Treatment with an SSRI plus supportive care is more effective than supportive care alone for patients with mild to moderate depression, at least for those with symptoms persisting for 8 weeks and an HRDS score of > or = 12. The additional benefit is relatively small, and may be at least in part a placebo effect, but is probably cost-effective at the level used by the National Institute for Health and Clinical Excellence to make judgements about recommending treatments within the National Health Service. However, further research is required.

This study compared resource use data and estimated costs over a 12-month period, based on patient self-report questionnaires and general practitioner records. The level of agreement was measured by weighted kappa. Differences in total costs were plotted against mean total costs. Of 324 patients with complete GP records for the period only 85 (26.2%) had complete self-report data. The recorded number of contacts per patient was higher for patient questionnaires than GP records (17.20 vs. 12.64), and the level of agreement between the two sources was moderate (kappa(w)=0.465). The plots of differences in total costs showed a slight downward bias, suggesting that estimation of total cost from GP records is lower than patient questionnaires. The incompleteness of patient questionnaires forces reliance on GP records as the main source of information for economic evaluations.

The findings support a policy of recommending SSRIs as first-choice antidepressants in primary care.

Sustained recovery from mild to moderate depression in primary care appears more likely for women, people who are married or cohabiting, have few somatic symptoms and receive their preferred treatment.

Background: Depression guidelines in the UK recommended a policy of watchful waiting for mild depression due to a lack of evidence for the effectiveness of antidepressant treatment for mild cases. However there has been relatively little research carried out in primary care to help establish the severity threshold at which antidepressant treatment is effective and cost-effective.
