Se define hiperparatiroidismo primario normocalcémico por las concentraciones elevadas de Paratohormona (PTH), calcio sérico normal y ausencia de otras causas de elevación de la PTH, su fisiopatología se debe a una resistencia tisular al efecto de la PTH. El objetivo es presentar una patología infradiagnosticada acerca del diagnóstico y tratamiento. Hombre de 25 años, estudiante universitario, consumo de alcohol regular, consumo de cigarrillo tres al día desde hace 5 años, abuela paterna con artritis reumatoidea, abuela materna con hipertensión arterial, con cuadro de tres años de evolución de ingresos por urgencias por cálculos urinarios, asociado a dolor en cadera, rodilla y cuello de pie derechos, recibió manejo con analgésicos sin mejoría, por hallazgos radiográficos, estudiaron trastorno de la PTH. En cuanto a la analítica, concentraciones elevadas de la PTH, niveles de calcio normales, fósforo sérico bajo, aumento en la concentración de la 1,25-dixidroxi vitamina D e hipercalciuria. Imágenes: ecografía normal, gammagrafía: normal, tomografía axial computarizada normal. resonancia magnética normal, tomografía por emisión de positrones normal. El estudio genético descarto neoplasia endocrina múltiple. Densitometría ósea, reportó osteopenia, requirió abordaje quirúrgico exploratorio: localizaron el adenoma primario en lóbulo paratiroideo izquierdo, realizaron resección, estudio anatomopatológico confirmó el diagnóstico. Posteriormente recibió suplementación con vitamina D por 6 meses. Paciente presentó una adecuada evolución, continúa en seguimiento periódico por endocrinología. El hiperparatiroidismo primario actualmente es una endocrinopatía común, que requiere atención oportuna, porque puede dejar secuelas graves en el organismo u ocasionar la muerte.
Introducción: La Diabetes Mellitus (DM) es una alteración metabólica crónica, creciente en la población pediátrica que conduce a múltiples complicaciones micro y macrovasculares. Es un problema de salud pública por los altos costos que conlleva el diagnóstico y tratamiento. Según la Federación Internacional de Diabetes (IFD), la incidencia anual en el mundo está aumentando, debido a que la prevalencia de edad entre 0-14 años es de 98.2% y de 0-19 años es de 128.9% entre el 2019 y 2021 con una incidencia de 108.3% y 149.5%, respectivamente, por 100.000 habitantes en 215 países. Objetivo: Describir las características clínicas de un grupo de niños y adolescentes que ingresaron al servicio de urgencias del Hospital Regional de la Orinoquía (HORO) de Yopal-Casanare, en el período 2019 -2020 con diagnóstico de novo o previo de diabetes mellitus. Metodología: Se realizó un estudio observacional, descriptivo y retrospectivo, tipo serie de casos, donde se incluyeron pacientes menores de 18 años con diagnóstico de novo o previo de diabetes mellitus en el período de 2019 -2020 en el Hospital Regional de la Orinoquía. Se analizaron un total de 15 pacientes. Resultados: Se muestra la clasificación de los pacientes según grupo de edades y sexo, se encuentra predominio del género masculino con un 53,3% de los 15 pacientes que cumplían con los criterios diagnósticos, mientras que la población femenina fue de 46,6%. La edad promedio de los pacientes pediátricos fue de 13 ± 13.2 Conclusiones: En el estudio realizado en el Hospital regional de la Orinoquía, la edad entre 11 -13 años es la más frecuente para DM, con un predominio significativo del sexo masculino, provenientes de áreas urbanas. Se halló que el bajo peso es un dato alarmante, además de valores de glicemia alterados que se relacionan con las complicaciones presentadas al momento del ingreso del paciente, lo cual representa un problema de salud pública entre niños y adultos jóvenes debido a la heterogeneidad, del factor genético y ambiental.
