Rev Asoc Argent Ortop Traumatol 150 ResumenSe realizó encuesta a 299 residentes de Ortopedia y Traumatología sobre los aspectos del ingreso, las actividades y las preferencias durante ese período formativo de posgrado. Durante el Congreso anual de la especialidad en 2015, se entregó un formulario a los residentes de Residencias acreditadas por la Asociación Argentina de Ortopedia y Traumatología. El 39,3% de las Residencias era de la Provincia de Buenos Aires; el 25%, de la Ciudad Autónoma de Buenos Aires y el 34,88%, del resto de las provincias. El 71,4% eran estatales y el 26,9%, privadas. El 80,9% de las Residencias tenían una duración de 4 años. El 55% de los médicos rindió examen una sola vez. Los motivos de elección fueron que la práctica quirúrgica era la más importante (70%). El 53% se sentía satisfecho con la Residencia, la poca satisfacción obedece a las fallas estructurales (58%), la sobrecarga laboral (47%) y el poco desarrollo académico (41%). Las Residencias estaban completas en el 55% de los casos; las Residencias incompletas se debían a renuncias (56%) y a la falta de postulantes (49%). El 94% de los programas de Residencia tenía incorporado el Curso Bianual de la Asociación Argentina de Ortopedia y Traumatología. Estos datos nos muestran el estado actual de la Residencia en nuestra especialidad, pero son parciales, al no contar con datos de las Residencias no acreditadas. En 2016, se efectuó una nueva encuesta, cuyos resultados están en proceso de análisis.Palabras clave: Ortopedia; Traumatología; Residencia; encuesta. Orthopedic Residency survey regarding several aspects of the program AbstractA survey was performed with 299 orthopedics residents to obtain data about entrance examination, their activities and preferences during this post-graduate period. During the Congreso de Ortopedia y Traumatología de 2015, residents belonging to Residencies approved by Asociación Argentina de Ortopedia y Traumatología received a survey form. Thirtynine percent of Residencies belongs to the province of Buenos Aires, 25% to Ciudad Autónoma de Buenos Aires, and 34.88% to the rest of the country. Private institutions run almost 26.9% of Residencies while 71.4% belongs to the public area. Almost 81% of Residencies have a 4-year-duration. Fifty-five percent of the applicants went through only one examination. Choice was based on the intense surgical practice in the 70%. Objetivo: El objetivo primario fue conocer la actual aplicación de instrumentos de evaluación de competencias quirúrgicas en las residencias acreditadas por la AAOT. Como objetivos secundarios, se describieron los tipos de herramientas utilizadas, el conocimiento de los diferentes instrumentos de evaluación y qué tipo de evaluación se utiliza para la promoción de año. Se analizó si existen diferencias en las características entre las residencias que evalúan las competencias quirúrgicas y las que no. Materiales y Métodos: Se realizó un estudio descriptivo, para lo cual se diseñó una encuesta enfocada en mostrar cuántas residencias evalúan las competencias quirúrgicas con una herramienta formal. Además, a través de ella, se logró responder a los objetivos secundarios. La encuesta se envió a los responsables docentes de las 123 residencias acreditadas por la AAOT. Resultados: Se obtuvieron 105 (85,4%) respuestas, el 59% utiliza algún tipo de herramienta para evaluar las competencias quirúrgicas. Solo el 12,9% de las herramientas utilizadas evalúan las competencias quirúrgicas en forma específica y el resto lo hace con un puntaje general. Un 61% conoce las herramientas disponibles. Para la promoción de año, la mayoría utiliza evaluaciones periódicas múltiples para competencias clínicas y quirúrgicas (63,8% y 67,6%, respectivamente). No hubo diferencias significativas en las características de las residencias que evalúan las competencias quirúrgicas y las que no. Conclusión: El 59% de las residencias implementa algún puntaje o formulario para evaluar las competencias quirúrgicas, solo el 12,9% las evalúa en forma específica, y el resto lo hace con un puntaje subjetivo global.
De manera creciente, la educación y formación de los especialistas, después de haber completado la residencia, se continúan en espacios asistenciales para profundizar el entrenamiento en disciplinas más específicas de la Ortopedia y Traumatología. Este fenómeno se produce también en otras disciplinas, como cirugía general, urología, neurocirugía, entre otras. Si bien la Ortopedia y Traumatología no está considerada como materia básica para estamentos administrativos sanitarios, desde la óptica funcional lo es. Luego de la graduación, en la Argentina, parte de los médicos deciden continuar con una formación de posgrado, al especializarse en alguna rama de la asistencia en salud. En el caso de la Ortopedia y Traumatología, se dedican cuatro años de formación como especialista en sedes acreditadas. Al finalizar y especialmente en zonas urbanas de gran complejidad y desarrollo, necesitan profundizar su preparación en subespecialidades. El desarrollo permanente, el incremento de los conocimientos, la responsabilidad profesional y legal, la demanda de los prestadores de salud y la misma población lo requieren. El crecimiento de las subespecialidades y su desarrollo han llevado a la creación de unidades y hasta servicios de dedicación exclusiva a distintos aspectos de la Ortopedia. Inclusive se han enriquecido con la incorporación de otras disciplinas médicas; son ejemplos la especialidad espinal o de columna vertebral que ha incorporado neurocirujanos y la de mano que se ha nutrido con cirujanos plásticos. En la actualidad, muchos Servicios de Ortopedia y Traumatología, acompañados por el desarrollo de la formación en subespecialidad, han conformado sectores y equipos específicos: Cadera y rodilla, Columna, Miembro Superior y Mano, Tobillo y pie, Ortopedia Infantil, Oncología, Neuroortopedia, Trauma, Deporte, Artroscopia y seguramente vendrán más. En algunas instituciones, determinadas subespecialidades de la Ortopedia ya se han constituido como Servicios. En estos espacios, sectores o equipos es donde concurren los médicos egresados de la Residencia de Traumatología y Ortopedia para proseguir especializando sus conocimientos y competencias. Esta situación, nada nueva, ha sido muy fructífera para el conocimiento y elevación de los estándares de calidad médica tanto asistencial como académica. Con el tiempo se ha observado la necesidad de consensuar algunos aspectos comunes, como ser: la denominación, el tiempo y el programa. Se encuentran variadas coincidencias como también una gran dispersión entre las diferentes ofertas de formación en la post residencia. Es claro que cada una de ellas puede tener distinto tiempo de formación y dedicación, pero a este aspecto, como a otros, se los prefiere ubicar dentro de un marco común que procure aspectos básicos en el que coincidan.
