oncológico, con propuesta de bloqueo neurolítico de plexos celíacos o hipogástrico superior como método de control del dolor y se realizó un análisis de las variables en el programa estadístico IBM SPSS Versión 19. Resultados. Se encontró alivio del dolor en el 79,4% de los pacientes intervenidos al poco tiempo del procedimiento y de 33,3% a los 3 meses. No se encontró significante mejoría en la calidad de vida evaluada con el cuestionario SF-36. Conclusiones. Se encontró en este estudio que pacientes con índice de Karnofsky <50 tuvieron alta tasa de mortalidad posterior al bloqueo. La técnica de fenolización más radiofrecuencia podrían tener mejor respuesta terapéutica. Son necesarios más estudios para evaluar posibles asociaciones.