Introducción: la osteoartritis de rodilla (OR) se define como una enfermedad crónica, inflamatoria, degenerativa y progresiva, caracterizada por el deterioro del cartílago articular. Tiene manifestaciones clínicas y etiológicas diferentes según el grupo etario y se encuentra asociada con múltiples factores que intervienen en la fisiopatología, entre los que están: edad, raza, sexo, sobrepeso, obesidad y síndrome metabólico. Objetivo: determinar los factores de riesgo que influyen en el proceso degenerativo de la OR en la adultez temprana y tardía. Materiales y métodos: revisión sistemática de la literatura científica en diferentes bases de datos que incluyeron estudios de cohorte, transversales, casos y controles, evaluando los factores de riesgo predisponentes. Resultados: se seleccionaron 50 artículos que cumplieron con los criterios, identificando diferentes factores que afectan el desarrollo o progresión de la OR como las alteraciones metabólicas, sistémicas y estilo de vida que influyen en esta patología, además de factores modificables y no modificables. Conclusión: la OR es una afección de carácter multifactorial, donde la obesidad, la hipertensión arterial (HTA), el índice de masa corporal (IMC) elevado, los niveles altos de LDL, la debilidad muscular e incluso la profesión del paciente, son los principales factores predisponentes para desarrollar o aumentar su progresión.
Introducción: el síndrome cardiorrenal se define como la coexistencia de desórdenes entre el corazón y el riñón en la que la alteración de uno causa la disfunción del otro, de forma aguda o crónica. Objetivo: determinar la frecuencia de síndrome cardiorrenal y las características sociodemográficas y clínicas en pacientes hospitalizados con falla cardiaca. Metodología: se realizó un estudio observacional y descriptivo que incluyó pacientes con falla cardiaca que fueron hospitalizados entre el 2018 y el 2021 en un hospital en Boyacá, Colombia. Se incluyeron las variables demográficas, clínicas, diagnósticas y los antecedentes, para la evaluación y el análisis de la información. Resultados: se incluyó a 347 pacientes, donde la frecuencia del síndrome cardiorrenal fue del 10?%, 20,17?% de zona urbana, 16,13?% de sexo masculino, 21,61?% con hipertensión arterial, 9,79?% con diabetes mellitus y 8,64?% con enfermedad pulmonar obstructiva crónica. Las características clínicas fueron: 12,10?% cursaron con insuficiencia cardiaca preservada, 10,08?% con insuficiencia cardiaca reducida y 12,68?% eran del cuadrante de Stevenson estadio C, todos con síndrome cardiorrenal. Conclusiones: el síndrome cardiorrenal es una afectación de origen multicausal, la presentación clínica depende del órgano causante de la disfunción y, típicamente, los pacientes con esta entidad clínica han cursado con alguna descompensación que genera ingresos a hospitalización, cambios en la calidad de vida, peor pronóstico y mortalidad.
Introducción: el dengue es una infección viral, transmitida principalmente por Aedes aegypti, se considera un evento de interés en salud pública y es una de las arbovirosis más importante a nivel mundial en términos de morbimortalidad e impacto económico. Materiales y métodos: se realizó un estudio observacional, descriptivo y retrospectivo. Se incluyó a toda la población con diagnóstico confirmado de dengue atendidos en el Hospital Regional de la Orinoquía ESE entre 2015 y 2020. La base de datos se registró en Excel® versión 2013 y se analizó en el paquete estadístico SPSS versión 22. Resultados: pacientes con diagnóstico confirmado 3395, el 51 % (1735) corresponden a dengue sin signos de alarma, 47 % (1599) a dengue con signos de alarma y el 2 % (61) dengue grave, la edad promedio 20 años, entre nueve (9) meses a 94 años, la letalidad correspondió al 0,2 %. Conclusiones: el dengue afecta a toda la población, continúa siendo una enfermedad emergente y de interés en salud publica en todos los países del mundo. En Colombia es una causa importante de morbimortalidad en todos los grupos etarios, se debe continuar estudiando su comportamiento y seguir reforzando las políticas públicas de prevención.
