These findings underscore the importance of therapists' interpersonal skills as a predictor of outcome and source of therapist effects. The potential utility of assessing therapists' and therapists-in-training interpersonal skills are discussed.
Zusammenfassung. Fragestellung: Auswahlverfahren besitzen an privaten wie an universitären Einrichtungen zur Ausbildung von Psychotherapeuten eine langjährige Tradition, obwohl für diese Verfahren keine Überprüfung der diagnostischen Validität vorliegt. Das Ziel dieser Untersuchung ist die psychometrische Überprüfung und Validierung einer Beobachtungsskala, die im Rahmen eines Auswahlverfahrens eingesetzt wird und auf die Erfassung der therapierelevanten interpersonalen Kompetenz der Ausbildungsteilnehmer abzielt. Methode: Mithilfe einer Skala von 9 Items wurden n = 82 Auswahlteilnehmer im Rahmen von Gruppensituationen eingeschätzt. Die Skala wurde psychometrisch überprüft und nach 5 Jahren anhand von mehreren Erfolgskriterien bei n = 20 Teilnehmern eines Ausbildungsjahrgangs validiert. Ergebnisse: Die psychometrische Überprüfung ergab eine eindimensionale Struktur der neun Items (77,5% Varianzaufklärung) mit einer internen Konsistenz von α = 0.96. Die gefundenen Zusammenhänge zwischen der Beobachtungsskala und dem eingesetzten Spektrum von Eignungskriterien liegen bei r = 0.5. Inhaltlich zeigte sich, dass sich diejenigen Ausbildungsteilnehmer, die bereits im Auswahlverfahren im Sinne einer spezifischen interpersonalen Kompetenz als weniger geeignet eingeschätzt wurden, sich im Verlaufe der Ausbildung weniger kooperativ in Verwaltungsangelegenheiten zeigten sowie zum Abschluss ihrer Ausbildung weniger reguläre Therapiebeendigungen, eine niedrigere Diagnostikcompliance und einen niedrigeren durchschnittlichen Therapieerfolg aufwiesen. Diskussion: Angesichts des 5-jährigen Prognosezeitraums sind die gefundenen Korrelationen als sehr gut zu bewerten. Die Frage, ob es sich bei der erfassten interpersonalen Kompetenz um spezifische therapierelevante oder um eine allgemeine Interaktionskompetenz handelt, gilt es in zukünftigen Arbeiten zu prüfen.
Zusammenfassung. Theoretischer Hintergrund: Mit dem Berner Inventar zur Erfassung von Therapiezielen (BIT-T) werden mögliche Unterschiede im Umfang und inhaltlichen Schwerpunkt der Zielsetzung kognitiv-behavioraler (KVT) und psychodynamischer Therapien (PDT) untersucht. Fragestellung: Neben der Reliabilität des BIT-T sollen Patienten, KVT- und PDT-Therapeuten hinsichtlich Unterschieden in ihren Therapiezielen untersucht werden. Eine mögliche Vorhersage des Therapieerfolges durch den BIT-T ist von weiterem Interesse. Methode: Die Therapieziele von 472 Patienten und aus 283 ambulanten Therapieanträgen auf KVT und 189 Anträgen auf PDT wurden mit Hilfe des BIT-T kategorisiert. Die Therapieeffektivität wurde mittels des Fragebogens zur Evaluation von Psychotherapieverläufen (FEP) bestimmt. Ergebnisse: Die Beobachterübereinstimmung des BIT-T erweist sich als überwiegend gut bis sehr gut. Bedeutsame Übereinstimmungen zwischen Therapeuten und Patientenzielen können nicht festgestellt werden. KVT- und PDT-Therapien unterscheiden sich in ihren angestrebten Zielen. Problem- und symptombezogene Ziele der Therapeuten sind mit dem Erfolg der Therapie assoziiert. Schlussfolgerungen: Die Beachtung von Therapiezielen erweist sich als bedeutsamer Faktor für Therapiequalität und -erfolg.
Zusammenfassung. Theoretischer Hintergrund: Die Rolle der Affektregulation bei der Aufrechterhaltung bulimischer Essanfälle und kompensatorischer Verhaltensweisen ist geprägt durch ein heterogenes Forschungsfeld. Der Vergleich dieser affektregulatorischer Funktionen zwischen bulimischen Patientinnen und Patientinnen mit einer Binge Eating Disorder (BED) fehlt weitgehend. Fragestellung: Die Studie untersucht die Frage, ob ein Essanfall zu einer Zunahme negativer Affekte führt, und ob diese Zunahme durch das Abführen wieder reduziert wird. Zusätzlich wird untersucht, inwieweit die Zunahme der negativen Affekte für die bulimischen Patienten spezifisch ist. Methode: Die differentielle Affektskala wird in einem problemanalytischen Explorationssetting zu Essanfällen und Episoden des Erbrechens bei 27 stationär behandelten bulimischen Patientinnen und 21 Patientinnen mit BED eingesetzt. Ergebnisse: Die bulimischen Patientinnen berichten im Vergleich zu den BED Patientinnen nach einem Essanfall eine Zunahme von Scham, Schuld, Ekel und Verachtung bei gleichzeitiger Reduktion von Interesse. Die BED Patientinnen berichten nur einen Anstieg von Schuld. Für die bulimischen Patientinnen reduziert das Erbrechen die erlebte Angst und den Ekel und führt außerdem zu einer Zunahme an Freude.
