Introduction Although second-line immunotherapy obtained better outcomes than chemotherapy for patients with advanced non-small–cell lung cancers (NSCLCs), it is expensive and only a minority of patients seem to benefit, based on early tumor progression post-immunotherapy. Notable host inflammation, characterized by biomarkers (e.g. neutrophil-to-lymphocyte ratio (NLR])), prolongs overall survival (OS) of surgery-, chemotherapy- and immunotherapy-treated patients. To our knowledge, no previous studies used biomarker evolution to analyze the immunotherapy impact on host inflammation. Immunotherapy mainly exerts its activity by lymphocyte reactivation. Methods This retrospective study was conducted on patients, selected by their progression status just before their 4 th nivolumab injection, and treated at Bordeaux and Limoges University Hospitals. A comparative group of at least 1-year responders was also selected. Clinical parameters and hematological data just before the 1 st (baseline) and 4 th nivolumab infusions were collected to calculate the NLR change (ΔNLR) between those two infusions. The combined impact of the different known prognostic factors was also analyzed with multivariable analyses. Results Fifty-nine patients were included. The 29 early progressors had significantly more frequent ΔNLR > 1 ( p = 0.0007), OR 18.08 [95% CI 2.96–246.24] with progressive disease as best response to prior treatment line ( p = 0.0014). ΔNLR < 1 prolonged OS (HR 0.001 [0.0007–0.18], p = 0.001); as did a partial response to prior line of systemic treatment (HR 0.14 [0.03––0.56], p = 0.005). Conclusion Based on selected early progressors given second-line immunotherapy for advanced NSCLC, progression as best response to prior treatment and ΔNLR > 1 characterized the early progressors and shortened OS after starting nivolumab. This phenomenon questions nivolumab utility in patients with a major host neutrophil inflammation.
Introduction Depuis mars 2020, des mesures extrêmes ont été prises dans le but de contrôler l’épidémie de la Covid-19 et la première vague a eu un impact non négligeable sur la prise en charge de nombreux patients, notamment dans les cancers broncho-pulmonaires. Les pathologies cancéreuses, qui avaient vu ces dernières années leur pronostic s’améliorer grâce à un diagnostic plus précoce et des traitements plus efficaces, ont vu une réduction de 23 % du nombre de cas nouvellement diagnostiqués en 2020 par rapport à 2019 [1] . Nous souhaitons objectiver au sein de l’unité d’oncologie thoracique du CHU de Limoges les répercussions de la Covid19 sur la prise en charge des CBP. Méthodes Dans une population de 450 sujets nouvellement diagnostiqués de CBP dont 250 en 2019 et 200 en 2020, nous avons réalisé à l’UOTC une étude descriptive avec recueil rétrospectif des données. La description des caractéristiques cliniques, des stades histologiques et du taux d’abstinence thérapeutique a été réalisée, ainsi qu’une analyse comparative entre les deux groupes. Résultats Les données de 450 cas de CBP sur les années 2020 et 2019 ont été recueillies, et nous présentons une étude préliminaire sur 101 cas en 2020 et 97 cas en 2019. La répartition d’âge et de sexe semble comparable entre les deux groupes avec un âge moyen au diagnostic de 66,72 ans ± 11,002 en 2020 et de 66,67 ans ± 9 en 2019. Sur les 101 sujets diagnostiqués en 2020, 64,89 % était au stade IV d’emblée, et 17,39 % n’ont jamais été traités, que ce soit pour cause de prise en charge d’emblée palliative ou d’une aggravation rapide ne permettant pas la mise en route de traitement. La survie à 3 mois était de 77,2 %, et la survie à 6 mois de 63,05 %. En 2019, la proportion de CBP au stade IV était à 54,6 %, et 12,37 % n’ont pas pu être traités. Les taux de survie à 3 et à 6 mois étaient respectivement de 80,42 % et 75,26 % ( Tableau 1 ). Conclusion Cette étude préliminaire montre une répartition différente des CBP entre les années 2020 et 2019, notamment sur les stades, alors qu’il ne s’agit que de résultats partiels. Nous avons retrouvé un taux d’abstention thérapeutique plus élevé en 2020 qu’en 2019, et il semblerait que la survie à 3 et à 6 mois soit diminuée en 2020 comparé à 2019, coïncidant avec la première vague la pandémie. L’ensemble des données sera présenté lors du congrès, ainsi que les analyses statistiques.
