Introdução: Segurança do paciente é a redução de riscos e danos desnecessários relacionados à assistência e é considerada como um fator de qualidade no cuidado. A presença de falhas nas práticas em saúde propõe a necessidade de discussão sobre temática, ressaltando-se que as principais carências estão relacionadas ao processo de administração de medicamentos e imunobiológicos, podendo também ser encontrado ao longo de todo o atendimento. Sendo assim, dentro da Atenção Primária à Saúde é considerado um serviço de baixa densidade tecnológica, porém é de alta complexidade a partir desse pressuposto é importante que a SP seja implementada, seguindo as normas do Programa Nacional de Segurança do Paciente (PNSP) para uma melhora na assistência, que busca captar o máximo de pacientes. Objetivo: Identificar a aplicabilidade e as barreiras na prática relacionadas à segurança na atenção primária visando a redução de riscos das infecções. Método: Trata-se de uma revisão integrativa, cujo intuito é reunir resultados de pesquisas diversas que refiram-se sobre o mesmo tema, tendo um enfoque também nas conclusões dos estudos apontados. Resultado: Os dados foram obtidos a partir de uma pesquisa realizada baseadas em publicações bibliográficas anexadas em bases de dados no formato eletrônico a partir do formulário de busca da BVS. Conclusão: Diversos fatores interferem na qualidade da assistência: falta de comunicação efetiva, déficit no aprimoramento técnico-científico, sobrecarga de trabalho e carência de padronização dos processos de enfermagem.
