The experience of symptoms, minor to severe, prompts millions of patients to visit their healthcare providers each year. Symptoms not only create distress, but also disrupt social functioning. The management of symptoms and their resulting outcomes often become the responsibility of the patient and his or her family members. Healthcare providers have difficulty developing symptom management strategies that can be applied across acute and home-care settings because few models of symptom management have been tested empirically. To date, the majority of research on symptoms was directed toward studying a single symptom, such as pain or fatigue, or toward evaluating associated symptoms, such as depression and sleep disturbance. While this approach has advanced our understanding of some symptoms, we offer a generic symptom management model to provide direction for selecting clinical interventions, informing research, and bridging an array of symptoms associated with a variety of diseases and conditions. Finally, a broadly-based symptom management model allows the integration of science from other fields.

Video display terminal (VDT) operators (n = 150) in the editorial department of a large metropolitan newspaper participated in a study of day-to-day musculoskeletal symptoms. Work posture related to the VDT workstation and psychosocial work factors were also investigated for their contributions to the severity of upper body pain, numbness, and stiffness using a representative subsample (n = 70). Self-report measures included Karasek's Job Content Instrument and the author-designed Work Interpersonal Relationships Inventory. Independent observations of work posture were performed using techniques similar to those reported by Sauter et al. [1991]. Pain during the last week was reported by 59% (n = 88) of the respondents, and 28% (n = 42) were categorized by symptom criteria potentially to have musculoskeletal disorders. More hours per day of VDT use and less decision latitude on the job were significant risk factors for potential musculoskeletal CTDs. Head rotation and relative keyboard height were significantly related to more severe pain and stiffness in the shoulders, neck, and upper back. Lower levels of co-worker support were associated with more severe hand and arm numbness. For both the region of the shoulders, neck, and upper back and the hand and arm region, however, the contributions of relative keyboard and seat back heights to symptom severity were modified by psychological workload, decision latitude, and employee relationship with the supervisor. Alternative explanations for these findings are discussed.

Increasing interest has been focused on understanding the role working conditions play in terms of the serious issues facing hospitals today, including quality of patient care, nurse shortages, and financial challenges. One particular working condition that has been the subject of recent research, is the impact of organizational climate on nurses' well-being, including occupational health outcomes. To examine evidence-based research on the association between organizational climate and occupational health outcomes among acute-care registered nurses, a systematic review of published studies was conducted. Studies assessing the association between organizational climate variables and three common health outcomes in nurses (blood/body fluid exposures, musculoskeletal disorders, and burnout) were reviewed. Fourteen studies met the inclusion criteria. Although most were cross-sectional in design and variability was noted across studies with respect to operational definitions and assessment measures, all noted significant associations between specific negative aspects of hospital organizational climate and adverse health impacts in registered nurses. While evidence for an association between organizational climate constructs and nurses' health was found, data were limited and some of the relationships were weak. Additional studies are warranted to clarify the nature of these complex relationships.

Subjects (N = 543) reporting on acute postoperative dental pain were classified into four major ancestral groups: Asian (N = 96), black American (N = 65), European (N = 296), and Latino (N = 88). Pain severity was measured using a 10-cm visual analogue scale following a standardized operative procedure. The subjects of European descent reported significantly less severe pain than those of black American or Latino descent. They also reported less pain than Asians, although this finding did not reach significance. Evaluation of covariates, including gender, age, education, generation in the United States, and difficulty of the surgical extraction, demonstrated that gender was significant, with men reporting less pain than women regardless of ancestry. Possible implications of these findings are discussed in terms of potential differences in physiology, in addition to social learning.

The purpose of this study was to translate the Nursing Stress Scale (NSS) into Chinese and test its reliability and validity among Chinese nurses in Taiwan. Potential participants were asked to self-administer a Chinese version of the NSS. The agreement estimation was used to determine the equivalence of the meaning between the Chinese and original English versions and was rated by five bilingual nurses as 92% accurate for the 34 items. The test-retest reliability for the NSS at 2 weeks was .71 (p = .022, n=10). Internal consistency reliability and factor analysis were tested with 770 nurses from 65 inpatient units at a medical center in Taiwan. The internal consistency of the Chinese version of the NSS for an overall coefficient alpha is .91 for the total scale, and ranges from .67 to .79 for the subscales. The Chinese version of the NSS explains 53.77% of the variance in work stressors among Chinese nurses in Taiwan. Overall, the Chinese version of the NSS is internally consistent but may not be stable over 2 weeks. There was adequate evidence of the reliability and validity of the NSS-Chinese as an instrument appropriate to measure work stress among Chinese nurses. The translated NSS could be a useful tool for examining the frequency and major sources of stress experienced by Chinese nurses in hospital settings, and for the development of appropriate interventions for stress reduction.
