Background We aimed to identify and describe demand-side factors that have been used to support ATLS global promulgation, as well as current gaps in demand-side incentives. Methods We performed a cross-sectional survey about demand-side factors that influence the uptake and promulgation of ATLS and other trauma-related CME courses. The survey was sent to each of the four global ATLS region chiefs and 80 ATLS country directors. Responses were described and qualitative data were analyzed using a content analysis framework. Results Representatives from 30 countries and each region chief responded to the survey (40% response rate). Twenty of 30 country directors (66%) reported that there were some form of ATLS verification requirements. ATLS completion, not current verification, was often the benchmark. Individual healthcare systems were the most common agency to require ATLS verification (37% of countries) followed by medical/surgical accreditation boards (33%), governments (23%), training programs (27%), and professional societies (17%). Multiple credentialing frameworks were reported including making ATLS verification a requirement for: emergency unit or trauma center designation (40%), contract renewal or promotion (37%); professional licensing (37%); training program graduation (37%); and increases in remuneration (3%). Unique demand-side incentives were reported including expansion of ATLS to nonphysician cadre credentialing and use of subsidies. Conclusion ATLS region chiefs and country directors reported a variety of demand-side incentives that may facilitate the promulgation of ATLS. Actionable steps include: (i) shift incentivization from ATLS course completion to maintenance of verification; (ii) develop an incentive toolkit of best practices to support implementation; and (iii) engage leadership stakeholders to use demand-side incentives to improve the training and capabilities of the providers they oversee to care for the injured.
La instilación intravesical de bacilo de Calmette-Guérin (BCG) es el tratamiento de primera línea para el carcinoma superficial de células transicionales de la vejiga. Aunque su uso es relativamente seguro, complicaciones graves como hepatitis granulomatosa, osteomielitis, neumonía, sepsis y disfunción orgánica múltiple se producen en algunos pacientes. Presentamos el caso de un paciente con diagnósti-co de carcinoma de células de transición superficial de la vejiga (T1), que fallece como consecuencia de una complicación sistémica grave con severo compromiso del sistema nervioso central, secundaria a la instilación intravesical de BCG. Palabras clave: BACILO DE CALMETTE-GUÉRIN IntroducciónLa resección transuretral puede erradicar completamente determinados tumores de vejiga, aunque la incidencia de recurrencia es relativamente elevada (48% a 70%)(1) . La terapia adyuvante intravesical con una solución de bacilo de Calmette-Guérin (BCG) ha demostrado su efectividad y está incluida en guías de práctica clíni-ca (2,3) . La vacuna contra el BCG, una cepa viva atenuada de Mycobacterium bovis, fue introducida por primera vez en humanos en 1921 (4) . La instilación intravesical de BCG fue descrita por primera vez en 1976 por Morales (5) y se utiliza para el tratamiento del carcinoma de células transicionales superficial de la vejiga. El efecto antitumoral parece ser mediado por inmunidad celular dependiente de los linfocitos T; sin embargo, su modo preciso de acción no es completamente conocido (6) . Esta terapia ha demostrado ser eficaz en retrasar la recurrencia después de la resección transuretral, en reducir el riesgo de progresión a la enfermedad invasiva del músculo, en la erradicación de tumores residuales en algunos pacientes con carcinoma papilar y carcinoma in situ y en mejorar la supervivencia (2,7) . OEl 80-90% de los cuerpos extraños ingeridos voluntaria o accidentalmente se evacuan espontáneamente, sin embargo se requiere de una endoscopia terapéutica en un 10-20% de los casos y de cirugía en menos del 1% de los mismos.Objetivo: Analizar las características de los pacientes asistidos por ingesta de cuerpo extraño en el Departamento de Emergencia del Hospital Maciel y llegar al diseño de guías de actuación en el contexto de un equipo multidisciplinario.Resultados: Nuestra serie constó de 31 consultas por ingesta de cuerpo extraño. El 52% provenía de instituciones psiquiátricas y el 42% eran reclusos, un caso no pertenecía a ninguna de las 2 categorías. El 27,5% de las consulta se habían producido en las primeras 24hs desde la ingesta, el 24% de las ingestas se habían producido con más de una semana desde la ingesta y el resto oscilaba entra ambos tiempos evolutivos En cuanto a la conducta, en 12 casos se otorgó el alta sin mediar conducta activa endoscópica y/o quirúrgica. Se realizó una FGC en el 64% de los casos en los cuales se constató radiológicamente la ingesta de un cuerpo extraño, siendo efectivamente terapéuticas para la extracción de objetos a nivel gastroduodenal en el 67% de las mismas. De las 28 consultas por ingesta de CE 6 pacientes (21%) fueron operados. En 5 de ellos por falla de la extracción endoscópica, el restante se presentó como una peritonitis aguda. La ingesta de CE representa una patología de consulta frecuente en el Departamento de Emergencia. El manejo del mismo se realiza mediante un abordaje multidisciplinario que en el caso de nuestra serie mostró porcentajes de extracción endoscópica efectiva cercana al 70% comparable con otras series internacionales. El análisis de las características de la población analizada y el análisis de la evidencia científica publicada nos permite plantear un protocolo de actuación.
